How Volume Loss Due to Aging Can Accentuate the Eye Socket, and Treatment for this Area - Duration: 8:14.

You submitted your question with a photo.

And you state in your question that you want to know what the fine circle around your eye

socket is called and how do you go about erasing it.

Well certainly, I can share with you how I approach patients in my practice who share

similar concerns as yours and explain what the different issues are that cause and create

this type of appearance.

A little bit of background, I'm a Board-certified cosmetic surgeon and Fellowship-trained oculofacial

plastic and reconstructive surgeon.

I have been in practice in Manhattan and Long Island for over 20 years.

Helping people with the under eye area has been a significant part of my practice.

And I have developed a lot of different strategies to help my patients based on an evolution

technology as well as greater knowledge about this particular area.

So it is important to have a sense of understanding about what is causing the line and what the

options are to improve this.

So to begin with, one of the critically important things to understand is that with facial aging

comes a significant amount of volume loss.

And volume loss means that there is a decrease in the bone structure particularly in this

area, the eye socket called the orbit.

The orbit and the cheekbone, the zygoma, are very important in their relationship.

So as there is volume loss, so loss of fat, loss of muscle, loss of the soft tissue, the

fat under the skin and the skin itself also gets thinner.

In a way, sometimes it's referred to as a skeletonization and you start to see some

of the underlying structures.

So when you see this line, essentially what you are referring to is a ligament that connects

the skin to the eye socket.

And when we look at the area, when we as a physician look at this area, I think it's

less productive to chase lines.

Essentially in many ways, the gold standard of wrinkle treatment has been to erase wrinkles.

And so whether it's with creams or with lasers or with injectables, there's always,

"chasing the lines".

What I think is more relevant and you can look at people who are considerably younger

is that they have similar issues.

There is an important understanding and balance to facial harmony.

And what I mean by that is when you look at your eye, I would say look at the eye and

the cheek together.

When you look at the eye and cheek together, there are certain things that you can notice.

One is you might be able to appreciate that above that line is a little bit of puffiness

which means a little bit of the fat that is normally around your eyes that is pushing

forward.

That's called lower eyelid fat prolapse.

Below the line, the cheek volume may be also diminished.

And so there is often a strategy that I would recommend with my patients that, depending

on what they look like in 3D, three dimension in front of me which is something I can't

necessarily comment on in that opportunity is basically, you look at different angles.

When I do an analysis in my practice, I take pictures from multiple angles so patients

get to see what they look from angles they don't usually see when they take a picture.

When they take their own picture, they are usually smiling, they are usually posing.

When you're taking medical photography, you're taking photos of faces at rest and

with specific movement if those movements are relevant.

So opportunities may exist such as the placement of fillers in this area to help improve the

valley next to the elevation of the fat pockets in an area where we often refer to as the

tear trough to orbital rim area, we can use fillers.

Fillers are usually in the hyaluronic acid family.

An important caveat is that placing fillers just in the edge there may soften that line

but it will not make you look significantly better.

This is where we suggest to a lot of our patients to consider procedures such as Structural

Volumizing.

Structural Volumizing is a very specific technique on using a thicker hyaluronic acid filler

such as Juvederm Ultra Plus or Juvederm Voluma.

By placing it at the bone level between the bone and muscle and restoring volume in the

cheek, you really create a very natural projection that very often contributes significantly

to improvement in the eye area.

We have to always think not only specifically and zone in an area of concern but look at

the face in the whole context and look facial harmony and balance.

So I would say that there is certainly a lot of options beyond just fillers.

There are ways to also improve skin quality be it with lasers or with emerging or significant

technologies in regenerative medicine such as platelet-rich plasma (PRP).

So I think it's important you meet with qualified, experienced, cosmetic surgeons,

physicians who really work a lot in this area who understand what it is you are concerned

about.

I find that a lot of patients come in who are essentially unsure on why they went to

a doctor, got a filler in the place they wanted and yet dissatisfied.

And I would argue that it's about harmony.

People want to look better.

We as human beings, we identify certain lines or characteristics more globally, then you

can really create a lot more of satisfaction by understanding the relationships that can

actually help you look better without just addressing individual lines.

How Lumps from Cheek Filler Placement can be Avoided with Deeper Placement and Thicker Filler - Duration: 8:56.

Thank you for your question.

You submitted your question with several photos.

And you stated in your question that you are essentially 18 days out after getting Restylane

placement in the cheekbone area.

And that you are concerned that there is still swelling and asymmetry.

And you stated that there was initially a significant amount of both swelling and bruising

that has gotten better and that when you have done research, you saw that most of this should

have resolved at 2 weeks.

And so you are concerned and you state that your doctor advised you to wait 4 weeks.

Well, certainly I can understand your cause for concern reviewing your photos.

