Hi, Everyone. I'm Dr. Zhao
Today is Chinese New Year's Eve
Tomorrow is the first day of "Year of the Dog"
I wish you all a happy NEW YEAR
Since it's new year, I'll play a longer video
Usually, I make 10 minutes videos
I picked some "fancy dishes series" videos from first half year of 2017 for you
I picked some classic videos
The first one is "Beef Jerky"
It looks like a black stone
It's very hard
Let's try to remove it using pick
I can remove it using pick on some patients
If I can get to the rear of this earwax after loosened up the edge, I can remove this with pick
But this earwax is very hard
This is different than cholesteatoma
Cholesteatoma is soft on the inside, hard on the outside
This is after soaked with sodium bicarbonate ear drops. I tried to use pick, but the patient is afraid of pain
Actually, any ear drops will be fine, water is fine
I don't wait untill the whole earwax is soften, because the earwax will expand, and that is uncomfortable
I only wait untill the edge is soften, that way the edges are lubricated
I let it soaked with ear drops abour 10-30 minutes
It won't soften the whole piece, but the edges will be softer
We will let it soak for few days if we don't have a endoscope. Then, remove it using washing method
This patient's ear canal is wide, and there isn't much of ear hair
So, we have a very clear view
Now I can get in there with pick after soaking with ear drops
Patient was afraid of pain earlier, and I couldn't use pick
Now it's loose
Take it out just like opening wine cork
I believe this video is from January
Some of you might remember the date
Last January (2017)
I removed the whole piece
The skin of ear canal are red from removal
This is how you deal with "fancy dishes"
Try it with pick first
Lubricate the earwax If the earwax is hard, and people who are afraid of pain, then we can use this method
This is smaller than the last one
This video is about 36 minutes long
This is like a compilation of endoscope viideos
Find a gap, get in there
I'm on vaction today. Some of my colleagues are still working
They are in charge of inpatients and emergency patients safety
Salute to my colleagues
Ok. back to our topic
It's difficult to do this procedure with naked eyes
It's hard to measure the distance
I think this patient probably soaked earwax with ear drops, because it's not dry
The ear drops didn't soak the earwax entirely
If it's soaked, it will be easy to remove with vacuum
You can't rush it. You have to complete the initial step
You might not take it out if you force it
It might also damage ear canal
Ok
It looks like it has been pushed by cotton swab
Because the earwax looks like this on the eardrum
It looks like it has been smashed
Second piece attached to the eardrum
We have to be careful with this
Ok
Second patient
Ok. Eardrum is in good shape
We will watch the third patient next
Today's compilations are all earwax removal
It looks like earwax, but now I think it might be mixed with fungi pieces
The earwax has been pushed by cotton swab
You can see the hole that was pushed by cotton swab
Pick the edge, and take it out with plier
It's loose now
It has earwax and fungi, because I saw the fungal hair
Get the edge of this with plier
Slowly
Patient felt relief when I took this out
Ok. There are some left in there. It looks like fungi
Remove it with vacuum
Changing the tube of vacuum
We use different tools depend on different satuations, I personally like to use pick
It's easy to work with pick inside ear canal
Ear canal is narrow, and the vacuum is small
Patient needs to apply antifungal cream after
Some patients feel dizzy when I use vacuum
More than half of the patients felt this way
I have seen this, so I think this is fungi
Most of the patients were fungal infection
The second most of my patients were earwax
Third place is cholesteatoma
And the rest are something else
That was the end of that patient
Next one
Shiny
You can clearly see the hole that was pushed by the cotton swab
It looks like "Tanghulu" (fruits covered in hard candy on bamboo skewer)
It looks like that hard candy
Switch it to pick
Hook the rear of earwax and pull it
"Gain weight during holidays"
I think I need to buy new pants after this vacation
The pants that I have right now cannot hold my fat waist now. haha
Hook it
Ok
One part fall off
