How Laser Treatment for Acne Scars can Benefit from Non-Laser Treatments Below the Skin - Duration: 8:41.

Thank you for your question.

You are stating in your question that for your acne scars, after you undergo CO2 laser

treatment, as you became aware that the laser treatment maybe up to 50% effective in improving

acne scars that you want to know when to follow-up the treatment with 1540nm laser and how long

after the laser can you do that.

Well, I can certainly share with you what I think the principles that are relevant with

this type of question.

A little bit of background, I'm a Board-certified cosmetic surgeon and Fellowship-trained oculofacial

plastic and reconstructive surgeon.

I have been in practice in Manhattan and Long Island for over 20 years.

I have been using lasers since the early to mid 90s including the original CO2 ablative

lasers and it's certainly a time in the world of laser treatment when we have so many

options.

When you have these many options, you also understand that there's clearly no one single

best approach to any particular problem.

And acne scars are one of the most challenging problems to treat.

And what you have to think about is really looking at your situation through the lens

of someone who is going to be able to manage you from all aspects and not just treat you

with the laser.

And the reason is that to understand that most people who have acne scars have a combination

of scars.

So whether they have rolling scars, boxcar scars, ice pick scars, atrophic scars, facial

fat atrophy, there are a lot of things that are usually the case when you evaluate someone.

And so before you start with any laser, what I do is I look at the foundation basis of

what gives a person smooth looking skin.

People always look at their skin and think it's all about trying to smooth out the

top layer.

But when you think about skin, you get a structure that's about 2 millimeters in thickness

in the face.

You also have to think about volume below the skin.

So when you ask a question like this, I would hope that you've also had a discussion with

the physician about strategies.

For example, when I evaluate a patient with acne scars, I take photos with direct light

and I take photos with backlight.

With backlighting, it shows me areas which particularly cast a significant shadow.

And a lot of times, I find that the most significant shadows are cast because of volume loss.

So a lot of times what I'll do is, even on a younger person, I'll do something like

Structural Volumizing by placing a long lasting filler like Juvederm Ultra Plus and Juvederm

Voluma at the cheek level on the bone and that results in a kind of a restoration of

volume.

And when there's restoration of volume, acne scars look smoother.

A lot of patients who come in who have experienced facial aging on top of acne scars, they really

appreciate that significant change probably more than anybody.

But the point of this is understanding that you have to work from the inside outward.

We would also combine other treatments as foundational ways to improve the skin's

ability to respond.

So I would sometimes combine this with platelet-rich plasma (PRP) injection, micro needling with

platelet-rich plasma.

Basically, I try to minimize more aggressive ablative procedures before doing them so that

we get the skin healthy, we get the skin full and you may end up needing more conservative

treatment.

There's a basic principle in wound healing and that is after you have an injury and that

includes the controlled injury of laser resurfacing, then there is a proliferative phase of generating

collagen that is essentially about a month or so.

And I think based on that concept, it might be safe for you to go ahead and do a non-ablative

procedure such as using this particular laser afterwards to try to stimulate collagen.

But I would caution you that there's also a wound remodeling phase.

And I would tend to lean more on the conservative before trying to stimulate the skin to generate

more collagen because you can only generate so much.

A lot of times people are frustrated because they get a laser and another laser and another

laser.

A lot of times patients have come to me and their skin after so many different laser treatments

actually looks really thin.

And so we do what we refer to as tissue rehab where we're using platelet-rich plasma derived

from your blood.

We sometimes combine this with Acellular matrix to help stimulate the fat cells and give the

skin some substance before treating it with another laser.

Interestingly, the world is coming full circle and a lot of the luminaries who have been

the biggest proponents for laser are now advocating PRP as well as microneedling.

So a little vindication is certainly something I'm enjoying.

Nonetheless, I really think you have to think through this process because you should have

a realistic understanding of outcome.

People, generally speaking, want to see significant improvement.

A lot of times acne scar patients get very frustrated because they don't necessarily

see the improvement, they only see where there's still loss.

And so it's very important to have a good relationship with your doctor and have clarity

with what the outcomes will be and the timing.

You know, we have a lot of tools but you have to know when to use them and when to hold

back and wait.

And of course, ultimately, it's my goal to try to do what is most effective with the

least amount of downtime and have a sense of what would really make the most impact.

You have to unfortunately accept that there's no perfect solution for acne scars.

