Intravenous Corticosteroid Premedication Administered 5 Hours before CT (November 2017 Radiology) - Duration: 10:49.

This is Herb Kressel and welcome to the Radiology podcast.

Today I am joined by Dr. Matthew Davenport from the Department of Radiology and the Department

of Urology at the University of Michigan Health System.

Dr. Davenport and his colleagues authored a provocative study entitled "5-Hour Intravenous

Corticosteroid Premedication as a Breakthrough Reaction Rate that is Non-inferior to that

of the Traditional 13-Hour Oral Regimen."

Welcome Dr. Davenport.

Matthew S. Davenport, MD Thanks for having me.

Yes and I should note all of your many titles.

You're the Associate Chair of Quality and it sounds like this is relevant to that duty;

you're the Director of Body Imaging; and Associate Professor of Radiology and Urology.

And in addition to those heady titles you currently chair the ACR Committee on Drugs

and Contrast Media.

I couldn't think of a better person to discuss this manuscript.

So welcome.

Thank you for having me.

Sure.

Now what are the current ACR recommendations regarding pretreatment with corticosteroids

for those patients who are at risk for allergic type reactions and who are scheduled for contrast

enhanced CT exams?

So we are currently in version 10.3 of the manual.

At the time that this paper was done we were in version 10.2, and the reason why that's

relevant is between those two editions the guidelines actually changed a little bit.

So our current guidelines are for patients at risk which is defined as a patient who's

had a prior allergic type reaction to contrast media of the same class meaning they previously

had a reaction to iodinated contrast, they're now getting iodinated contrast again, general

class, then steroid medication would be indicated for that patient.

And traditionally it is given over a 12 or 13 hour period in oral format and people have

tried using faster IV regimens just because logistics have demanded it, but to date there

really wasn't any significant studies to justify that behavior.

Okay can you just kind of review sort of what is the underlying evidence base that this

actually is a reasonable approach to these high risk patients?

The best evidence we have are from two randomized trials by Lasser, et al and they did two of

them.

One was in 1987 studying high osmolality contrast media that we don't use anymore for intravenous

administration, and they tested average risk patients which means just the general population

getting contrast and getting steroids prevented reactions of all severities.

So that seemed like a pretty good thing.

Then in 1994 they repeated the same study design but due to monetary reasons or logistics

or whatever, they were under powered to talk about moderate and severe reactions.

So although again they tested average risk patients, they showed a reduction in aggregate

reaction rates and mild reaction rates they were unable to show differences in moderate

and severe reaction rates.

Some people criticized the evidence base for that reason because we don't have proof

that giving a high risk patient steroid works and we don't have proof that giving average

risk patients moderate contrast media reduces the risk of moderate or severe reactions.

Most experts, not all, believe that there probably is an effect and it's not the question

of whether it works, it's a question of effect size, how many preps do you need to

give so that somebody doesn't die.

Sure.

So what is – now you focused on the specific issue of the 5-hour oral prep so why did you

decide to do that study?

So traditionally we had a 12 or a 13-hour oral prep and what we compared it to was a

5-hour IV prep.

So a different mode of administration in over a shorter time period, and the rationale for

that is there was a case series of like 9 subjects from the 1980s that tested something

like this and found they didn't have any reactions during that tiny cohort.

And based on that very tiny shred of evidence, the ACR has stuck this regimen in its contrast

manual for a long time and we were curious to know whether it actually worked.

Right, right.

It sounds like a good idea.

So this is actually as you've mentioned a non-inferiority study.

Can you explain the study design for viewers and listeners who may not be familiar with

this term?

What is the design and when is it appropriate?

So the average hypothesis driven study that someone would do uses what's considered

superiority assumptions for their power calculation.

I want to know whether X is better than Y and sometimes we end up with a negative result

in those cases but then we don't have a lot of confidence the negative result means

anything.

That's what leads to all this publication bias where only significant results get published

and non-significant results don't.

You don't have too much confidence the negative result means something.

The power study using non-inferiority assumptions or equivalents which is even a higher standard,

you're trying to say up front I believe that regimen A is non-inferior or at least

as good as regimen B and you would assign the power appropriately.

And when you do that you say what risk threshold am I willing to tolerate.

And in this case we knew the breakthrough reaction rate was somewhere in the 2 to 3

percent range with the 13-hour oral prep.

And we said how much are we willing to tolerate if this risk for this other regimen, this

5-hour IV regimen, is higher?

And we say well about maybe four percentage points.

We don't want to go higher than six percent.

We think six percent will be within a clinically acceptable range or breakthrough reactions.

So we set our confidence intervals upfront say how much width on our confidence are we

willing to tolerate and we power it appropriately.

So non-inferior design goes into it saying regimen A is not worse than regimen B. It's

like a one-tailed test.

So in a lot of the non-inferiority designs we've seen the margin, non-inferiority margin,

is drawn from prior literature or something that gives you confidence and yet you did

it based on sort of clinical intuition it sounds like that if it was beyond this we

would really worry that we wouldn't want to use it; so it was just clinical judgment.

Yeah I think when you choose a non-inferior margin the clinical judgment is actually more

critical probably then what the literature says because you want to know what are you

willing to tolerate if this is actually worse than regimen A how much risk can you tolerate

from a clinical standpoint.

Sure.

