I had my interview with the Bettin brothers 20 years ago,
It was with all of them, maybe because I was the first graduate to be employed.
I was one of the pioneers for DBA's activities abroad
because, a few months after employing me,
I was sent to the Balkans
to work alongside Telekom Srbija.
Eighteen years later, DBA acquired Actual
and began its expansion into the Balkans.
I've worked on many things at DBA,
first, in structural engineering, which was what I studied.
Then, I worked in mobile telecommunications,
in fuel distribution networks and also in purchasing.
Now, I'm TLC Business Unit Manager for DBA Progetti.
This is one of the company's main sectors
because TLC networks,
especially ultra-wideband networks, are rapidly expanding,
and they represent the future for DBA and for Italy
for the next four or five years.
I gained a lot from working for DBA.
I have grown both professionally and personally.
When I started at DBA, it was like a big family.
Now it's a large company with great people at its heart,
each with their own skills, professionalism,
their own talents and ambitions.

Hello beloved one! Welcome to the Biga's tips of this week. I want to start this week
reading a text for you. 1 Peter 5:8 says: "be serious and keep watch", in other
words the bible is saying to you don't distract yourself. The question what we have this week is do not
distract myself with what? I want to give you 5 heads-up to us avoid distractions.
1º: do not distract yourself with two visions. Beloved one of the strongest and right
ways to take off a focus from the person is giving her another focus.
And many people are lost thinking in two or three things when actually the person
just have one focus in her life. Why people change the focus? Either Because of lack of perseverance or
because she doesn't belive in herself, or because she doesn't believe in God's promisses. Because
every promise that God give to us, you need to have a relationship with God, every promise
will come in a process, He will not show to you the whole picture of your life, He can give
you a word but this word will make you to obey just in parts. Every great vision
in someone's life had become true because this person obeyed in the small things. There
were baby steps of obedience what did the whole picture become true, the whole project. Sometimes
people give up during the process what is not easy but it's Worth. So
beloved if you are in a little distract thinking about making two things, I
want to give you a tip, the whole week I'll talk about it, have sensibility to God.When
you don't know what to do just have some time with the Holly Spirit because this is what
I do. When I don't know what to do I start to pray in my spirit and I say: Spirit
you know the deepths of God, you know my life so guide me,
I don't wanna lose my time with two visions. I want to pray for you.

Porsche Panamera 4 E-Hybrid (2017) review
Once upon a time, the Porsche Panamera was the mighty answer to a question that few people raised.
But thanks to careful development of the second-generation Pan-am, and a comprehensive extension of the model range, there's now a version for just about anyone – assuming they want a 170mph-plus coupe-style saloon.
The Panamera range is well served with V6-engined models, with petrol, diesel and plug-in hybrid options to choose from.
The 4 E-Hybrid tested here gains a 135bhp electric motor, shared with the 918 Spyder, which on a full change (that takes 5.8 hours on mains electric) is claimed to provide up to 30 miles on battery alone.
It'll also do 87mph in whispering EV mode – presumably for far fewer than 30 miles. Porsche claims it has overcome PHEV range anxiety, though.
The 4 E-Hybrid comes with two modes which will alleviate the need to plug in: the E-Hold setting maintains the charge roughly at a constant level, and E-Charge uses the V6 engine to recharge the battery.
Guess what that does to the fuel consumption. Just how clever is the new Panamera 4 E-Hybrid?.
Well, it's at the cutting edge of PHEV technology right now. And it's a good platform to be based on – as we've detailed extensively before, the second-generation Panamera makes great use of VW Group's adaptable MSB chassis.
From a driver's perspective, the electric motor integrates beautifully with the V6 internal combustion engine.
There's no step in torque as the electric motor wakes up – as before, where it only kicked in when there was more than 80% throttle – just what Porsche engineers describe as waves of creamy-smooth power, and a side order of fun.
Let's get the claimed figure of 94mpg out of the way first.
Yes, it'll do that on the official lab test, but in the real world, where you're less likely to keep it fully charged, expect something in the low 30s – if you're being gentle.
As to how much fun it is, we've driven the E-Hybrid on some very challenging roads in South Africa. Hybrids just want to have fun. The official figures only tell part of the story of the Porsche Panamera 4 E-Hybrids outright performance.
