Youtube daily report w Apr 10 2018

Okay folks

Here's the thing

I'm leaving to China

I'm not moving there, don't feed your hopes

I'm leaving for a week

Because in China is going to happen global vape expo. The most huge expo ever been

I've been invited by organizers, so I could come, visit, look, did highlights and be able to tell you what's going on there

Sadly, I'm gonna experience the internet issues in China, so I won't be able to do Youtube live streams

But most possibly I'll be doing Instagram streams. And that's what Chinese organizers asked me to do. We'll see how it'll work - everything is depends on their internet

Anyway I'll get VPN and will try to use everything as I used to, but I don't know how it's going to work

Also, while I'll be in China I'll try to visit few interesting factories - Sigelei, Dovpo, Asmodus, Vandy Vape. I'll try to get to Aspire as well. Overall - I'll try to be everywhere, so you will have interesting content to watch

Jeff is coming with me, he's gonna film all of this and I think that we're gonna film a lot

But due to I'm leaving, there is going to be some limitations on the channel. It's not gonna affect video upload schedule - videos will be uploaded every two days, but I'll try to shorten them.

I'm talking with you in the beginning right now - I won't be doing it in next videos. It's going to be like that for like 8 videos

So let's prepare for a fast reviews. Let's roll. intro...

Hehey! Hello everyone! Alex from VaperMD is with you

And i'm glad to welcome you on my channel

And today our guest is VCIGO K3(this name is abbreviation for short circuit in Russian) by Sigelei

Not the best name for a box mod, K3, it sounds scary in Russian

Anyway, it's K3 by Sigelei - box mod that is completely made out of plastic and can handle very low resistance

It comes with a weird tank, but all of that we're going to see when we dive down

So let's go

Okay folks, let's unbox this white packaging. It's written VCIGO K3 kit, lower we have a picture of device and Sigelei logo

It's also written that this kit is perfect for beginners and great for vapers

Great(klass)

On this side we have a logo and barcode, this side we have description of kit contents, a little bit of information and technical specs, QR codes, and certificates

Under the cover we are met by huge pile of papers, a bag with silicone airflow adjustment ring and o-ring for a glass, spare coil head in non-seal packaging

MicroUSB-USB cable for charging and QC pass

And now, as always, a little bit of technical specs. This mod is made out of plastic and available in 4 colors - Green, Blue, Black and Red

This mod is driven by 150W chip with support of TC mode for popular wires and supports resistance down to 0.05Ω. Maximum voltage is 7.5V

It's powered by 2*18650 and can be charged via MicroUSB with 2.5A current

Also this mod fits atomizers up to 27mm in diameter

This kit is comes with a O9 sub ohm tank with 2ml capacity, top fill and airflow adjustment with separate silicone ring

Diameter of this tank is 24mm

Okay, we're done with specs. Now it's time to give it a detailed look. And we'll start with the tank

This is O9 tank by Sigelei and this isn't new - Sigelei already used that tanks in kits of their other products

But let's quickly look at it. On the top we have a drip tip - this drip tip have threading and knurling for convenient unscrewing. And by unscrewing it like so we're getting access to fill slot

And I guess you understand, that you are unable to change this drip tip and when you will be filling the tank you're gonna touch it with your fingers

To replace the coil head we need to do next things - for a start we need to get rid of the glass, which is often uncomfortable

Next we need to unscrew the shaft and after that unscrew the coil head itself

Pre-installed coil head is absolutely the same as the spare one - SM2-H. This is 0.2Ω coil head, recommended wattage is 60-100W, best at 70-90W

Vertical single coil

Lower we have a deck without airflow adjustment. Airflow adjustment is available with silicone ring, which we have in the kit and which I showed you earlier

Okay friends, let's fill it up and let it sit and saturate. And we'll have time to talk about box mod

Okay, I'll repeat myself and say, that this box mod is made out of plastic. On the top we have metal connector - as you can see it's not centered, but it fits up to 27mm atomizers anyway

Inside of it we have slightly spring-loaded gold-plated pin with flat head cutout. I tried to screw or unscrew it - nothing happened

For example 26mm atomizer will look pretty nice - as you can see it have no overhangs and it fits perfect

Let's continue. Over here we have control panel - fire button in shape of a shield with a logo of this device, the screen, + and - buttons and MicroUSB slot

On the sides we have nothing interesting - only some design curves

On the rear side we have battery door with logo and name of this mod

On the bottom, as always, we have vent holes and notch for opening the door

I'll say - this battery door isn't held very well - it's loose, but if you shake the mod it'll not fell off, but it removes easily

I would install stronger magnets in here

Okay, this is the slot for 2*18650. It have a ribbon for convenient batteries removal, correct polarity marks and contacts on the bottom is spring-loaded

To make this mod working let's insert 2*18650 batteries

Right after we inserted the batteries the mod is turning On and we can see important information

In regular state the mod will display wattage, voltage, resistance, battery life and current mode