Now you used a statistical test that I wasn't familiar with, the Farrington and Manning

non-inferiority likelihood score for you figure of merit to assess non-inferiority.

Can you tell people viewing and listening what this is?

They may also not have been familiar with it.

So I am not a statistician, so I'll get that right out front, but basically it's

a way of doing non-inferiority tests comparing two proportions where you look for the difference

in the rates.

So let's say rate A is 4% and rate B is 2% and you establish up front an expected

(inaudible) and a difference between the eventual rates and you set the threshold appropriately.

So in this case the upper bound on our threshold was 4%.

So it's appropriate when what's being compared is a percent rather than an absolute

number.

Um hum.

Good.

Okay so now we understand all of that, what did you actually find in your study?

So essentially we found that if you give patients who are at risk, meaning they've had a prior

allergic type or unknown type reaction to iodinated contrast media and you give them

premedication with a 13-hour oral regimen, or you give them premedication with a 5-hour

IV regimen, their rates are statistically indistinguishable.

In other words, the 5-hour IV rate is non-inferior from the 13-hour oral rate.

Which, if true, is important because it really would change how patients get premedication

in the inpatient and ED arenas where they already have an IV, they're already a captive

patient population and it's not an additional challenge to give this kind of prep to them

and it can dramatically speed up how much time it takes for them to get their scan.

Good.

So going back to the power issue, was your study out adequately powered to support this

conclusion?

I would say yes.

When you have a non-inferiority study design, you establish what your difference is that

you're willing to tolerate and assuming your difference ends up being with that competence,

then you say yes I have confidence in the conclusion.

That's the whole nature of doing a non-inferiority design, to have it powered that way.

Sure.

Getting back down to things at your institution, have your findings altered the approach to

these patients at your own institution?

Yes.

It's affected two things actually.

On a national level now the ACR manual version 10.3 has changed its recommendations for how

steroid preps are given in part because of this article.

Now what it says is if you are a patient in which delay caused by premedication would

adversely affect care decisions, which basically is all the time when you're an inpatient

or an ED patient, you really should be using this 5-hour IV regimen when it's indicated

as opposed to a 13-hour oral regimen.

And simultaneously with that we also changed our local practice.

So essentially we don't use a 13-hour oral regimen in the inpatient ED arena at all.

If someone comes from four hours away and they would have been indicated for prep, we

sometimes would use the 5-hour regimen on that patient also.

So yes, it's changed.

I imagine it's been well received at the institution?

Oh yes.

How do I get rid of fat on my arms? And other commonly asked fitness questions. - Duration: 19:59.

I get asked a lot of questions about diet, exercise & weight loss and today

I am sharing the ones that I get asked the most and the answers. Some of

them actually might surprise you. I'm Melanie Ash of Melanieashfitness.com

personal trainer, a fitness nutrition specialist, a weight management

specialist. I am passionate about helping people find realistic, sane, and

sustainable ways to health, fitness and weight loss and an overall a healthy

lifestyle. I am NOT your quick fix 30-day diet

trainer, I'm all about doing things for the long term so that you can look good,

feel good, and have healthy habits for life. I get asked a lot of the same

questions and that's because even though sometimes you might feel like you're all

alone in this, we all have a lot of the same struggles; a lot of the same

challenges and so many people are dealing with the same frustrations,

concerns, and comments that you have. So today I want to answer some of

these super common questions, maybe help clear up some confusion for you and I

want to give you the chance to ask me something that's on your mind that

perhaps is not on this list. If you're tuning in go ahead and in the comment

section if you have a question let me know I am all ears I want to answer the

questions that you have today and maybe hope to help you find some

solutions. I'm gonna cover three questions that are probably the most

common ones that I get asked, so one of these might be a question or concern

that you have. #1 How do I get rid of this or this or this

and people will grab a certain part on their body and say "how do I

get rid of this" and sure we all have areas of our body that carry more body

fat than we would like and everybody wants to know how do I get rid of this.

