Year after year Americans watch their cost of living go up but their Social Security

check stays the same.

Even the recent increase, thanks to President Trump and a booming economy, is not keeping

up with the cost of Medicare.

And now a new report shows that Congress is cheating Americans out of proper increases

so their payments will never keep up with cost of living.

Democrats claim that Republicans want our seniors eating dog food, but the reality is

both parties of Congress are complicit.

They need to start working together to take care of people who have paid into Social Security

faithfully over the years.

(Reuters / Fred Prouser) Americans receiving Social Security benefits

in 2018 can expect to see the program's largest payment increase in years, according

to the trustees report released Thursday.

In January, recipients will receive a $28 monthly increase in benefits, which, although

amounts to just 2.2%, is much larger than the 0.3% increase in benefits recipients collected

this year.

In 2016 the program offered no payout increase.

The average monthly payment for the program's 61 million beneficiaries is $1,253.

Despite the good news, Social Security and Medicare are still dealing with looming cash

shortfalls.

OASDI maintains funding levels will run dry by the mid-2030's, with Medicare Part A

projected to be depleted in 2029—one year later than projected in last year's analysis.

If Congress allows either fund to be depleted, millions of Americans living on fixed incomes

would face steep cuts in benefits.

Neither Social Security nor Medicare faces an immediate crisis.

But the trustees warn that the longer Congress waits to address the program's problems,

the harder it will be to sustain Social Security and Medicare without significant cuts in benefits,

big tax increases or both.

"Lawmakers should address these financial challenges as soon as possible," the trustees

wrote in their report.

"Taking action sooner rather than later will permit consideration of a broader range

of solutions and provide more time to phase in changes so that the public has adequate

time to prepare."

Republicans in Washington have long clamored to address the long-term financial problems

of Social Security and Medicare, the largest benefit programs run by the federal government.

But don't expect them to do much about it.

Over the years, House Speaker Paul Ryan, R-Wis., has insisted on overhauling those programs,

proposing a voucher-like system for Medicare and calling for partially privatizing Social

Security.

Now that Republicans control Congress and the White House, Ryan says he doesn't want

to tackle Social Security.

Instead, Republicans and the White House are focused on repealing and replacing former

President Barack Obama's health care law.

President Donald Trump has promised not to cut Social Security or Medicare, though his

budget proposal for next year would reduce Social Security's disability benefits by

nearly $70 billion over the next decade.

The savings would come from encouraging, and in some cases requiring, people receiving

the benefits to re-enter the workforce.

But even if Trump finds the savings, it wouldn't come close to solving the program's long-term

financial problems.

Social Security is independently funded by payroll taxes, so it is not subject to annual

spending bills approved by Congress.

AARP hopes it stays that way.

"Social Security should remain separate from the budget.

Medicare can improve if we reduce the overall cost of health care, rather than impose an

age tax, and if we lower prescription costs, instead of giving tax breaks to drug and insurance

companies," said AARP CEO Jo Ann Jenkins.

Over the past decade, Social Security and Medicare made up about 40 percent of federal

spending, excluding interest on the debt — and that share is projected to grow in the future,

according to the nonpartisan Congressional Budget Office.

Blast the bonding surface of the Ti base with aluminium oxide.

Observe the instructions of the Ti base manufacturer.

Remove the silicone or wax and clean the Ti base with ultrasound in a water bath or with a steam jet.

Apply Monobond Plus bonding agent to the dry bonding surface of the Ti base

and allow to react for 60 seconds.

Then, evaporate any remaining excess with compressed air that is free of water and oil.

Seal the screw channel with a foam pellet or wax. Be careful not to contaminate the bonding surface.

The IPS e.max CAD ceramic structure must also be conditioned before cementation.

Under no circumstances should the ceramic structure be sandblasted.

Apply IPS Ceramic Etching Gel to condition the bonding surface

and allow to react for 20 seconds.

Then, rinse off with water. Blow dry the restoration.

Apply Monobond Plus to the dried bonding surface and allow to react for 60 seconds.

Then, evaporate any remaining excess with air that is free of water and oil.

Now cement the IPS e.max CAD ceramic structure to the Ti base

using the self-curing Multilink Hybrid Abutment luting composite.

Apply a thin coating of Multilink Hybrid Abutment directly onto the bonding surface of the Ti base.

Then, apply a thin coating of Multilink Hybrid Abutment

to the bonding surface of the ceramic structure.

Next, place the ceramic structure over the Ti base

in such a way that the position markings are aligned.

Verify the correct final position at the transition between the Ti base and ceramic structure.

Then, press the parts firmly together for five seconds.

Carefully remove excess Multilink Hybrid Abutment from the screw channel with a microbrush

or brush using rotary movements.

Now, carefully remove the foam pellet from the screw channel.

Remove circumferential excess cement only after the initial setting phase,

which is two to three minutes after mixing.