And we've seen this chip before - it's sorta old and standard Sigelei chip - it's old by it's working

You can adjust the wattage with + and - buttons with 0.1W increments

If we click fire button 3 times we'll get into Menu, to be more exact it's the Mode selection

In here we have Power, TC for SS, Ni, Ti and TCR more

If we press and hold Fire button and + button simultaneously we'll get into Preheat settings - you can adjust the wattage and time of preheat

If we press and hold Fire button and - button simultaneously we'll lock +/- buttons, but Fire button will be working. We can unlock it the same way

If we press and hold - and + button simultaneously we can lock the resistance, but only in TC mode

And by clicking Fire button 5 times we can turn On and Off the box mod

And now little size wise comparison with other 2*18650 mods. Our today guest is kinda tall

Overall, that's all regarding this mod. Let's go back to face-time and talk of its pros and cons

And of course we'll try this atomizer

Thanks for your attention in close up. Let's go

Overall, folks. Everything is working and it's good by itself. But anyway, I found some cons and pros, which we are going to discuss right now

Let's begin with a tank. First thing that I don't like is plastic threaded drip tip which screws into metal and it's a top cap of this tank as well.

it's not good - by screwing and unscrewing this top cap plastic threading could be damaged - metal can easily do it with the plastic and we'll left without top cap. It's a con

This design is a picture of how things shouldn't be done - when you are outside - for example in subway(underground) and you need to refill the tank - you have to unscrew the top cap. Don't vape in subway, you hear?

You was touching handrails, handshake with someone and after that you starting to touch your drip tip. It's not good for your health. That's why it's a con

Next con of this tank is complicated coil head replacement. To change the coil head you need unscrew the drip tip, remove the glass, unscrew the shaft, unscrew the coil head, then screw down new coil head, screw the shaft, put the glass and screw the drip tip

LONG

Okay, I'm done with the tank, let's talk about box mod

About the mod(reference to fashion - words mod and fashion are very similar in Russian language). Hello to Anastas

First con is the battery door. This door plays in all directions, held loosely and removes too easily. Two magnets are too weak for this door. Well, battery door, as always, is a weak side of Sigelei. It's 2018 and Sigelei still didn't make good battery doors for their mods

You need to work on this, Sigelei

Next con is very subjective. I don't like this edgy/angular design. This design is not for everyone and this is my personal subjective con

Folks, don't forget that this mod is made out of plastic. And plastic isn't the most durable material. In case you drop this mod on some hard surface - on a tile or concrete - I suppose this plastic will be damaged

So be very careful with plastic mods

Okay, I'm done with cons. Let's talk about pros

You know perfectly what I'm thinking about vertical coil heads - so I will say this - this tank have some flavor due to short distance between coil head and your mouth

And yes, this coil head can produce some flavor. Not the amount as rebuildable attys do, but it's here. And I even give this tank a pro for that

This tank have non-standard appearance. I don't know if its a pro or not, but it have kinda bubble glass and if it fall down on hard surface - the glass will shatter

So treat this kit very carefully

Now let's talk about box mod itself. First pro I'll give for a chip. It's a good old familiar chip with great button response and it can handle low resistance

So I'll give a pro for a chip, because it's simple, logical and understandable

Next pro to this box mod I'll give for fitting large diameter atomizers - up to 26-27mm. It's good. Though connector isn't centered there is enough free space

This box mod is light. When summer comes this mod will not be a stone in your pocket. Or in a bag. So it's very comfortable to carry it in summer season

Yes, it's big. It'll be stick out of you pocket, but in the same time it's very lightweight and it's a pro

Overall, this mod is assembled pretty good, excluding battery door of course. But aside from that everything is great - button doesn't rattle and clicks nicely, +/- buttons, the screen - everything is good

If we exclude the battery door - it would be perfect in terms of quality of manufacturing, but the battery door fails it

It's easy and undemanding in use. And that's it for pros and cons

And now we'll discuss the price

On the official Sigelei site this kit cost 56.99$

On other sites this kit cost 45$

Even if we ignore the official price, 45$ is too much for this kit. It's expensive for a plastic box mod. I think, this mod should cost, even without atomizer, somewhere around 30$ and this would be good price. But 45$ is too much

Okay, folks, let's summarize. I don't want to talk about atomizer, let's talk about box mod

If it's gonna cost 30$ - then I can recommend it, because it's a convenient box mod to use in summer season. You take it for summer and forget about it after, because it's cheap

But if this mod gonna be expensive - like 40$ and above - I don't see a point buying this box mod

Overall this box mod is good. Great chip, nicely assembled....Guys, guys from Sigelei, PLEASE, do the battery doors better. PLEASE make these battery doors good. PLEASE

If you do so - I can easily recommend this mod. But at this moment it's up to you to decide - is this box mod suits for you or not

And i'm done at this point

I wanna thank YOU for your attention

I wish you a delicious vape and let the vaping come into your life and stays there for long

Rosberg reveals Formula E investment, will drive Gen2 car

Nico Rosberg has revealed that he became a Formula E investor "some time ago" and the 2016 Formula 1 champion will give the series' new car its first public demonstration.