I think belly fat is probably the most common one that I hear about but women

specifically also ask about the backs of their arms, maybe the backs of their

thighs. Sometimes that question comes with what

exercise can I do to get rid of this; first of all I always discourage clients

from grabbing parts of their body and kind of making a bad face or feeling

really negative towards it, we all have those things it's totally ok and the

truth is there's no specific exercise that you can do to target body fat in

one area of your body. The truth is you cannot spot reduce body fat. We'd all

like to know that we can go into the gym and do a hundred tricep pull-downs and

that's gonna get rid of this or maybe you've seen a story online from an

online magazine and it says the five exercises to get rid of belly fat or

backlinks or whatever other negative name that we're using to speak to

certain parts of our body that we're not happy with. Those exercises will

strengthen the muscles in those areas, it will tone them, they will not reduce

body fat. You can do 3,000 crunches and you're not going to lose body fat unless

you also are meticulous with your diet and you do a little bit of fat-burning-

cardio basically because that's how you're gonna burn fat by putting out

more energy than you take in, and burning body fat overall which eventually it

will come off of those areas. What's frustrating about that is we wish

we could point to a part of our body and say this is what I want the fat to come

from and the truth is based on our genetics, our body type and our diet

history when we start to lose body fat it's gonna come off in certain areas

first, the areas that are usually the first that we gain fat in are the last

place we're gonna lose it from and it's an overall process over time that

eventually we will start to see that body fat come off from maybe those

places that were pointing to that we aren't super happy about. So how do I get

rid of this the answer is by watching your diet, getting plenty of activity

strength training and metabolic conditioning or cardiovascular activity,

and doing that consistently over time along with healthy habits like; getting

enough water, getting enough sleep, and walking every single day. You can't spot

reduce body fat but you can overall reduce body fat, which will eventually

take it off of those troublesome areas where we would like to see it reduce

from. One of the second most common questions I get asked is someone will

say I'm working out, I'm eating healthy, why am I not losing weight? That is a

super difficult question to answer on the surface because just working out or

eating healthy means different things to different people

and so it's hard for me to know what specifically could be going on. Sometimes

it's a matter of maybe you're not eating, maybe you're eating healthy but maybe

you're not in a calorie deficit, you can eat a lot of calories in super healthy

foods, things like coconut oil or nuts that we know are healthy and have great

health effects but add up really fast calorically, maybe you're over eating

calories of healthy foods. Maybe you feel like you're eating healthy but three

days out of the week you go out and eat at a restaurant or you're having wine a

few nights a week, maybe those little things are actually holding you back from

getting results. Perhaps your workout program isn't challenging enough, perhaps

you're doing too much and you're putting your system under too much stress,

you're working out too much you're doing too much cardio and you're riding along

a response. So there's no one simple answer to" I feel like I've been doing

everything right why am I not losing weight" and this is one where I start

asking questions and learning about that person and trying to kind of dial into

some of these specific issues to see if I can help them figure out what might be

missing or what might be the thing that holds them back and oftentimes when I

start kind of peeling away the layers and asking questions. We can pretty quickly

grab on to something it might be you're under eating, it might be you're

not measuring your portion sizes you're just eating healthy and I'll say you

need to you know take a food log and write down what you're eating for a few

days to see if maybe you're under estimating how much you're eating which

is very very common. I had a client who started working out she really increased

her activity, she was feeling really good she said I know I'm making better

choices but the scale not really moving and I said well the next step would be

for us to have you write down what you're eating for a week and see what is

really happening there. It only took her about three or four days to realize that

the alcohol that she had while she watched the game you know she had three or

four beers, the snacks that she had at that time, a couple of other little bites

and picks here and there and oh there were two dinners out last week I ordered

this when I promised to order that. Pretty quickly what she saw was her

perception was that she was eating healthy and reducing her portions and in

many ways she was, but she was still leaving a lot of room for overeating

which created a calorie surplus not a deficit and that's why she wasn't losing

weight. So if you feel like you're doing everything right you're working out,

you're eating well, but you're not losing weight you've got to start kind of

digging into what you're specifically doing. Write it down have somebody take a

look at it, ask a trainer for advice, see if someone can help you pinpoint that

one thing even on my website if you go to Melanie ash fitness.com I have

several articles there that help you pinpoint what you might be missing and

how to make changes if you feel like you're doing everything right and the

scale is stuck and not moving at all. #3 The third question really common

question and this is kind of similar to what I discovered was.. How many calories

should I eat. I'll meet someone or maybe there's someone in one of my online

Facebook groups and they'll say okay how many calories should I eat? I cannot

answer that question for you that's a very difficult question for me to know

if; I don't know you personally, I'm not working with you, I don't know your

activity level, I don't know your current intake, your diet history.

Calorie needs are very specific to an individual so if you're going on to my

fitnessPal and you're just putting in your age, your

height, your weight, and they're telling you to eat 1200 calories that is such a

generalization. Depending on what you're doing you might need a lot more calories

than that, so what I'll often recommend because

that sounds super overwhelming right I don't know how many calories you have to

eat in order to lose weight, the best thing to do is to get an estimation and

there's some great free online calculators that will do that or there

are nutritionists who will help you do that, determine what a good baseline is

for you to start with and then it's trial and error. You've got to do that

you've got to stick with it fairly consistently and then you've got to give

it about three months see what your body does and then adjust from there.

Listening to your body as well are you pretty satisfied, are you never hungry, if

you're trying to lose weight and you're never hungry and you're not losing

weight you're probably eating too many calories in order to lose weight we need

to be just a little bit unsatisfied. So there's this magical little place

between I'm satisfied and full and I'm so hungry that I'm like going crazy and

it's really hard for me to stick to this diet. Its a magical place in between where

we're just satisfied enough that we're not gonna dive headfirst into the cookie

jar but we're also not completely satisfied, that's where weight loss

happens. Rather than worrying about a specific number

try planning out your meals watching your portion sizes, listening to how your

body feels, doing that consistently for several weeks and in fact a few months

at fast and see what happens does a scale go down does it go up does it stay

the same, adjust from there. If it goes up you need to eat less, if it goes down too

fast you need to eat more and if you stay the same you need to eat just a

little bit less, that's my best advice to answer the question how many calories

should I eat. #4 Finally probably the other most common question that I get is..