It's kinda big
Still got one piece left
Ok. Eardrum is clean
All the patients in today's video have wide ear canal
Some patients have so much ear hair, this one doesn't have that much
It affects your watching experience
This one is easy
Beef Jerky
So easy
Get this small piece
Eardrum is good
Ear hair is a problem
I might be the ear hair that causes earwax trap inside your ear
We don't do anything about the hair. If we cut it, it's a waste of time
It would take more time cutting ear hair than this procedure
It's troublesome to do it
We will trim it if the patients need middle ear surgery
Usually, we don't do that with this procedure
because you need special scissor
Hook it
Another piece
Most of the patients came to me by my reputation
Some are arranged by my colleagues while I'm in clinic
One more piece
Ok
This patient's eardrum was damaged. There is a hole
If we use washing method, then he might get tympanitis
There is no protection after the eardrum is damaged
The liquid will get to the middle ear if we use washing method
This patient used ear drops
It's still hard
Ear drops won't be easy to get inside if there are too much earwax
Just like a glass of concrete. There is no room for eardrops
It doesn't have any effect
Try to use pick
Hook the rear
It's stuck in there
I didn't have much patients like this in the second half of 2017
I cleaned most of "fancy dishes" patients who were from Beijing
Ok. pick it
The pick is short
Switch to a longer pick
switch to plier
Let's see if we can get it
switch to a longer pick, and hook the rear
Big beef jerky
Let's see the eardrum
Using endoscope to remove earwax in outer ear is easy. It doesn't require much skill
Middle ear is complicated, such as cholesteatoma
Remove cholesteatoma on the tympanic cavity is harder
Middle ear procedure will bleed more. haha
It's not appropriate to play it on media
Usually, it's bloody, and you don't understand what we are doing
Unlike this procedure, It won't bleed
Some will bleed just a little, and some of your can't watch that
It's stuck
Keep going
Ok. It's out
Ok
Eardrum is clean
1, 2, 3, 4, 5, 6, 7, 8, This is the Ninth patient
It looks like dead skin
It's like cholesteatoma relapses
It's not that big, but it's very hard
Ninth patient
It's like hard sticky candy
It might be cholesteatoma relapses
I wasn't paying attention, we are missing one part
There were some problems with graphic card, I changed it at the end of 2017
This was after removing that big piece
Now you can see it's cholesteatoma relapses. A layer of white epithelium
That big piece was removed
Graphic card had problem
One piece left
There is a hole at the rear
There is a hole on the bottom
So much
Tenth patient
It looks big, but it's a empty radish
I can remove it with plier
There are small pieces left in there
Ok
Eardrum is clean
There are leftover on the edges
Some of you who have OCD cannot take this
Eleventh patient
small beef jerky
It's not big. Get it with pick
This is easy
Adjust the angle
Flip it
That is also empty
Hook it
Clean
Eardrum is clean
There are 12 zodiac animals, so we'll have 12 parts today
This is the twelfth patient
Beef jerky
Most of today's patients have beef jerky
But there isn't much look exacally like it
He is very talented
There is gap on the edge. This patient is simiilar to the first one
Pick the edge
Remove some of it or else it will stuck when I pull it
Pick it
Hook the rear
This is kinda painful. Children cannot take this
It's stuck
Ok. It's out
We can't rush it
You have seen this, but you will only see one patient like this in a video
I picked 12 in today's video
It's from January to April
I'll upload more on Monday
This is wide, and there isn't that much hair
I know some of you want to kick me away, and do it yourself
You can't do it
This is a human being. This is not your toy
If you are not gentle, the patient will feel pain
He will try to avoid it
Hook it
There is no anesthesia in my clinic
Patients have to hold it on their own
Patients will move if there is any stimulation
The camera is stable because the patient can tolerate this
It's almost out
Remove the edge, so it will be easy when I take it out
It's like giving birth
Ok. Hook the rear
I hope I can upload this ASAP
This is it for today. Happy Chinese New Year
Check it. Eardrum is clean
Bye~