And I can certainly suggest to you a strategy that might be relevant and should be discussed

with your doctor.

A little bit of background, I'm a Board-certified cosmetic surgeon and Fellowship-trained oculofacial

plastic and reconstructive surgeon.

I have been in practice in Manhattan and Long Island for over 20 years.

The use of injectable fillers is a very significant part of my practice.

And helping people get nice looking cheekbones has been something of an evolution in my own

practice.

And I'll certainly share with you my philosophy that guides how I essentially avoid situations

like what you are in currently.

So to understand first of all your doctor's intention, your doctor wants to add volume

to compensate for volume loss.

And the general philosophy of fillers is essentially to place fillers within the skin which is

intradermal or under the skin, just below the layers of the skin which is the fatty

cell layer.

Now the challenge the way I look at facial aging maybe is what really guides my approach.

And I look at this area, particularly in the cheekbone area as a function of bone loss.

And so as a function of bone loss, the placement of filler more superficially can commonly

result to something of what you are experiencing now.

When I look at your photos, I see a very distinct outline of the filler particularly in the

right cheek area.

And it is often perceived as, it can look okay but if it looks obvious, then it's

not so great.

And what is often a criticism is that the filler is placed superficially.

When it comes to these layers of tissue, we're looking at very thin layers and superficially,

it's relative.

So understanding that this filler was maybe placed in a way to try to mimic the appearance

of the cheekbone is something that is commonly practiced and we have seen patients with this

situation.

And essentially what I've done is I've dissolved the filler and I've started from

the beginning.

When I say the beginning, essentially what I do is a technique called Structural Volumizing.

And the point of Structural Volumizing is to address the volume deficit at the level

of the bone.

That means that through specific techniques, I place filler between the muscle and the

bone using something called blunt cannulas.

I also use a different type of filler.

I used fillers that are in the thicker category such as Juvederm Ultra Plus and Juvederm Voluma.

When placed at the bone level in a specific way, we've been able to actually achieve

very nice cheekbone definition with no evidence of irregularities, avoiding swelling and bruising

and ultimately getting very nice balance and symmetry.

Our patients very often, we'll just give them the mirror and they'll see the results

immediately.

And I can certainly tell you that it's been a significant paradigm shift in my approach

to helping people with the same issues that you are dealing with right now.

So essentially, the solution is defined by defining the problem and the problem is bone

loss.

However, the majority of colleagues place fillers more superficially with the idea of

trying to mold and shape.

But when it's under the skin, it's a little less easy to manage creating that shape and

it's a challenge to get it to behave in a way that is consistent and predictable.

So I think that this discussion should be brought to your doctor.

Certainly at this point in time, this degree of asymmetry and the appearance of swelling

is not optimal.

Therefore, it is up to you and your doctor to decide what to do.

It may mean some dissolving.

It may mean changing the strategy overall.

You know, I think this frustration that you're experiencing is actually very common and we

see patients with this challenge very frequently.

A lot of times, people see others who have had fillers placed in their cheeks and they

don't like the result because they look too rounded or pillowy.

And with Structural Volumizing, I kept going back to it because working from the bone structure

itself is much more deeper and therefore, the layers of tissue over it prevents this

very obvious outline kind of appearance that is more characteristic of the material and

the layer in which it's placed.

So I don't think you need to abandon your doctor.

Certainly if this is the first time you're doing this, it's important that you and

your doctor figure out what's best for you.

But I think that it's fairly straightforward to me that your anatomy is reflective of many

of your contemporaries where soft tissue loss, bone loss and thinner skin is kind of a set-up

for a limited volume correction unless the material is placed deeper.

I think that's basically the strategy that I would employ for a person like yourself

in my practice.

Ellen DeGeneres Reveals Special Gift She Received From Above 10 Minutes Before Her Dad Died

It has been a week of heartbreak for famously positive, generous, and upbeat television

host Ellen DeGeneres.

During Thursday's show, she told her viewers there was a lot going on in her life.

She said she needed them to be there for her just like she has been there for them all

these years.

During that show, she talked about the devastation in her hometown of Montecito, California,

where mudslides viciously ripped through neighborhoods, destroying cars and homes and taking at least

17 lives.

"[T]here are families missing, there are people who are missing family members,"

she said.

"They're finding people and bodies and I mean, you hear the word mudslide and you

have no idea the impact that it has, but after the largest fire in California history, it's

catastrophic.

It is beyond recognizable."

Today, though, she revealed her even greater heartbreak: her father, Eliot DeGeneres, passed

away unexpectedly this week at the age of 92.

"Before we go to break, there's something else I want to talk about," she said.

"I mentioned yesterday when I was talking about all the mudslides that are going on

in my community of Montecito, which is heartbreaking, I said that I had a lot going on in my life.