Its bi-turbo V6 is potent enough in isolation, but when combined with that 140bhp electric motor driving the rear wheels, it's quick enough to keep the brilliant 4S Diesel in sight.
Its claimed 0-62mph time of 4.6 seconds is rapid enough to be interesting, even if the method of its delivery lacks outright drama. You just mash the throttle and go.
If you must, you can blame the four-wheel drive system managed by Porsche's 4D Active system for how unruffled it is. On quick getaways, this quells wheelspin and any sign of unruliness, and just gets on with the job.
But it does so much more, as we'll find out in our 4 E-Hybrid review. Quick driving is made so much easier by its eight-speed dual-clutch PDK. It's the transmission set-up used in conventionally powered Panamera models.
And it's exceptionally good here, too, changing gear smartly – missing ratios if it's the most efficient course of action. We rather like that. Come on, is the hybrid as good to drive as a 'normal' Panamera?.
In a word, yes. All the best bits of the standard car have been maintained, despite being packed with so much planet-saving tech.
The handling and ride are as sublime as ever, and here is a car with incisive handling, pliant damping, and all the communication through the wheel and pedals you could ask for.
And given this is effectively a two-tonne supercomputer on wheels, that's as much as you could ever hope for.
Porsche's 4D Active system incorporates very clever traction management, which spoon-feeds all four wheels with just the right amount of torque to quell any trace of understeer or oversteer.
It's less dull than it sounds, distilling any corner into a place where you can play, while marvelling at what it can do at insane speeds.
Before you get too carried away, it's not perfect. The hybrid system clonks and whines in traffic, and the braking system is noisy in town.
You'll never be allowed to forget just how big this car is. It feels – and is – wide, and at five metres long, it'll take up a lot of road if you're really cracking on.
But on the motorway, the Pan-ams natural stamping ground, it's comfortable, settled, and absolutely magnificent. As for B-roads – yes, it's fun, but give yourself space, and learn to turn it in smoothly, and shift its enormous bulk around with finesse.
To let the world know you're a tree-hugging (tax-efficient) kind of car owner, the Porsche Panamera 4 E-Hybrid has plenty of green details. There are special green badges and eye-searing green brake calipers.
Inside, it's business as usual, with exquisitely-designed (and crafted) seats and steering wheel fighting for attention with the trio of screens.
A big improvement inside is the 12.3-inch touchscreen in the centre of the dash, and that's complemented by a pair of seven-inchers in the instrument pack. They're easily navigated using the steering wheel scrollers, which is just as well.
We won't detail all their functions – life's too short – but you'll never get bored of playing with the menus, and analysing the data this car throws up, especially when it comes to that hybrid set-up.
But don't let any of that take it away from the 4 E-Hybrid – it's a very special place to spend time in. Verdict.
Aside from having the ability to bring out your inner geek – thanks to the Porsche Advanced Cockpit's endless information stream – this is a fabulous car.
It is fun to drive, just as Porsche promised, and it is blessed with all the dynamics of its conventionally-powered brothers, as well as a goodly amount of silken punch.
Its big inside, wonderfully crafted, and now the controls have been tidied up, easy to navigate around.
The boot isn't very big if you're cruel enough to compare it with a Mercedes-Benz S-class or Audi A8, but at least the hybrid drivetrain doesn't rob it of any more room.
But here's the thing – it is a bit too clever for its own good, a Porsche for the head, and not the heart.
And if you want a fiscally-effective version, it's hard not to ignore that elephant in the room, the brilliant 4S Diesel.
But with the 4 E-Hybrid, you get the added appeal of silent running in town, and potentially good (urban) fuel consumption, assuming you can keep it charged up. It's a good choice, but then, so are many other Panameras.

in this episode of Java in two minutes, I'll explain
how to use the boolean algebra.
For helping myself with the examples, we imagine to be
in a room with a switch, a motion sensor
and that according to these two inputs, we have to turn on a light bulb
which will be our output
First of all we create a boolean varaible called "stratoInterruttore"
with false value, to do that, I'll write "boolean statoInterruttore = false;"
then we will create two more relatively
for the motion sensor and the light bulb.