The former Williams and Mercedes F1 driver, who visited the 2017/18 FE season opener in Hong Kong and is a shareholder in the electric series, will drive the Gen2 car at next month's Berlin event.

Rosberg will have a 10-minute demonstration slot prior to the action starting at the Tempelhof Airport-based ninth round of the season. "I'm really excited to get behind the wheel of the Gen2 car," Rosberg said.

"It will be great to experience this high-performance e-mobility car and lets see if I still have it in me!.

"It makes it even more special that I'm able to do this at the Berlin ePrix, as I've always loved racing in my home country - Germany.

"Since my retirement, I've become very interested in the e-mobility movement and its potential to positively impact our society and our planet.

"I've therefore also long believed in the potential of Formula E and came on board as an investor some time ago.

FE CEO Alejandro Agag hailed Rosberg's decision to invest in the championship and expressed his excitement at the upcoming inaugural running of the new car.

Seven of the nine season five FE manufacturers gave their new machines a first outing at the Monteblanco circuit in a group test last month – BMW and NIO were absent – but the running took place behind closed doors.

The new car, which has increased battery storage compared to its predecessor that means drivers will no longer have to swap cars mid-race, was unveiled at the Geneva Motor Show last month.

"It's a very special moment to see a new car running for the first time in public and it requires an equally special name to have that honour," Agag said.

"To see a name like Rosberg taking the wheel of the Gen2 car in front of the German fans in Berlin is a great moment for the series.

"He's such an incredible ambassador for world motorsport and a champion - so it's also another fantastic endorsement for the ABB FIA Formula E championship to have Nico as an investor and shareholder of Formula E.

The Secret to Mass Personalization & Personalized Content with AI (2018) | AI for Business #3 - Duration: 5:51.

Hi and welcome back to another of our videos on artificial intelligence. We

know that personalization is a powerful way of influencing consumer behavior but

a few years ago the generation of personalized content used to require

much more effort on the company side. Now, the increased availability of

artificial intelligence, combined with marketing automation, made sophisticated

segmentation less costly and faster to implement. So, let's get into depth and

see how AI can improve your personalization strategy. First of all,

computers are now able to perform profile and classification of customers,

based on the data that they actively or passively provide, the so called digital

footprints. In the last few years this practice became known as personalization

at scale. McKinsey has summarized what personalization means to customers

according to this simple formula. So let's analyze this relationship. It is

directly related with the relevance of the content, for example consumers have

reported to preferred suitable recommendations that they wouldn't have

thought by themselves. The traveling industry makes use of text mining to

create and test recommendation systems, based on the similarity of the

destinations. For this, they use the reviews that previous travellers have

made, their most co-occurring words to describe a particular destination. With

that they can set retargeting campaigns suggesting similar destinations to

previous travelers. Another factor in the previous formula is timeliness. Users

report to prefer to be approached when they are in a shopping mode and because

nowadays people are always using their smartphones to check their emails.

Sending automatic messages at midnight might not seem very timely, or it just

don't make sense to their daily schedule. So, how do we make these things in a way

that does not deteriorate trust and does not interfere in their privacy? After all,

what are the boundaries of digital mass persuasion?

Recent research has shown that people usually do not behave logically when it

comes to privacy. For example, we often share intimate information with

strangers, while we keep secrets from our close ones, the so called privacy paradox.

This helps to explain why that on average just 65 like Facebook pages

allow behavior analysts to understand someone's

personality traits better than friends do, 120 to understand them better than their

family members an 250 to understand them better than a partner or spouse.

Nevertheless, behavioral science has identified some factors that predict

whether people would be okay with the use of their personal information. I

will illustrate this using some experimental examples. What happens when

we know that a friend has reviewed something personal about us to others.

We usually get upset and those norms can also be applied to our digital life. The

researchers use the dimensionality reduction method to find groups of

practices that consumers tend to dislike. They did that based on a list of common

ways in which Google and Facebook use consumer personal data to generate ads.