In addition to what's going on in Montecito, I lost my dad this week."

She shared an old picture of her with her dad.

As she held back tears she told her audience, "I don't know how old I was, but I do

remember that picture."

"He was 92 years old, he had a good long life, and he lived his life exactly how he

wanted," she continued.

"He was Christian Science his entire life, he never had medicine his whole life — never

went to a doctor.

I never had a vaccination.

We never had medicine growing up.

And he lived to be 92."

Though Ellen's parents divorced in 1970, it's clear she and her father continued

to maintain a close relationship.

"He was very proud of me," she said.

"He loved this show and he was a kind man, very accepting man.

There was not one bone of judgment in his body."

She also shared that her dad was "very funny" and said she believes she and her brother

got their sense of humor from him.

While her father lived a good, long life, it is never easy to lose a loved one, especially

so unexpectedly.

Thankfully, though, Ellen was able to talk to him and say goodbye just before he died

and she said she received a touching gift from him just 10 minutes before his passing.

"We only took one family vacation," said the talk show host, who films on the Warner

Bros. lot in Burbank, California.

"When I was a little girl, we came to this lot, to the Warner Bros. lot, and took the

tour and went around.

And now, I work on the Warner Bros. lot and I have my own stage with my name on it.

So he was really proud of that."

After saying goodbye to her dad, Ellen said, she hung up the phone and walked outside.

When she looked up at the sky above the building where she films, she couldn't believe her

eyes.

"I was leaving the building, and I looked back and there was a rainbow over the Warner

Bros. studio," she said.

"This is what I saw after I talked to my dad and he died 10 minutes later after that.

Pretty amazing.

… I got a rainbow before he died."

Our hearts go out to Ellen and to all those who are hurting from the loss of loved ones

or the loss of their homes and possessions.

The past few months have brought difficulty and hardship for so many across the country

and the world, but this rainbow is a reminder from above that there is still hope even in

it's an mahogany body with a solid quilted maple top and that top is

just really deep and nice-looking, I feel like going fully nude right now and

just jump in for a swim. Hard maple bolt-on neck with ebony fretboard, it has

a Gotoh GE1996 floating tremolo and the Gotoh locking nut and tuners.

And to describe the feel of the neck it has a slight D shape that feels very

comfortable, very smooth and even though it's a bolt-on neck

reaching to that 24th fret is not a problem at all. Now look at the back here

I've always loved when there's mahogany on the back, because of it being a bit

more red or brown it gives them more of a "roustic" feeling...

Let's go with elegant, it gives a really elegant look to the guitar

And when you see the neck you see it's a three piece maple neck guitar.

She's Confused When She Sees Man Bowing By Road, But When She Realizes Why She's

In Tears

It seems like honor and respect are becoming things of the past.

People don't respect their elders or honor the flag or the country's veterans like

they once did.

In decades past, there was a code of honor that everyone followed, but these days it

seems no one remembers it or cares to follow it.

That's why it was especially poignant when a total stranger stopped everything in the

middle of the day to show a veteran honor and respect as he was taken to his final resting

place.

Vietnam War veteran Phillip "Paw Paw" Dooley Sr. lost a long battle with cancer

on the Sunday after Thanksgiving.

His family was heartbroken to say goodbye to this beloved man gone too soon.

"It's always tough saying goodbye, but to the man who was the rock in our family,

it was something that I didn't expect to do when he was 65," said Shelby Sams, Dooley's

granddaughter.

As a procession of about 30 cars made its way from Langeland Memorial Chapel in downtown

Kalamazoo, Michigan, to the Fort Custer National Cemetery in Battle Creek, Dooley's family

noticed something that touched all of their hearts.

While some cars pulled over to let the procession pass and others sped around the line of cars,

one man got out of his car and stood by the side of the road.

"There was a man who actually got out of his car, stood by his driver side door with

his head bowed and his hands clasped in front of him for the entire procession," Shelby

said.

Shelby and her sister, Courtney Steffler, immediately burst into tears when they saw

him.

"It was the greatest gesture of respect that I've ever seen," Courtney said.

Another of Dooley's grandchildren, Garrick Dooley, shared a photo of the kind stranger

on his Facebook page in an effort to track the man down.

"My Grandfather was a simple man.

He worked harder than anybody I've ever met—he loved even harder.

This photo of the man showing respect, speaks volumes.

It's very rare to see in today's society," Garrick wrote.

"To the man that gave respect to my PawPaw as we passed by on the way to Fort Custer,

I want to say thank you.

This lifted up many lowered heads today."

Garrick asked friends and family to share the photo so he could find the man and shake

his hand.

Shelby agreed.

"I just want to say thank you.

I'd like to take him to dinner and buy him ice-cream – that's what my 'Paw Paw'

would have done," she said.