Let's start by testing the conjunction "And"
for this example we have to make sure that to turn on the bulb
both the switch and the motion sensor are activated
after this premise we have that in Java the conjunction "and"
is expresses with "and commercial"
threrefore we will set the program output using the code
"output = statoInterruttore & statoMovimento;"
adding a print statement at the end
we will obtain that the output will be false
changing the values of the two starting variables to "true" the output will become true.
Moving to the "Or" conjunction
we must assume that the light bulb will turn on
when at least one of the two inputs is active
the conjunction "or" in Java is expressed with a vertical bar
so in the program we will set the output with
"output = statoInterruttore | statoMovimento"
in this way we will obtain true,
but if we put both inputs to false we will get false.
We continue with the "XOR" conjunction
here we have to assume that it is impossible that the light is on
when both inputs have the same value
that said in Java the "xor" is expressed conjunction with circumflex
to set the output of the program I will write
"output = statoInterruttore ^ statoMovimento"
in this way we will obtain false
but if we turn on the switch, the output will be true.
For the conjunction "Not", take the program we just wrote
but we need that he act conversely
so when the two inputs have the same value

Goku and Hit vs Dyspo and Kunshi- Dragon Ball Super Episode 104 Review - Duration: 9:03.
Dragon Ball Super Episode 104 was an amazing episode.
The Fan Favorite Hitman Hit fought the way all the fans wanted him to, and the entire
Goku-Hit duo was a delight to watch.
The Speedstar Dyspo brought in some lesser shown or pointed out materials of the show.
First of all, let's get the Super Saiyan God thing out of the way.
Hell Yeah, after almost 2 years and more than 90 episodes, Dragon Ball Super featured The
Red God Goku once again.
Even though I'm not the biggest fan of this transformation due the muscularity issue,
it was refreshing and fun to see it after such a long time.
So what was the reason behind using it again?
In my preview breakdown, apart from talking about if SSG is faster than SSB which was
not the case, I pointed out this exact reason too.
Just like the Manga stated previously, Super Saiyan God form consumes less energy and stamina
than the Blue form, and that is the main reason.
It's faster than any of the Super Saiyan forms, but slower than SSB.
Here in this scenario, Goku couldn't afford to go SSB full time while dealing with Dyspo
because he already did that multiple times and has to reserve his energy for the later
part of the tournament.
However, at the same time his base form or Super Saiyan form wouldn't be enough to
get the job done.
So, Super Saiyan God form was something in between that was fitting the requirement perfectly,
it gave him some speed and at the same time wasn't burning much energy.
So, for this particular scenario going red was the best thing Goku could do.
Thankfully, they made it clear that Super Saiyan Blue is stronger, faster and is much
superior than SS God.
To be honest, even though it's obvious, I'm really grateful that they spelled it
out word to word.
Otherwise some fanboys would keep claiming that SS God is more powerful, and they already
had been doing this since the preview came out.
You will not find them now; they will not apologize in the comments either.
Anyways moving on, Dyspo The Speedstar turned out to be an amazing addition in the Dragon
Ball Universe.
A moment of silence for fans who kept claiming Goku can't deal with faster than light opponents,
as he did exactly that against Dyspo this episode.
Anyways, this Dyspo guy is extremely talented and has some crazy skills.
He is way faster than sound and light; additionally he can increase his movement speed suddenly
by thousands of time in an instant.
Not only that, he has Super hearing abilities, which allows him to do stuff like listening
to opponents muscle movement vibration and figuring out the attack they are about to
use.
So with his huge arsenal of techniques, he was really giving Hit a hard time initially,
but much like Goku, Hit started to analyze and breakdown Dyspo's fighting style and
strategies, instead of attacking or defending.
He realized that Dyspo suddenly changes and increases his speed just as Hit gets ready
to use the timeskip.
It's not necessarily the speed of Dyspo, but the swift and huge change of speed is
what's catching him off guard.
Suppose you are playing Baseball, as the ball is coming towards you, the speed of it suddenly
increases by even 5 times halfway through, you are in trouble.
So, that's what was happening here, Hit was preparing the Timeskip for a certain impression
of speed that Dyspo gave initially, but Dyspo was instantly changing it and taking out Hit
before he could do anything.