The results suggest that obtaining information from third party platforms

and deducing information about someone from analytics, are more frequently

disliked. Previous research has also tested whether varying the copywriting

would also affect consumer behavior, within the same ad but with different

disclosure designs. So in one design a group of participants saw an ad that had

the following copywriting - you are seeing this ad based on information that you

provided about yourself. A second group of subjects saw - you are seeing this ad

based on information that we inferred about you and a control group saw the

business as usual no disclosure ad. Participants who viewed the ad framed as

inferred behavior analytics showed much less interest in purchasing

than the other groups did. Also, booking.com tested variations of

copywriting to check which was more effective to increase conversions in an

email marketing campaign. In their case the less intrusive variation was less

effective than the one obtained by an LDA model, a type of natural language

processing applied to the users reviews. This variation said that based on your

past trips their team of travelers scientists thought that you probably

have a passion for a romantic landscape, local food or shopping. I find this

example interesting because it shows how important it is to test different

variations of copywriting and looking at how humans react to them. Artificial

intelligence is helpful here because machine learning improves our ability to

predict what person will respond to what persuasive technique, for which channel

and at which time. This combination between behavior analytics

and automation is now called digital nudging. For example, digital nudging can

help some companies to reframe their services within a personal advice

approach. This can make it easier for them to acquire customer data. Also, it helps

to comply with the GDPR regulations, concerning privacy issues and

information sharing. If you want to know more we have a video about that so check

the link in the description. Also in a recent podcast hosted by the channel

behavioural grooves, Rebecca blank the chief behavioral officer at merits,

discussed in detail how customers might react differently, according to the way

companies communicate with them through personal content. In summary there are

three takeaways. Any disclosure is less creepy and will convert better than no

disclosure. Deduced information on the customer will convert less than an open

explanation of why they are seen a specific ad and finally, this might seem

corny, but trusts is a key factor in dealing with this new world of shared

information and data gathering. In conclusion we see that personalization

is an efficient way to influence consumer behavior, especially when it's

powered by artificial intelligence. But we have to mitigate backlashes by

testing different layouts, image and text that provide information on how your

personalized content was generated. What do you think about data privacy? For

example I think it's psychological profiling but please let us know your

opinion in the comments below!

How to Write the Perfect Professional Email in English - Duration: 3:42.

8 Tips for Writing Emails in English

Writing an email in English isn't as simple as translating word-for-word from your native language.

There are many English expressions you should know how to use in your professional emails.

To avoid awkward sentences and literal translations,

here are some tips on good email style and content.

1. Remember the KISS rule

That means: Keep It Short and Simple.

Basically, try to avoid long and complex sentences.

Aim for an email that is short, clear, and concise.

2. Stylistic tips

No surprises here! Like in other languages, there are 4 things to consider:

1. the "sender" (from)

2. the "recipient" (to)

3. the subject, and 4. the body.

We suggest writing the main body first, then picking out key words for your subject line.

Your subject should not be longer than 8 words. Remember, KISS.

3. Content tips

- 4. Who are you writing to?

Always start by addressing a specific person if possible,

using their first name, last name, or title.

If you know the person well, write "Hi Mark" or "Dear Mark."

If you have never met, it's best to use "Mr. / Ms. Smith" or "Dear Mr. / Ms. Smith."

If you do not know the person's name, you can use "Sir" (man) or "Madam" (woman).

- 5. How to begin an English email

If you have never spoken before: don't bother with a long introduction.

Keep it short and tell the person how you got their contact information: "Your name was given to me by..."

To respond to someone, acknowledge their previous email: "Thank you for your email from... (date)"

To refer to a previous email or phone conversation: "In reference to your email..."

If you are writing to provide information, make your intentions clear: "I am writing to inform you that..."

To ask for information, start with: "I wish to have further information about..."

- 6. The body

Don't translate word-for-word!

Use set phrases and common questions...

you can learn lots of these with our Learn English app!

And don't forget: keep your sentences short.

Good practice: one idea per sentence.

- 7. Sign off

If you are expecting a response, you can write: "I look forward to hearing from you."

To show that you care: "Don't hesitate to contact me with any questions"

or "If you need / would like more information, feel free to contact me."

Finally, end your email. Some examples: "Yours sincerely," "Kind regards," or "Best wishes."

8. Don't forget about attachments!

It's something everyone does every now and then.

Announce any attachments using one of these set phrases:

"Please find enclosed / attached..." or "Attached, you will find..."

Я СВАЛИВАЮ В КИТАЙ! l VCIGO K3 150W Kit l by Sigelei l ENG SUBS l Alex VapersMD review 🚭🔞 - Duration: 13:00.

Learn How to Introduce Yourself in English | Video with Subtitles - Duration: 5:45.

Hi Bob the Canadian here.

Sometimes when you're in a classroom or a when you're with a group of people, you

find yourself needing to introduce yourself.

Well I'm pretty sure you already know how to say, "My name is….Bob the Canadian,"

so in this video I'll look at a few more things you might be needing to share about

who you are with a group of people.

Hey welcome to this video, if this is your first time here, don't forget to click the

subscribe button below, and at some point during the video give me a thumbs up!

So, you have to introduce yourself.

Maybe you're in a class for the first time and the teacher wants to go around the class

and have each person introduce themselves, and say something about themselves.

So, again, I'm pretty sure you already know how to say, "My name is…" or "I am…"

and if you didn't those are the two ways to tell people what your name is.

My name is Bob the Canadian, or, I am Bob the Canadian.

But there's a few other pieces of information that people might be interested in.

So let's take a few minutes to look at each of those.