What is the best type of exercise for weight loss. I'm going to answer this

kind of tongue-in-cheek if you want to lose weight just weight

on a scale you should do cardio, you should do a lot of it because especially

if your sedentary, sweating a lot, running a lot,

will drop weight off of you quickly however you could be losing water and

could also potentially be losing muscle mass. Do you want to lose body fat as

opposed to scale weight; then you need to strength train and you need to do

metabolic conditioning cardio which is higher intensity shorter bursts and

shorter duration which helps increase your metabolism, increase how much you

burn, even when you're at rest is shorter is a lot probably safer because you're

not gonna wear down your joints as much, it's more fun in my opinion but not

everyone agrees and you got a strength train because you need to build that

muscle mass that is going to help fuel your metabolism for the long term. So if

you're only interested in the number on a scale and you just want to see a quick

drop and who cares if it's water or if it's muscle then sure just do cardio, you

can drop a lot of water really quickly but if you want to lose fat, fat loss

body fat then you need a strength train and you need to do metabolic

conditioning cardio. So yes both are important and there is time for both;

realistically if you're gonna work out even just three days a week you can do

two days of strength training and one day of metabolic conditioning and that is

enough that is great that is a really wonderful way to lose body fat you just

got to be consistent to it and you also have to watch your diet. Those are

four of the most common questions I get asked. If you're watching and you have a

Renee. Renee's 49 and menopausal, she needs to lose 30 pounds and she wants to keep it

off of course yes we do. Should I focus on cardio weights or diet? Renee I don't

know if you're gonna like my answer but you have to focus on all three. That

sounds a little bit overwhelming so if you're completely starting fresh, what I

would recommend is starting with 1 getting that under control and then

moving on to the next two and typically the truth is the

it's probably gonna have the most impact for you is your diet, the unfortunate

thing is once we hit menopause as ladies our calorie needs actually go down, our

resting metabolic rate goes down, and so we tend to need to eat less in order to

lose weight. What sucks about that is you're menopausal so your appetite is

gonna go up too in which case it's often helpful to do exercise along with getting

your diet under control because in my experience dieting without the exercise

sometimes is really hard, a lot of times the exercise is the motivation that

makes us want to eat better, now that's not true for everyone so if that's not

true for you then I recommend starting with your diet really focusing on

portion control as I said listening to your body being just a little bit

unsatisfied, eating really healthy foods as much as possible knowing that as

you're menopausal you need protein, you need to get lots of nutrient-dense foods,

you need your healthy fats, so getting in your coconut oil your olive oil

lots of fresh veggies a little bit of fruit and some good lean proteins as

well as some lower gi starches like winter squash is a little bit of

sweet potato white potatoes okay those kinds of things. With that being said the

next thing I would do is add in your strength training, your strength training

is gonna help you build and maintain lean muscle which you're losing as you age

especially after menopause and it's gonna help keep you motivated to your

diet, so doing maybe a little bit lighter weight and a little bit higher

repetition helps you get some cardiovascular benefit as well then if

you're kind of killing two birds with one stone, the last factor that I would

add on would be cardio I would keep it minimal I would do as little as possible

but I would walk a lot. My advice would be if you're just getting started

start with your diet, get one to two days of strength training in there especially

once you get kind of more comfortable with the diet getting under control

and beyond that walk, walk a lot. Try to walk at least 30 minutes a day maybe get

a Fitbit keep yourself accountable to a step ball. Walking lowers cortisol

which is really important for everyone but especially when we're menopausal

it's gonna keep your body moving it's gonna keep you loose and feeling good

and that low-impact activity is really really helpful and beneficial for

weight loss. Renee said you believe in three meals and no snack, mini meals and

does it really matter for weight loss? Renee that's gonna be specific to an individual, there's no

one way you know no research has found that any specific number of meals is

best although what we do know is you need a little bit of time in between

meals for your body to be in a fasted state, which means you don't want to be

nibbling and picking constantly all day long there should be at least two

hours in between a meal. Some people can go larger meals and eat every four or

five hours, some folks start to get hungry or you start to

get a little bit low blood sugar and you may need to eat every two to three hours

it's going to depend on how you feel your best what allows you to be

consistent which is probably the most important thing of all if whatever

you're doing is something that you're struggling with you're not gonna keep

doing it. So what's easy what fits your lifestyle, we all have different types of

jobs you know myself personally for example I go in for my evening

clients I start with clients at 3:30 and I work straight through until 8:30 p.m.

So I'm gonna eat something around 3 o'clock before I begin by evening

clients I don't eat dinner until almost 9:00. I'm

hungry so I do have to eat before I go to bed but I know that I'm not gonna be

able to eat in that window so for me if I were trying to stick to 2 to 3 hours

you know eating every 2 to 3 hours I can't do that it's

unrealistic, I don't eat in front of my clients so you've got to know what works

with your schedule and not being trying to force yourself into something that

doesn't really fit that you might only be able to do 50% of the time then

you're not gonna get results. So the answer is it doesn't really matter it's

whatever works best for you and allows you to be consistent and feel pretty

good about what you're doing I hope that answered your question.

Sandra, hi Sandra. Should you eat complex carbs at

night with dinner? So you're referring to complex carbohydrates such as sweet

potato, potato, rice, as opposed to simple or excuse me complex carbs now you got

me backwards. Complex carbs are your vegetables, simple carbs are you're

easily broken down sugars like you would find in bread and pasta and things like

that, again it depends on a person if

you're someone who does a hard strength training workout right before dinner

then you can absolutely get away with eating those simpler sugars at night

with dinner because your body's gonna be ready to absorb them. If you're someone

who does your working out earlier in the day or you don't work out at all,

you're probably going to want to keep your carbohydrates at night a little bit

lower so you're gonna want to do veggies at night before you go to bed. I usually

recommend fruit earlier in the day in the morning is when it's probably best

to replenish your liver glycogen but this again is unique to an individual

there's no blanket rules and that's good thing that I think is hardest for me to

get across to you guys because we see so much information out there that says

this is the thing, now this is the thing, here's what you have to do, the truth is

there's no rules it's what works best for you and it depends it just depends.