But, figuring that out wasn't going to solve all of Hit's problem.
He still needed to know, how Dyspo was able to time the speed change so perfectly.
Then, Hit does something, Goku often does and even did last episode.
He started to willingly take blows from Dyspo, and only blocked the vital moves.
He did that to adapt the speed of Dyspo and at the same time try to Understand how Dyspo
was able to figure out his movements so swift.
After an interruption they battle again, and Hit figures out that Dyspo has Super Hearing
Abilities.
When attacking Hit, Dyspo basically doesn't depend on what he sees or feel but what he
hears.
Both of them are charging at each other at insane speed, and all Dyspo depends on is
ability to listen.
When Hit uses TimeSkip his muscle tense up, and Dyspo using his hearing abilities is able
to listen to the identical muscle vibrations that is created during TimeSkip technique.
Don't forget that Hit is a popular assassin and Dyspo was able to recognize him, and also
had prior knowledge about Timeskip.
The equation was then clear to Hit, so he started faking the muscle movement of Timeskip
while using other attacks, and much like what Dyspo was doing with speed change, Hit applied
the same principle on altering his techniques totally owning and outsmarting Dyspo.
Luckily, Hit had some other next level techniques, like being able to momentarily transfer his
body to a different dimension as a mean of dodging an attack.
So, Hit totally got the better of Dyspo and almost eliminated him twice but Dyspo got
his ass saved by Kunshi.
Who however later got eliminated by Hit.
Hit also used an interesting technique against Kunshi, as Kunshi had himself surrounded by
his mines.
Hit used a shockwave that only comes in contact with people, and thus directly attacked Kunshi
going through all the mines, such next level attacks, and such a handy attack for an assassin.
These are the type of feats those DC, Marvel fanboys would claim as some kind of hack attack
that can take out anyone irrelevant of strength.
You won't find them glamorizing Hit's abilities though.
Anyway, Goku again also showed his fast learning abilities, he had already read Dyspo and heard
Hit explaining what he was doing.
Then Goku only took a little time to adapt his movement, and then in a way used Dyspo's
strategy on Dyspo by momentarily going SSB from SS God, instantly changing and increasing
his speed and was able to land one good strike on Dyspo.
Also, he pointed out how the Job was easier because Dyspo's movements were too linear,
so it did no good despite having a lot of speed.
Overall, Dyspo has some really good moves, but the way he fought, it appeared as though
he lack experience or simply doesn't have high battle intelligence like Hit and Goku.
He might be a comparatively newer addition to the pride troopers, but he has a lot of
potential.
He just needs to learn to do justice to the immense power he possesses.
Team Goku-Hit was really fun to watch, they have such an amazing understanding almost
as if they have fought together for years.
Vegeta must be getting jealous though, keeps reminding me of that TFS video, also he was
getting jealous after Hit came to kill Goku on Earth.
The way everything is going, it just feels like Hit is the one after Piccolo and Vegeta.
After this episode it makes me think of Hit will end up being the one to win the tournament
of power, maybe I'll make a theory video on that later!

Fans were talking about Zeno not being able to keep up with Hit, Dyspo action.
I don't know why exactly, but I think because of the way Zenos are, You know Pokemon like
they have this relaxed mode when they are like that.
Otherwise it doesn't make sense, because Vados mentioned she was having no difficulty
keeping up with it, neither did other angels or The Great Priest seemed to be troubled
by it.
However the new function added to the app thingy was funny.

The bag I´m using is from uhlsport
It´s black with blue
It´s 50l and has got 3 different big compartments
In the smallest compartment are things like tape
and in the other small compartment are clothes for before or after the training
onto the main compartment
first are nike neymar shin guards in green with black
and here are the matching socks
next are elbow guards
in summer I´m using them when I´m wearing a short sleeve jersey
because our training court is pretty sandy
then the socks
they are black with a bit white from adidas and Real Madrid
next is a undershort
it´s white from adidas techfit
then a padded gk short
it´s from adidas, too; nothing special
next are my shorts I´m wearing for training
It´s blue from adidas with white stripes
Then i´m wearing an undershirt
it´s a green one from adidas techfit without sleeves
of course, there must be a jersey,too
it´s the home gk jersey from the world Cup from spain

General Astrological Predictions: At 18:31UT the Moon conjuncts the Sun causing the New
Moon and a Solar Eclipse in Leo.