There are ways to treat and improve appearance but some things cannot be completely reversed.

everyone this is another of the Toys R Us exclusive brick Tober event 2017

figure packs this one is for LEGO City as one of the more plain packaging

designs of the series of four and as with the others this is available in

multiple countries exclusively at Toys R Us and different places handle it

different ways with the promotion in the US for the most part I've seen these

being sold for twenty dollars at the register or in the Lego section or you

can get them for free with a purchase of $50 or more worth of other Lego stuff

now though this is an exclusive pack as far as I can tell it only has one

exclusive element in it out of all the pieces of those fingers now these all go

with the summer 2017 city jungle or jungle exploration theme so it's kind of

a booster pack for that theme and they've just given us a selection of

figures that fit right in with that and mostly just combine different parts from

different figures that we've already gotten I like the orienteering compass

of the one on the far left that's just a nice little printed tile piece there if

it's not new to this set and I also personally very much like the I guess

sign just general scientists torso here because it's just a useful piece we

don't get you know scientists I feel that don't have some sort of themes

specific logo on them but the exclusive piece here I believe I don't think I've

seen it anywhere else is the torso for this mechanic lady here so that's kind

of the female counterpart to the male mechanic that they put into a couple of

the sets from this series and I think that's that's cool you may have heard me

many times complaining about unnecessarily gender specific torsos

with the key word there is unnecessarily here I think that both of these kind of

make sense and they kind of need to be gender specific just because of how the

clothes fittin everything and it's nice to have

that counterpart to show especially young kids might not know any better

that women can do you know dirty jobs too can work on cars and vehicles and

can do welding and stuff you know it's it's just it's cool just to have a

little more variation the magnifying glass works all those do the woman with

the exclusive torso print does have an Arctic exploration headpiece which has

that Arctic orange goggle set none of these have alternate faces on them but

nice prints on the backs of all of these torsos kind of too bad that the print

for the scientist here doesn't go well I guess just isn't set up better to to

show whatever device that is you know whether that's like a just some sort of

phone or it's a PDA or something whatever it's supposed to be a be nice

to see more of that but overall not a bad little collection but probably will

be one of the less desirable of them just because this wasn't a really

high-profile theme you know I think it was a good theme but doesn't have a

whole lot of desirability relative to some of the other things I like getting

these parts but that's just it for this set so I will move on thank you for

hello everyone once again I has a bag not a box this time I has a bag inside

of this bag I definitely expect to find Lego stuffs and to get that bag open it

looks like it's been used in a couple times now which is fine at least it was

taped up properly all around the edges so I'm confident that everything is in

there even if it got beaten up worse on this trip to me in this life this bag

still looked like it was going to hold on to all the stuff inside pretty well

so packages can be ugly and they can still be safe for stuff inside that's

that's what I care about to open this bag because it is kind of messed up I

has a knife so I will be using this knife here

not my favorite knife but has some redeeming qualities and it came fairly

sharp from the factory so that's good slices nicely looks like you got a it

paperwork here I'll just take that out and it's inside I know one of the things

be reviewing one of these I got a spare because I want all these stickers I'm

collecting more and more Lego set LEGO version of Lego set stickers and printed

pieces and I want to eventually do a larger Lego store or something just to

take advantage of all those parts have so many and they keep making so many

more that I feel like it would be a shame to not you

I've used a bunch in my Lego store that is in my mall and has been in my mall

for years now but one more this is a rock was owned was it heavy not sure

whatever they did a bunch of teams back then and I tend to confuse the names but

when I do the review of it look it up just haven't looked it up this time

really interesting figure there and no just a fairly straightforward take on

the small submersible design but some interesting prints and color schemes use

there and then just a bunch of animal oil pieces in here because I went to

this store for this in particular data good low price i didn't want to keep

spending the big bucks at lego to get more and more but they're the set they

had meant some wedge plates older wedge plates of lips new designs had these on

my my wish list at rick link just general wish list for a long time so i

think i have a few of those now and some decorated pieces with some difference

interesting designs useful designs hopefully it's a alder gray piece with a

little bit of metallic printing on it not a lot of great interest these are

part of process of slowly collecting the first ever lego space set trying to do

it by parts i might just break down and get the real deal at some point but for

now i'm trying to do it by parts that's more printed and or sticker pieces here

ooh like that in blue i have a bunch of them in that regular gray but not in

blue yeah so just some little stuff and then off to the side from another parts

all I've got some other pieces they just happen to have more of these masonry

bricks I've used up a bunch and need a lot more here's yet another of these so

two for two there some more corner pieces and the little white modified

stud or tile with mini-pig all sticking up very useful base of Angry Birds style

of balloon but in gray very useful for planetary defense for something jump

packs just really cool pieces that I may use for plant planetary defense for

stuff for you know for actual as they were intended for jump packs or I may

use them for something else more likely to use them or something else because

they're just like the attachment points they have in the textures that they have

on there and these oh-oh-oh ones get away from me got it you get

away from me never yeah okay that's that so some cool

pieces especially printed and or stickers parts a vintage ish small set

most importantly another of these for me specifically for sticker sheets used

towards a future build that is going to have to happen that's it for this haul