So Sandra if you do your workouts in the evening and dinner is your

post-workout meal and you strength train then you absolutely can get

away with simple carbohydrates at night before bed no problem your body's ready

to rapidly absorb them. If you do not if you either workout at another time of

day or you're not working out currently then I would say probably avoid the

simpler ones that late and stick with something that's going to be higher in

fiber lower blood sugar spike and more satiating even like sweet potatoes are

totally fine those I would consider a complex

carbohydrate, winter squashes and so on. I hope that answers your questions

after the life and answer those but if I leave you with anything today it's that

the answer to most of your commonly asked health and fitness

Big October Vegetable Garden Harvest (Zone 5): Local Food at Its Best! 🍅🌶️ 🥗 - Duration: 11:43.

it's getting close to the end of October,

but there's still a lot to harvest from our zone 5 garden.

Today I'd like to invite you to join me

for a walk around the garden as I gather today's harvest.

Let's start here where I'm growing Malabar Spinach and Flamingo Beans up the same trellis.

I'll harvest the Flamingos Beans first.

These beans can be eaten whole when they're younger,

but we're letting them grow to maturity on the vine and dry on the vine.

We'll be eating them as dried beans this winter.

We'll also save some for seeds.

These beans were a beautiful pink color when they were younger,

but now, as they're drying, they're turning purple.

A couple of people from Chile have told me that this particular bean is very popular

in a Chilean soup,

and they've sent me the soup recipe.

So, it's something I'm definitely going to try this winter

with our dried beans.

I'll put a link to that soup recipe in the description.

This is the first year we've grown these beans, but I think we'll grow them again.

They're really pretty.

They make a great dried bean for the winter,

and people who have had tried them tell me they really like them.

I'm getting close to wrapping up today's harvest.

Some of the beans aren't quite dry, so I'll leave them to dry on the vine

I want the beans to rattle almost like a shaker.

Once I get all the beans harvested, I'll dry the whole beans in a windowsill until they're good and dry.

Then I'll shell them and also dry the shelled beans in the windowsill.

And we'll store them in the winter in glass jars.

Okay, there's a look at today's harvest,

and to give you a look at the beans themselves....

There they are - pretty little beans.

Now let's harvest our Malabar Spinach.

One of my priorities now is that our first frost could be as early as next week,

so I want to harvest all of our frost sensitive crops so we don't lose them.

And Malabar Spinach loves the heat - definitely frost sensitive -

so I want to harvest every leaf off of these plants today.

A viewer from India shared one of his favorite Malabar Spinach recipes with me.

I almost always make saag paneer with my Malabar spinach,

but I want to try something different this time,

and I'll definitely try that recipe (Thambuli).

I'll also give you a link in the description below.

The plants are starting to grow their little berries and hopefully time to

produce some viable seeds in the berries before the plants are killed by the frost.

It's going to take me a while to finish this harvest,

so I'm going to go ahead and finish it, and I'll be back with you when I'm done.

Okay, this is taking even longer than I thought it would.

I've got most of the larger leaves harvested.

It's got some small Malabar leaves left on the plant.

And these larger leaves you see here are from the beans, which we won't be eating.

This way we'll enjoy all of our Malabar spinach rather than losing it to the frost.

And the beds behind me will get more sunshine.

Now let's go see if we have any carrots to harvest.

We succession plant carrots from February through late summer,

and we harvest them year-round - under cover in winter.

We also harvest carrots in succession and only harvest them as needed for meals.

Here's a little Parisienne carrot.

Here's a slightly larger one.

We like to go these small varieties because they do well in partial shade,

and this area it's definitely partially shaded.

Let's see if we have any more.

Today I'm just picking enough carrots for my wife and me to snack on during the day.

Another Parisienne carrot.

These might also be called Paris Market carrots.

Many of our carrots will be protected during the winter.

The carrots in this bed won't be,

but they can handle a frost. I'm not not hurry to harvest them all.

I will want to harvest them before we start getting heavy snowfall.

So, I think this will be enough for my wife and me to snack on today.

Now let's go pick some turnips.

Turnips can also handle a light frost, so I'm not not hurry to pick them either,

and we'll probably cover this bed with a cold frame once it gets cold.

Today I just want to thin out some of my Tokyo Market Turnips

to make them for other turnips to grow.

Plus, they're ready to be harvested - some of them are anyway.

There's a beauty! That's about the size they get.

And these turnips taste good enough to snack on, so

we'll just snack on these along with our carrots.

Let's see if there are any others ready.

Oops! Accidentally picked a little one.

Okay, there's our little turnip and carrot snack for the day.

Turnips and carrots can handle a frost, but cucumbers can't,

so I want to harvest the last of our cucumbers today.

And here's a beauty!

This plant is pretty much spent,

but we still have a few good looking cucumbers on it.

We'll eat the good ones, and the rest will go to our composting worms.

By the way, these are all Straight Eight cucumbers, which we've had a lot of luck growing.

The ones that look like this may not taste good to us but our composting worms will love them.