For the clean-up, use a suitable instrument, such as a Le Cron.

Apply glycerine gel such as Liquid Strip to the cementation joint

to prevent the formation of an inhibition layer before the luting composite autopolymerizes.

It takes Multilink Hybrid Abutment approximately seven minutes to autopolymerize

Secure the components in the correct position to one another and do not move them.

Once autopolymerization is complete, rinse off the glycerine gel with water or steam.

If cement residue is left in the screw channel,

remove it with suitable rotating instruments.

Be careful not to damage the Ti base.

Carefully polish the cement joint using rubber polishers, for instance OptraFine,

at a low speed of no more than 5000 revolutions per minute to prevent overheating.

Carefully place the ceramic structure on the Ti base and check the fit.

Please note that you should not adjust the contact surface to the Ti base as this may compromise the fit.

Smooth out the attachment point. Take the shape of the emergence profile,

crown margin and proximal contacts into consideration.

Surface-grind the occlusal surface with a fine diamond

to smooth out the surface structure created by the CAD/CAM procedure.

A clinical try-in may be performed at this stage to check the fit of the ceramic structure.

For this purpose you should temporarily secure the ceramic structure to the Ti base

using silicone material, for instance Virtual Extra Light Body Fast Set.

Seal the screw channel of the untreated Ti base with a foam pellet.

Insert the Virtual cartridge into the dispenser.

Screw on the mixing tip and attach the Oral Tip.

Apply Virtual Extra Light Body Fast Set to the Ti base and to the ceramic structure.

Introduce the Ti base into the ceramic structure.

Firmly hold the components in place for approximately two and a half minutes

until Virtual Extra Light Body Fast Set has set.

Carefully remove protruding excess material using a suitable instrument such as a scalpel.

Also remove the foam pellet and excess Virtual Extra Light Body Fast Set from the screw channel.

You are now ready to carry out the clinical try-in.

Place the temporarily secured hybrid abutment crown on the implant.

Caution: Do not perform any functional occlusal checks at this stage.

Check the proximal contacts and, if necessary, adjust them using suitable grinding instruments

Manually screw in the dedicated implant screw.

Check the geometry and fit of the hybrid abutment crown in relation to the gingival margin.

if adjustments are necessary, unscrew the implant screw and remove the hybrid abutment crown.

Amendments by grinding should be done extraorally.

Now check the occlusion and articulation.

Make adjustments using suitable grinding instruments, as necessary.

Once the clinical try-in is completed, take out the implant screw

and remove the hybrid abutment crown.

Clean the IPS e.max CAD ceramic structure with a steam cleaner

to remove any contaminations and grease residue.

Only use the IPS e.max CAD Crystall./ Glaze, Shades and Stains to glaze and characterize the ceramic.

If necessary, dilute the glaze paste with a small amount of IPS e.max CAD Crystall./Glaze and Stains Liquid

Use the same method for the IPS e.max CAD Crystall./Shades and Stains materials.

Apply the glaze paste in an even coating onto the outer surfaces

and the occlusal surface of the hybrid abutment crown.

Make sure that no glazing material reaches the bonding surface to the Ti base

or the screw channel as this may compromise the accuracy of fit.

Characterize the emergence profile using IPS e.max CAD Crystall.

/Shades and Stains and imitate the natural dental roots.

If you would like to enhance the chroma on the buccal surface,

apply IPS e.max CAD Crystall./Shades or Stains.

For imitating the incisal area, apply IPS e.max CAD Crystall./Shade Incisal to the incisal aspect.

If you would like to apply minor shape adjustments, for instance at the proximal contacts,

use IPS e.max CAD Crystall./Add-On.

Mix a small amount of IPS e.max CAD Crystall./Add-On-powder

with a small amount of IPS e.max CAD Crystall./Add-On Liquid

to achieve a consistency that is easy to contour.

Apply the mixture directly to the areas in need of adjustment on top of the unfired glaze.

Perform the crystallization firing in a Programat furnace

for instance a Programat CS2 with preinstalled program settings.

When selecting the program,

pay attention as to whether the ceramic structure has been milled from an LT block or an MO block.

Place the ceramic structure in the centre of an IPS e.max CAD Crystallization Tray

and place the tray in the centre of the furnace.

Start the selected program.

Once the firing process is completed, remove the IPS e.max CAD Crystallization Tray

and the ceramic structure from the furnace.

Allow the restoration to cool to room temperature in a place protected from draft.

Remove the IPS e.max CAD Crystallization Pin XS from the ceramic structure.

Remove residue adhering to the ceramic structure either in a water bath or with a steam jet

Do not use aluminium oxide or glass polishing beads

to remove IPS Object Fix Putty or Flow residue.

At the end, check the fit of the crystallized, glazed and characterized ceramic structure on the Ti base.