After the funeral, the family noticed something else about the photo that made the man's

actions even more meaningful.

Dooley was a hard-working family man who enjoyed the simpler things in life.

One of his favorite songs – one Shelby called his personal anthem – was "Daddy was Never

a Cadillac Kind."

They even played the song at his funeral.

When the family noticed that the man in the photo was standing in front of a Cadillac,

they took it as a sign from their Paw Paw up in heaven.

"[He's] looking down on us saying, 'We know you're in pain but everything is going

to be okay,'" Shelby said.

"I believe he was there and that was really the perfect message for him and maybe from

him," Courtney added.

The family hopes to track down the man so they can let him know how much his gesture

truly meant to them.

"My Paw Paw really would have loved to shake the hand of that man, so if we could do that

in his honor he would be very happy," Shelby said.

If nothing else, they hope others will see the photo and be reminded that a small act

of kindness can go a long way, even in the saddest and darkest of times.

When I was very young, I think 8 years old,

my father gave me a book of Sholem Aleichem.

Sholem Aleichem was a writer...

a Jewish writer, a Jewish humorist. And he described

the life of the Jews in the little villages and towns

in Russia, in Poland of today, Galicia.

My father was such a boy in one of those little villages

near Lviv, near Lemberg.

And I felt that he was giving me the key to his childhood.

Because, usually he wouldn't talk about his childhood.

But, through the book, I think he felt he could give me

the key to his childhood. And I started to read, and...

I was fascinated. It was a reality that I never knew.

I was an Israeli boy, in the first years of Israel.

A country that built itself after the Shoah, after the Holocaust.

A strong country. A country that's tuned to the future.

Does not want to look back at the atrocity and the horror

of the Holocaust.

And, suddenly there was another melody in these stories.

It was a whole world that

I believed existed in parallel to my life then, in Israel.

I had no idea that this was the world that was

exterminated during the Holocaust.

And it was a whole world with its

codes, and constitutions, and characters, and a special melody.

And I felt he gave me a treasure, really.

Because nobody at my age knew about Sholem Aleichem,

nobody knew him. And I read it like today

a child would read Harry Potter. It's a magic world.

Even I remember...

the numbers of the pages were not in digits, but in letters,

as it is in the bible, or the Talmud. So,

I think I thought I got a kind of

some secret, that only me and my father knew.

And I read it. I read the whole book.

And I was taken by the reality of the Jews

in those little towns.

And how they were afraid of the non-Jews, of the gentiles.

I never met someone who was not Jewish until then!

I thought all the world was Jewish!

And the life of this little orphan Mottel,

who became my hero, and I identified so much with him.

And I think that in a very short time

I read all the stories of Sholem Aleichem,

that most of them were for grown-ups. But,

because of the attraction of the childhood of my father,

I inhaled all of it. I read them through and through.

And I remember there was, when I was 9,

the radio, the Israeli radio, it was a governmental radio.

And we had only one station.

And they announced a national competition

of Knowledge on the writings of Sholem Aleichem.

And I told my parents, "I want to go, I know Sholem Aleichem!"

And they laughed at me, they said, "You're a kid."

I was less than 10, I was 9.

So, I went and bought the first postcard of my life,

and I mailed it to the radio authority,

as they announced on the radio.

A week later, came a letter from the radio

inviting me to be auditioned.

My parents almost fainted. For them it was as if

David Ben-Gurion, the mythological leader,

called them and told them, "Give me your son!"

So, my father accompanied me to the audition

and, of course, when we entered, they said, "No, no, the boy is out,

and you stay here." And my father said,

"No, the boy is the matter here."

And they started to ask me questions and I really knew them

I had a very fresh, good memory of a young child.

And, then, this is funny,

they said, "We have to accept you,

but we cannot, because it's not educational

that such a young boy would win such a big money prize."

It was something like, I think, 100 dollars, or something.

Israel, at the beginning of the 1960s

was a very ascetic, also poor country,

so they thought it might corrupt me or something.

So, they said, "But, we shall allow you to be in the studio

while we're broadcasting this."

"And when one of the competitors would not know the answer,

we shall turn to you." And so, it was, and each time

one of them didn't know, they asked me, and I answered.

And there was some people there from the radio had the idea that,

"Here there's a boy we can use to make reports,

and to interview people." And they suggested it to me.

So, at the age of 9, I became a kind of a young correspondent,

the only young correspondent in Israel. And I interviewed

the president, football players, theatre players.

In a way, I stayed in radio for 25 years.

And, later, I did everything, I was anchorman

of the main news magazine, and I had my own programs.

But it was so sure how Sholem Aleichem influenced my life,

how literature actually changed my life.

And I'm indebted to him,

always I will be.