Okay, I think I've harvested all of the cucumbers for my wife and me, and our composting worms.

And, as you can see, these vines are spent,

so later today I'll come and remove all these vines and the trellis,

and that will give more sun to our fall crops, which are right behind me.

Next I'll harvest tomatoes and pole beans, which definitely need to be harvested before our first frost.

I've already featured these crops a lot on my last two harvest videos,

so today I'll be brief, but I do you want to show you what's still growing.

Our Honey Drop tomatoes have produced very well for us this year,

including late into the season.

These are an heirloom, and I'll definitely be saving seeds to plant next year.

We're also still harvesting Trionfo Violetto and Kentucky pole beans from this area,

but since I covered these crops a lot in the last two harvesting videos,

today I'll just show you some highlights.

In addition to Honey Drop tomatoes, we're still harvesting Blue Berries tomatoes,

and Black Cherry tomatoes, which we've grown for many years.

And this is unexpected

I found another cucumber while looking for beans.

Nice one too!

Like last year, I'm finding a lot more Trionfo Violetto pole beans than Kentucky pole beans.

They produced later in the season and ripened first in the spring.

And I can always count on my Sweet Million tomatoes.

They're the first tomatoes to ripen in the spring and they produce well late in the season.

These yellow pole beans have also done very well for us.

Unfortunately, I can't remember what they're called.

My wife planted these, so I'll ask her if she can remember.

And if she remembers the name of these beans, I'll post it on the screen.

Again, they've done very well and we've enjoyed eating them.

Nice big long yellow beans.

Well, I harvested all of the ripe tomatoes and pole beans that I can find.

I'll be back at least once more to harvest these before our first frost.

Now let's go out front I harvest eggplants and peppers.

Our peppers are thriving in late October -

thanks, in part, to the warm microclimate created by black grow bags on our concrete steps.

The concrete absorbs heat during the day and releases it at night to keep our plants warm,

even as temperatures have dipped into the 40's at night.

Today I'm harvesting all of the ripe peppers.

And I'll return to harvest all of them before our first frost, which could be as soon as next week.

I may also try to keep a few of our most productive plants growing past the first frost

by placing them under cover.

I've made a small solar dome covered with 6 mil greenhouse film for this purpose.

We eat most of our peppers fresh from the garden,

but will likely have a surplus at the end of the growing season.

I'll preserve the surplus by dehydrating some peppers and freezing the rest.

Our eggplants are also still thriving and ripening.

There are several Japanese White Egg eggplants to harvest today.

Small varieties like this do very well here.

I'm so glad we expanded the garden into the front yard.

We never had luck growing eggplants in the backyard garden, but they do great out front, where it's more sunny.

I'm also harvesting a number of Black Beauty eggplants.

I'm planning on making Eggplant Bharta with this harvest. (Baingan Bharta)

Finally, though I'll wait until just before our first frost to harvest our sweet potatoes,

I will harvest more of the sweet potato leaves today.

I'm planning on making Chao Fan Su Ye with these leaves.

It's a very simple, but delicious, Chinese dish with sweet potato leaves and garlic served over rice.

Our sweet potato tuber production isn't that great here in zone 5,

but they're worth growing because we eat the tubers and the leaves.

I may do a quick video on our sweet potato harvest, probably next week.

And here's a look at today's complete harvest.

It was a lot of work but well worth it.

I hope to bring you more harvest videos in November, December, January, February, and so on

to give you an idea of what we're harvesting throughout the year from our zone 5 garden.

I added a dark shade first

and then erase from the outside, leaving the shade at the edges

Then add some lighting like white on other edges and erase from within again

I thought for today I'd do a sort of, like, a different style recipe

because I usually eyeball all the ingredients for my chocolate chip cookies

so I have no idea of how much of each ingredient I put in

so today I'm going to eyeball the ingredients but also measure them

I don't know if it makes sense

but you'll see

so... let's get started!

I have all my ingredients here

and I haven't measured them yet

I'm going to eyeball them and use measuring cups

so that I can show you how much of them I'm using

so

for now I have flour

then I have butter

then sugar

eggs

vanilla extract

baking powder

So, now I'm going to

figure out how much I need of each thing

and...

let's start with the flour

I think for the flour I'm gonna go with

one cup

let's see

maybe one cup and a half, let's do one cup and a half

so, one cup and a half of flour

maybe half cup of sugar?

yea, it looks like a decent amount

okay, half cup of sugar

butter

I'll put like one stick of butter

I have to melt it though because this is too hard

I usually just use the cap to measure it

so I'm just gonna do one little cup

which I think it's about

1/4 of a teaspoon

or half teaspoon, do half teaspoon

for baking powder, it depends

I want them to be really, like, poofy

so I'm gonna add some extra

so we're gonna do

two teaspoons

the butter is ready

so I'm gonna do two teaspoons

these measuring cups are really weird

because they include a measure for two teaspoons instead of, like, just one

but, so, this is two teaspoons of baking powder

okay, so

we have our melted butter here and

it didn't melt completely but

it's good enough

so

this is one stick of butter, so eight tablespoons

and I'm gonna mix now

let me get something

what should I mix it with?

a fork

just a fork is fine

okay, I'm noticing that they are a bit dry

so I'm going to add a tiny bit of milk

well, let's get a tablespoon of milk

which I don't have because my husband

drank it all this morning

I'm just adding a tablespoon of water, just winging it here

so, now I'm gonna add the chocolate chips

you could cook them a little bit longer if you want

but I like them a bit more

you can see that they start to get brown on the sides and the back

that means that they're ready

okay, I think they came out pretty good

if you like your cookies extra sweet

add a bit more sugar, but otherwise

I think they're fine like this

so, as you saw, these cookies are pretty easy and quick to make

they don't make a big mess in the kitchen either

so if you don't feel like cleaning but you want something sweet...