The first thing they might want to know is where you are from, or where you were born,

because they might be different things.

For instance you could say, "I was born in Canada, right now I live in a town called

Hamilton."

You could say, "I was born, perhaps, in France, but right now I am living in the city

of Toronto.", or, maybe you were born and live in the same city, and you could say,

"I was born and raised in the city of Toronto, and I currently still live there."

So that's one more additional piece of information you could give people.

You could say, "Hi!

My name is Bob the Canadian.

I was born and raised in Hamilton, Ontario and I still live there."

Another thing you might wanna tell people, this is up to you if you wanna share it, is

how old you are.

So you could say, "I am 46 years old."

You could tell them how old you are but not everyone wants to share their age the older

people get.

Sometimes they don't want to say exactly how old they are, so they might say something

like, "I'm in my late forties." or "I am around forty-five years old."

or "I am in my forties."

So going back to the beginning I could say, "Hi!

My name is Bob the Canadian.

I'm from Hamilton, Ontario and I am in my late 40's."

Because I don't quite wanna say exactly how old I am because I'm starting to feel

older every day.

So that's another thing, another piece of information you could share with people.

You could tell them how old you are, or about how old you are.

Another piece of information that you might want to share with people is your marital

status, and whether or not you have children.

Marital status refers to if you are married or not.

So I might also add to my description that I am married and I have five children.

Or I could extend on that and say, "I have been happily married for twenty years, and

I have five kids.

So going back to the beginning, you could say, "Hi!

I'm Bob.

I'm from Hamilton, Ontario.

I'm in my late forties, and I'm happily married and have five children.

The other thing you might wanna share with people is what you do for a job.

So you might say to them, "I am a currently a high school teacher, I am a construction

worker, I am a truck driver, or I currently work in a hotel."

So to put it all together again, "Hi I'm Bob the Canadian.

I'm from Hamilton, Ontario.

I'm in my late forties.

I'm happily married with five children and I currently work as a high school teacher."

So those are five things that you can use yourself to introduce yourself, it's really

good to be prepared for this.

It's really good just to get a piece of paper and write down five or six things that

you're willing to share about yourself, so that when you're in a situation you can

actually describe who you are really nicely.

The other thing it does, which is very helpful, is it prepares you in case somebody asks you

these questions.

So if someone was to say, "What's your name?"

You could say, "My name is Bob."

If someone was to say, "Where are you from?"

You could say, "I'm from Hamilton, Ontario."

If someone was to say, "How old are you?"

You could say, "I'm in my late forties."

If someone was to say, "Are you married?"

You could say, "I'm married with five children."

If someone was to say, "What do you do for a living?"

You could say, "I am a high school teacher."

Well, Bob the Canadian here, I hope you enjoyed this short video.

Please give me a thumbs up if you liked it, subscribe below by clicking the red button

and the bell icon, and for sure share this video with someone.

Bob the Canadian here, learn English with Bob the Canadian.

I hope you have a great day!

Trump says U.S.-North Korea summit will happen in May or early June - Duration: 2:25.

U.S. President Donald Trump has confirmed that his meeting with North Korean leader

Kim Jong-un will take place as agreed,... although it may happen later than originally

anticipated.

Trump also spoke about the chance for the two sides to make a fresh start after years

of going toe-to-toe.

Our Lee Seung-jae has more.

President Trump says he plans to meet with North Korean leader Kim Jong-un next month,

or in early June,... in order to "hopefully" reach a deal on denuclearizing the regime.

Trump made the remarks at a cabinet meeting on Monday,... expressing optimism over the

talks,.. and what the summit could mean for the future.

" North Korea by the way as you've probably seen, we've been in touch with North Korea,

we will be meeting with them sometime in May or early June.

And I think there will be great respect paid by both parties and hopefully we'll be able

to make a deal on the denuking of North Korea.

They've said so, we've said so, hopefully it'll be a relationship that's much different

than it's been for many many years."

This comes after the White House had previously said President Trump would meet with Kim Jong-un

before the end of May.

Trump said direct talks should've happened years ago,... but stressed that he was looking

forward to the summit.

"This should have been done by other presidents and they decided they didn't do it.

They couldn't have done it, but it would have been a lot easier if it were done five years

ago, 10 years ago, 20 years ago, a lot earlier than now.

But we have a meeting that is being set up with North Korea, so that will be very exciting

I think for the world."

Over the weekend,... the White House confirmed North Korea had told the U.S. that it was

willing to discuss the denuclearization of the Korean Peninsula,... offering a clearer

picture of the agenda for the talks.

However observers remain cautious despite the easing tensions,... with John Bolton officially

starting his position as Trump's National Security Advisor on Monday, replacing H.R.

McMaster.

Bolton had previously outlined his advocacy of an attack on North Korea.

Do You Ever Need Both Surgery and Radiosurgery? Chapter 16 — Brain Metastases: A Documentary - Duration: 0:56.