Actually, almost two hours after the eclipse, the Moon will enter Virgo.
The energies of the eclipse have started raising just after the Lunar Eclipse and will be active
until the next Full Moon on the 6th of September.
But, of course, they will be most powerful during the day of the Eclipse.
For Australia and most of Asia this will be the 22nd of August, but for the rest of the
world, it will be on the 21st.
As we�ve already seen, just to make things more� interesting, most of the planets are
retrograde and this will be the case until the 26th when Saturn will turn in direct motion,
but the balance won�t shift until the New Moon, the eve of which Mercury will also turn
in direct motion.
From the 28th degree of Leo, the Solar Eclipse conjuncts with �Al Jabhah� of Leo�s
constellation.
A fixed star of mixed nature, giving riches but can also bring out violence.
War can break out shaking human kind.
The whole scenery is not so favourable as the delays and obstacles created by the retrograde
planets face the emergencies created by the Eclipse.
The incompatibility of the energies the skies create is too great to easily deal with.
But, thankfully there are some good news too.
Mars hexagon with Jupiter is still strong.
Mars is also forming a triangle with Saturn.
Saturn�s hexagon with Jupiter is weak but still active.

Why Small Chance of Swelling and Pain After Botox® Migraine Treatment isn't Caused by the Botox® - Duration: 6:20.

You're asking a question, can Botox® for migraines cause pain inside the ear.
And you describe a scenario where about an hour and half after you got some Botox® for
migraines, as described or as pointed out or illustrated in the diagram, that you developed
an area of swelling.
And although your neurologist was away, you had your general practitioner noted that there
might be a possibility of a lymph node and that the pain in the ear got worse with chewing.
So I can certainly share with you my thoughts on what I think is the ideology of your pain
after this injection.
A little bit of background, I'm a Board-certified cosmetic surgeon and Fellowship-trained oculofacial
plastic and reconstructive surgeon.
I have been in practice in Manhattan and Long Island for over 20 years.
And I have been treating patients with Botox® since 1993.
And in fact, oculoplastic surgeons and neurologists have been the first doctors to use Botox®
for neurologic issues such as facial spasms.
In fact, migraine is something that evolved out of the treatment of facial muscles where
patients who have migraines noted that they appeared to get better when they had their
Botox®.
So I think that there is most likely an explanation for you in this situation.
The muscle that the Botox® was injected was the temporalis muscle.
And this is a muscle that is important in terms of helping you with the migraines but
is also a muscle that is used for chewing.
And it seems likely, based on your history, that when the injection was placed that a
vessel could have been traumatized and it led to bleeding within the muscle or hematoma
and that's where swelling is likely to be the origin especially with that timeframe.
So it's not specifically the Botox® itself but rather the trauma of the placement of
the needle.
Now this is an area where it's a muscle and it's fairly vascular.
Therefore it's not surprising.
It could have been just a small vessel feeding the muscle.
It doesn't have to be a major vessel.
That being said, as that hematoma clears, there's going to be some pressure on nerves
that also happen to provide sensory and innervation in the ear.
And the fact that it's aggravated by chewing further verifies this idea.
Most likely, as this hematoma clears and as the inflammation of that activity of this
area clears then the pain should get better.
I think that it's not again the Botox® itself but rather the swelling that is responsible
for this pain perception.
So I think that once you're able to discuss this with your neurologist, you can probably
clinically see improvement even in the course with a few days as this resolves.
So talk to your neurologist about this.
I think that from my own practice and a lot of patients do have Botox® for migraines
or actually come to me because they get Botox® for cosmetic reasons and helps them as a benefit
for migraines.
But people who have more extensive migraines or more frequent migraines typically go to
neurologists who at this point have more expertise and experience with Botox® for migraines
at least in comparison to an aesthetic specialist like myself.
So review this with your neurologist.
If there isn't resolution, you might consider some imaging studies but chances are, it's
going to get better without any intervention or further investigation.