Now she's back to whine more, saying that as a result of that she's been "blacklisted"

by the entertainment industry.

Well, sorry Kathy, even liberal Hollywood lunatics have some very basic standards.

As evidence of her persecution, Griffin talked about a job telling jokes at the Hollywood

Reporter Power 100 Women in Entertainment Breakfast but having it revoked when she did

her sick anti-Trump stunt.

"I do not buy their official reasoning and I believe 'the word has been put out'

on me," Griffin said.

"I can't help but take this as another occasion where Hollywood is blacklisting a

woman and silencing her.

After decades of hard work in this industry, it's my opinion that this is not right nor

justified.

first one so Snyder was originally scheduled to direct the Justice League

One and just as eek to the reason it was going to be part 1 and part 2 with the

second one coming out the late 1/2 of 2019 I think around June but it was then

revealed that it's not going to be a two parter that the first

the movie is going to be an origin story as we all know here on the channel and

then the second one was going to be a standalone but I think they've obviously

already teed this up guys I think there's going to be some post-credit

scene and I think we can predict the dark side is going to show up in a

post-credits scene so does the fact that they're talking about the Justice League

to before the first one tell us that they've restored credit they have

restored belief in the Justice League which is awesome because everyone

praised Wonder Woman they praised the hell out of it but what you guys don't

remember is if you watch The Wonder Woman movie I've recently bought the

blu-ray and the screenplay has been done by Zack Snyder that Snyder casted Wonder