With surgery, we can never guarantee that we've removed 100% of the tumor.

Sometimes, even when we remove everything we can see on the MRI,

we know that there are microscopic cells left behind.

The data is very clear on this that patients do better if they receive radiosurgery

to the area where we've surgically removed a tumor.

Treating with radiation before actually doing surgery is really an intriguing idea

that's starting to gain some traction.

The concept is that if we treat with radiation before we do an operation,

we start killing tumor cells with the radiation, so that when we go in there to remove the tumor,

much of the tumor is already dead and the cells that get spilled during the operation

can't spread and set up new tumors. When we treat with radiation before surgery,

it's much easier to draw a line around the boundary of the tumor and know exactly

what the border between the tumor and the normal brain is.

Where I've been - Duration: 4:10.

Oh my god, Cassie where have you been?

Howdy long time no see

I suppose you probably are wondering where I've been. Allow me to explain.

To begin with, I am no longer a 백수 (unemployed person). I am no longer unemployed.

I serendipitously got offered a job at the same company that I used to learn Korean starting way back in 2009

Talk to me in Korean.

I honestly... I can't... I can't even believe that I can call myself part of the team,

but yeah, that's what I do now and

what I actually do, as for what I really do at my job, what my job entails

you know, all of those things,

it's kind of a long story like, you know, how I got the job and everything... there's a lot to talk about so

I'm not gonna get into that right now, but yeah, I've been doing that since June of last year

And that's what's been taking up the majority of my time

and along with the job

you know, as most of you know if you have watched my channel before you know that I am kind of a loner,

it's partially by circumstance and a very large part by choice

But because of that I do everything alone

everything that is involved with a video I do all on my own because my channels still so small

I can't really afford to hire any help

I don't have anyone to help me edit or anything like that

the second reason is that when I began youtube I was going through

The worst depressive pit in my entire life.

I was undergoing some really Intense life changes,

and I just didn't know how to cope so you know when you have

This like the proverbial cloud of depression

Just weighing down on you,

you know, it's it's hard for your true self to shine through so I have

such a hard time looking back at my old videos. I just...

They're cringe-worthy

honestly, because at the time I was forcing things

It's not to say that I was being fake, because I am a happy bubbly person normally

but with that cloud you have to kind of

push those parts of your personality out there and unfortunately it shows

So you could see it in my face and everything and you know

It's just... it's hard to be your normal self .

so because of that this past year. I've been focusing on health.

Particularly mental health and I've also been focusing on physical health.

So, last year I went into therapy. That was immensely helpful,

I was able to learn some coping skills so that even though I'm not in therapy now if I ever have a bad day,

I know how to cope with it,

so yeah,

It's just my focus for the past year has been on health and recovery.

When you're focusing on something so big like that, you know,

other things just get kind of pushed to the wayside and YouTube was one of those things for me.

I can't guarantee that I will upload regularly

although I do have lots of ideas

because

third reason for why I haven't posted

Even though I have filmed

dozens of videos is

Because I felt like I was doing it

Too much to please other people I was doing things that I thought other people would enjoy

even though they were things that I enjoyed myself, but

I cared too much about what other people think.

From now on I'm gonna keep it as real as I can. I'm gonna do things that I enjoy and I hope that by

Doing things I enjoy it will shine through and you will enjoy it as well.

So...keep it real.

How are Patients Immobilized During Radiation? Chapter 8 — Brain Metastases: A Documentary - Duration: 3:13.

The benefit of frame-based radiosurgery is that the patient is held rigidly on the table

with the assurance that the patient isn't moving and that the target

is going to be accurately identified. Frameless radiosurgery uses a

thermoplastic mask that is custom-molded to the patient by heating this plastic,

molding it and then cooling it so that it becomes rigid.

It still holds the patient on the table but does still allow about a millimeter to two

millimeters of micro motion. That's much more comfortable than being rigidly

affixed to the table, but it does introduce the possibility of some error.

There are frameless technologies that track the surface of the patient and use

that to then somehow triangulate an internal target that you can't see.

Optical external surface tracking technology can be quite compatible to

various sites, for example, in treating breast cancer. In treating brain tumors

we have the most demanding spatial tolerances of any site within the body.

The sensitivity of the technique can be affected by skin color. That skin can move,

it can deform, it can stretch and ultimately that's the limitation of that

surface matching approach. One option to accommodate limited precision of

surface matching is to actually add a margin around the tumor that we're

treating, but it's our goal to actually minimize those margins since that

expansion equates to knowingly treating healthy brain tissue.

If we can imagine for a second a brain metastasis that is 15 millimeters across,

if we were to add a margin that is just 2 millimeters, we would be approximately doubling

the volume of the tumor that we would need to treat. What we really would

like to do is have a more direct means of monitoring where the tumor is actually located.

With an image-guided frameless technique like ExacTrac in real-time you actually see where

your beam is pointing. So you can constantly monitor the patient if they were to move,

for some reason, cough, jump, whatever, you know it and you can adapt your treatment to that.

When you add imaging and micro motion adjustments into the

thermoplastic mask, you really achieve the same level of accuracy as a frame-based system.

This can be done through a system such as ExacTrac, that uses

in-line X-Ray imaging combined with a robotically moving couch to make minor

adjustments in patient position to account for any movement of the patient.

In my opinion, treating hundreds of patients with both frame-based and

frameless technologies, the patient and provider experience is better with the

frameless immobilization. From the patient's perspective, they don't have to

have a painful anesthetic injection, they don't have to be sedated, they don't have

to have the discomfort of wearing the frame for the entire day of the treatment.

They are only immobilized while they're on the treatment table under treatment delivery.

Then it doesn't matter if you're giving one treatment, three treatments, five treatments, you can

get the same accuracy and you can choose the number of treatments that's best for the patient and their tumor.

What is the Future of Brain Metastases Treatment? Chapter 18 — Brain Metastases: A Documentary - Duration: 3:07.

It's very early on in a movement to really address the treatment of

metastatic cancer and metastatic brain, in particular.

The importance of a film like this and the American Brain Tumor Association being a part of it is,

it really gets back to our mission. If people understand that there is time for them

to understand their tumor, that there is time for them to understand that diagnosis,

and there is time for them to better understand their treatment options,

it's a better trajectory, it's a better outcome and I think that that is

a mission fulfillment for the organization.

If we give treatments that are toxic and cause cognitive decline and those patients won't be able to

enjoy the extra years that we've bought them with our new therapies and so when treating cancer

patients in the modern era, we really have to consider the quality of life for the time they have left.

We want for people to continue to live, continue to work, continue to do all the things that are meaningful for them.

Radiosurgery is an essential component to the next wave of cancer care.

I no longer think about dying, you know, I don't.

And that's, I think, pretty dramatic.

If we can give you that plateau of survival, imagine what new protocols might be available

to you that weren't available even three years ago and that provides patients with realistic hope.

Technology has evolved to the point where we can treat them quite well and often cure

their intracranial disease. Most patients with cancer who have brain metastases

are going to find that their ultimate prognosis is really dependent on what's

going on with the cancer in the rest of their body. The future is really bright

for these people because our systemic therapies are really progressing and

survival is better than it ever has been in the past.

I think the future is really going to be changing cancer from a fatal disease to a chronic disease.

When is Radiosurgery Performed More than Once? Chapter 12 - Brain Metastases: A Documentary - Duration: 2:57.

Increasingly, we're faced with more complicated, complex tumors, larger tumors, and tumors that are

in close proximity, they are very close to critical structures, for instance, say the optic chiasm

or the brain stem or one of the major cranial nerves. Giving a single fraction would be problematic,

it would cause too much of a side effect. And so, the technologies that we have today with our relocatable

frame, with our image guidance and our ability to vary the intensity of the radiation beam, has allowed us

to give an effective dose of radiation by breaking it up into smaller fractions,

something called "hypofractionation". For instance, for the smallest tumors, we may give 21 or 24 Gy,

and that's a measure of radiation, in a single fraction. But for a tumor that's say 3cm,

we may give you a much lower dose, which is 16 Gy and that doesn't make any sense

because a bigger tumor will have more cells that need to be killed.

So why are we giving a lower dose? It's because, if we tried to give 24 Gy to that 3cm tumor,

the side effect, meaning swelling and necrosis, which is dying brain tissue, would be unacceptably high.

In between the treatments, the normal brain can repair. In between the treatments, some areas

of the tumor that were more resistant to radiation, maybe because they weren't getting enough oxygen,

can get more oxygen and become more sensitive to radiation.

All cells in the body require oxygen

and tumors themselves have a higher demand on oxygen consumption

just because they're growing so rapidly.

They can actually outgrow their blood supply.

And when they outgrow their blood supply, they become what's called 'hypoxic'.

So there is little oxygen towards the centers of the tumor.

Incidentally, by giving a smaller dose of radiation but giving it in multiple fractions

we'd get the benefit of having the tumor shrink and then that portion that was

hypoxic actually has now more blood supply and as it further shrinks it gets

even more blood supply, so the radiation is now more effective

because there's more oxygen around, there's more free radical to damage the cancer cells

and you get a better response to treatment.

For some large tumors, the future might be to be giving three and five treatments

as opposed to just one treatment and again, if you have a system that

doesn't rely on screwing something to the patient's head, then you have that flexibility

of what's best for this tumor. If it's three treatments, we'll give three treatments.

If it's one, we'll give one, and the quality and the accuracy will be the same.

What Happens After Radiosurgery? Chapter 14 — Brain Metastases: A Documentary - Duration: 1:02.

If you just give someone radiosurgery and you never see them again

well, unfortunately, it's about 50/50 that they'll get new brain metastases and if

you don't follow them, the only reason you'll see them back is because they're sick and in the emergency room.