Woman so Zack Snyder is a inhalation so I really think the fact that the Justice

League too is being written backs up that there is gonna be some post-credit

I dunno crime syndicate there's endless opportunities of who it could be

so everyone knows his action I just stepped away from the movie for personal

family reasons but I believe he's stepping back into kind of like I didn't

know like advertising the thing because on it's very recently he's posted image

with the atlantean King funny enough in the Aquaman suit and he's in that image

which is pretty cool he's also posted a welcome to Central

City sign he's also posted some Amazon's next to

Batmobile and he's also posted an image of him with ray Fisher who plays cyborg

saying welcome to Veera so if this doesn't back up the claim that he will

be at the world premiere I don't know what he would do and I really hope he is

at the world premiere because it would be a real shame if he wasn't because

after all guys it is his movie alongside Joss Whedon as Joss Whedon has actually

done some sensual work on it so guys I'm gonna need to address this

because I keep seeing us everywhere so Marvel The Punisher comes out the same

day as the Justice League movie on the 17th of November now guys I don't know

why you guys are freaking out about this you're saying Punisher versus Justice

League there's absolutely no issue with this because the Punisher is aimed at

older people it's a Netflix service anyone can watch at any time Marv what

Marvel Netflix do is they just drop the whole season in one go I mean if you've

been what if you've been looking forward to Justice League for months you're not

going to not watch the Justice League just because Punisher is there something

that you could watch whenever but at the same time guys kids can't go and watch

Punisher unless that like parents let them do that but kids can go watch it

just to see maybe I mean yes it could indirectly affect sales of the Justice

League but I don't think it will everyone loves Wonder Woman everyone

loves Superman everyone loves Batman everyone loves

Aquaman I'm not so sure about cyborg or cyborg is calling them either here or

there and also also in other news the runtime for Justice League is apparently

121 minutes which would make it two hours I mean I can't personally see it

being two hours long because that is a very short if you compare it for the

runtime for Wonder Woman the run type of bat another Superman I predict it will

be somewhere between 2:15 and 2:30 I mean I'm still gonna watch it just

doesn't matter to me how long it will be but how I will go watch it if it was

five hours long but the longer they make the movie the more storytelling there

would be so surely it's a good thing if if it's longer which is kind of cool I

think so guys and let me know what you think and also I found this kind of cool

quote that was kind of interesting ears from Jason Momoa at the plays Aquaman or

Aquaman it says Zac is someone who kind of saved my life this opportunity to be

to be where I am is all because of him so if you like a command you can pretty

much thank exact Schneider instantly because none of this would have come

true if it weren't for him Jason Momoa I think that is such a nice thing to say

about Zack Snyder because as you guys already know is that

Schneider he found he found workout gadot wonder woman he found Ezra Miller

the flash who appears to be pretty cool he's quite funny quite quirky Batman I

think he's found a good person in Batman I really like that character I just

think when he's a bit more like hearted people over not to him a bit lot more

who else cyborg I mean we don't know a lot like

cyborg but apparently due to reshoots they've kind of make made him a lot less

a lot more like hearted so I think that Schneider is really gonna come to his

own and you're gonna understand that he is made a mammoth other movie that could

be compared to anything so alongside everything

the first kind of team-up Justice League movie is going to be compared to it

Avengers now I don't think I've seen some people comparing the Justice League

movie to infinity war now I think that's a bit unfair because like the Marvel

kind of MCU has been going around for 10 years the Justice League has literally

really it has Man of Steel it has Batman vs Superman

it has Suicide Squad and it has Wonder Woman so you can't really compare a you

can't really compare the Avengers to Justice League it's so unfair I mean I

think give them another five years then you can compare the two you guys know

here now I now kind of review Marvel related stuff we can't wait for Thor

Rock or knock I just drop the video I put a card at the end but I don't I just

can't understand why some comic book fans you can't appreciate how good

Marvel is and how good DC is I mean I really wish because I'm such a fan boy

for Marvel I mean ant-man's one of my favorite characters I can't wait for

next and man I can't understand why one day we can't have Avengers versus

Justice League or a Marvel and DC crossover movie that would be so good

The Beldam Is SATAN?! - Coraline and the Bible | PhulaTheories - Duration: 4:58.

So, I'm finally covering Coraline.

Now, just a wee bit of a disclaimer before starting, this isn't gonna be some huge

lore researching of this movie, delving deep into the story and finding little easter eggs

to support my claim, but rather, this theory is gonna be quite a small one, you know, because

enough people are already tearing this movie apart brilliantly decoding its lore….

But yeah, basically this is just a small, simple claim concerning this movie.

Now I'll begin the theory.

*INTRO*

Now, in one of the last scenes of the film, we see the Ghost Children finally free.

However, when they are free, they talk to Coraline, and behind them is the famous painting,

Starry Night.

Now, here's one of the major pieces of evidence for this theory: That Laika was inspired by

biblical stories, and applied some of them into the film.

No, no, no, I'm not saying that the entire film is an analogy of one biblical story or

the Bible, but rather that a couple of aspects in the film were inspired by certain stories

from the Bible.

For example, the story of the Ghost Children.

Like I [or Laika, amirite ;)] previously mentioned, when they are freed thanks to the efforts

of Coraline, for some reason they are surrounded by Starry Night.

Now this painting has a lot of meaning and symbolism put in there by its painter, Vincent

Van Gogh.

Now one of the many things this painting was thought to have symbolized was the story of

Joseph in the Bible.

Basically, the story of Joseph is that Joseph's brothers were jealous of him as he was the

most loved by his father, so they put him in a well, only for Joseph to be sold to a

servant of the Pharaoh of Egypt.

This servant had Joseph put in jail, due to a lie by his wife.

Eventually, the Pharaoh brought Joseph to him as he wanted a very strange dream to be

explained to him, and then when Joseph explained it, the Pharaoh put Joseph in charge of Egypt.

Eventually, his brothers came to Egypt, and they bowed to Joseph, as he is now a high-ranking

person.

Now in this story, Joseph had a dream, where all the brothers bowed to him, which is what

happened at the end, then eleven stars, the sun, and the moon all bowed to Joseph as well.

Now, the moon, the stars, and the sun were all drawn in Starry Night, just like Joseph's

dream.

Furthermore, Joseph was thrown in ditch, imprisoned, and sold into slavery, much like Vincent Van

Gogh himself.

Now it's this part that I really want to associate with Coraline.

Just look at each of the Ghost Children: They each fell into a pit, at least perhaps not

literally, as it could be represented through the tunnel in the house.

However, if the well is, in fact, a portal to the world of the Beldam, then it could

be exactly like Joseph, but only if the children actually reached there through that well.

Furthermore, they each got enslaved in a metaphorical sense, as they each got their eyes replaced

with buttons and were pretty much mind slaves for the Beldam, and were completely seduced

by her world.

Then, they were imprisoned and cast away by the Beldam, just like Joseph.

Eventually, they were freed, just as Joseph was.

So, yes, that's the reason why the Ghost Children had Starry Night and I believe that

they're story was based off of, if not, inspired by Joseph's story.

Yes, you might be thinking that Starry Night also represents isolation, and that is what

the Ghost Children had felt for all the years they've been ghosts.

However, considering that their entire story concerning them coming across the Beldam lines

up with Joseph's story, I think that this painting being behind them could mainly represent

their stories being similar to the story of Joseph.

Now to the other aspect of Coraline which I think was inspired by Biblical stories,

and that's the Beldam, the oh so mysterious Beldam.

Now I think she is an analogy for Satan, or at least, Lucifer, while her world is representative

of the Forbidden Fruit, in a sense.

You see, what basically happened was that Satan, who was known as Lucifer at the time,

tempted Adam and Eve, the progenitor humans, into eating the Forbidden Fruit, which was

a fruit God had specifically, very strictly, told Adam and Eve not to consume.

So, yes, what I'm saying is that the Beldam is Satan while her world represents the Fruit,

as she tempted children to go into her world and stay there, just as Satan had tempted

Adam and Eve to take a bite out of the Forbidden Fruit.

Furthermore, Christianity often considers committing a bad deed as "listening to Satan"

as he is constantly tempting humans to do wrong, with some people, such as the three

Ghost Children before they were ghosts, to be tempted (the only reason I chose the Forbidden

Fruit as an analogy is because that's the most famous and specific example).

However, unlike the other children, Coraline had resisted and didn't take a bite.

Now remember, I'm not claiming this is an analogy, but rather that specific individual

aspects took inspiration from the Bible, which is why the two aspects I mentioned don't

line up well.

So, yes, that was my small, small theory on Coraline.

I doubt I'll cover this again movie in the future.