But if you see them every few months and you do an MRI, you might find that new metastases when it's only

a few millimeters and you can zap it before it causes any trouble and often the patients,

despite having advanced cancer and brain metastases, they're not going to die from the brain

metastases because we can control them by following them and treating them when they're small.

And that's really part of managing cancer as more of a chronic illness with these periodic tests.

It's been part of the care of the body, PET scans, periodic CT scans, it needs to be part of the care of the brain.

If you have a garden and you notice a couple of weeds,

you can either go and pull those couple of weeds or you can tear up your entire lawn.

There might be two or three other weeds that you didn't see. If they grow up later, you'll just pull them out later.

What are Brain Metastases? Chapter 1 — Brain Metastases: A Documentary - Duration: 3:06.

Cancer.

The emperor of all maladies.

It is a frightening disease for which currently there's no cure.

However, we are unquestionably in one of

the most dynamic and exciting periods of medicine for cancer treatment.

I felt that something was a little off with my right breast.

It turned out to be breast cancer.

I went through seven months of chemotherapy, a mastectomy, radiation.

I got to a point where there was no evidence of disease.

My oncologist suggested a brain scan, because I hadn't had that tested

since my original PET scan when I was first diagnosed.

There were three tumors.

A brain metastasis is a collection of cancer cells that have traveled from some part of the body where it started.

They cause a lot of swelling and that pressure on the brain and the swelling in the brain causes

neurologic symptoms and can actually even cause death if not treated appropriately.

My first thought was OK, you know, what's going to happen next?

What's my quality of life going to be and - actually I was prepared to die.

It's terrifying. They're scared that they're going to die soon.

They are scared of the treatments that we might be offering them.

We did a study a number of years ago where I took my own patients

and I tried to predict how long somebody would live. We shared that data with

17 top people in the world. It was fascinating because what we found is

that the doctors really couldn't pick, at an individual level,

who was going to beat the odds and who was going to struggle.

Maybe we're not that good at predicting how long people are going to live.

We'll find the cure for cancer eventually but in the meantime, you know, we want people to be able to

live with their cancer, as opposed to for their cancer.

We now have capabilities in radiosurgery to treat tumors

that are in deep and inaccessible areas with almost relative impunity.

We can do it with such targeted approaches

that the side effects are no longer significant.

This has revolutionized neurosurgery and radiation oncology because prior to that

there was only the options of open surgery or radiation to the whole head, as opposed to precise targets.

The advantage of radiosurgery, by focusing radiation, is sparing the surrounding

structures in the brain from radiation injury.

Patients are surprising us. They are living longer, they're beating the odds.

We're seeing things we never saw before.

We have a high confidence that we can eliminate the cancer in the brain

and return you to your quality of life and activities of daily living.

What is Whole Brain Radiation Therapy (WBRT)? Chapter 4 — Brain Metastases: A Documentary - Duration: 2:29.

Whole brain radiation is as it sounds; We're treating the entire brain with radiation.

Conventional radiation, the way it's been given for a long time, is that

a little bit of radiation is given every day and that's repeated five days a week

up to seven or eight weeks. In that process, there's a lot of healthy cells

that receive the same amount of radiation as the cancer cells but the

healthy cells are better at repairing between the different radiation treatments.

So if you give a treatment today and you give another one 24 hours later between those two

treatments the healthy cells are repairing at a faster rate than the cancer cells.

Unfortunately when a patient is diagnosed with brain metastases, there are

many more lesions that are not detectable on an MRI or any imaging modality

that we have today. They can have the potential to grow.

Some have referred to this as the "dandelion effect". When you blow on a dandelion all these

seeds shower an area. It's not dissimilar to that phenomenon where there is a

primary cancer somewhere in the body and therefore there's a possibility for

cancer cells to seed and deposit multiple areas of the brain.

So whole brain radiation therapy is used to try to sterilize not just the visible brain metastases

but also the microscopic brain metastases that we just can't see yet.

Radiation has been around for about a hundred years and it's been a really important part of treating cancer.

For some people that's an appropriate treatment but it comes with a lot of side effects.

Almost everybody's tired, sometimes really severely tired. You lose your hair. You can get inflammation in

your ear, sometimes that leads to hearing loss. It makes people nauseous sometimes.

What worries patients the most and what causes us the most concern is it

can have an effect on memory, concentration, and other brain functioning.

Whole brain radiotherapy has a devastating impact on cognition.

We're targeting the areas where new memories are formed, the hippocampus,

as well as all of the interconnections between different parts of the brain

that are critical for higher-level thought and memory.

You can't expose that much of the brain to toxic radiation and expect to have no consequences.

So if we are going to strive for a cure or at least long-term suppression of cancer

and keep patients alive for years and years, we really have to think about

what their quality of life is going to be and quality of life starts with what's up here.