How Hair Loss can be Controlled in People with Premature Hair Transplants Done in Their 20s - Duration: 13:01.
Thank you for your question.
You're 22-years-old and you recently underwent 1500 hair grafts in what appears to be the
frontal hairline based on the photo you submitted which was apparently strip surgery, FUT surgery.
And you state that you've had hair loss since age 17 and you've been on finasteride
since that time which appears to not have stabilized your hair loss. Men in your family
are Norwood 6 and you were advised that you would need about 10000 more grafts from your
donor area as well as 6000-8000 from your beard. And now you're getting conflicting
opinions and now you're concerned on whether or not the procedure was done ethically.
Well, I can certainly discuss a little bit about my strategy as it applies to patients
like yourself who we see in our practice. A little bit of background, I'm a Board-certified
cosmetic surgeon and Fellowship-trained oculofacial plastic and reconstructive surgeon. I have
been in practice in Manhattan and Long Island for over 20 years. Hair transplant has been
a very big part of my practice. I am the founder of TrichoStem™ Hair Regeneration Centers,
a system we developed to help men and women with genetic pattern loss using a non-surgical
approach to help people with hair thinning that we developed over the course of the past
7 years which actually evolved from our desire to improve hair transplant surgery.
So you know, I think that we're definitely dealing with a relatively somewhat controversial
area when it comes to transplantation in younger men. As far as my perspective is concerned
and this is my perspective and I don't think a lot of people have actually looked at patients
this way because I think essentially, the unfortunate reality of hair loss treatment
as it is generally practiced is that if you go to a hair transplant center or clinic,
you're essentially judged as to whether or not you are a good candidate for transplant.
But I would say that in the overwhelming majority, you aren't going to be someone who they
will want to manage and take care of in the long-term. It becomes more of a transactional
approach. If you're a transplant candidate, they'll do a transplant. If you're not
a transplant candidate, they'll tell you, go somewhere else. Then in the medical side,
typically, patients will go to dermatologists who will offer them basically finasteride
and minoxidil and then the rest is basically limited.
Essentially, what I've been looking at for many years is with this treatment called Hair
Regeneration which I'll explain briefly. It evolved out of our desire to help people
with hair transplant. From what you've probably learned from your own experience that the
donor area is a limited area and the frustrations with hair transplant have to not only do with
the limit which is they can only transplant so many hairs from this donor area at one
given time but the survivability of the transplanted hair.
So we use a material called Acellullar matrix to help improve the survival and the viability
of the transplanted hairs as well as to improve the appearance of the donor scar. A fortunate
side-effect was that thinning hairs became thicker. Based on that observation, we've
spent many years developing a process of injections in a way that can stop progression or at least
significantly slow it down, prolong the growth cycles, thicken thinning hair, reactivate
hair that's not growing. This has been a significant improvement in our approach to
helping people with genetic pattern loss.
Now looking at a younger person, I would say in the category of younger males, I would
say that by definition, by having a younger onset of hair loss, you fall into a more aggressive
hair loss pattern. And I would even propose that there is, in terms of relative sensitivity
to DHT or dihydrotestosterone, that men in your category are more sensitive to dihydrotestosterone.
Now, you've been on finasteride and you feel it hasn't stabilized but I would consider
that in actuality that you would probably slowed down your progression by reducing the
level of DHT. But unfortunately, the nature of your hair loss is such that the growth
cycles are shorter. And therefore, even with the prolongation of the benefit of a DHT blocker,
your hair growth cycles are stopping earlier than before puberty and resulting in this
more progression which is different from your contemporaries. The question we always have
to ask is why is that 80% of your contemporaries still have hair. Now as you enter your 30s,
40s and beyond, the percentage will grow of male pattern hair loss.
So it has been my observation that essentially, there are two groups of men in their late
teens to early 20s who develop hair loss. One group has a more highly aggressive and
more advanced hair loss which I think might be likely the category you fall in and the
other group has an early onset but moderate progression. They may have diffuse thinning.
They may still hold onto hair longer and get into their 30s. They may still have significant
hair loss but it will happen more slowly. As opposed to your category which may lose
hair faster because of your early age of onset.
With Hair Regeneration, we've been able to help people in your category prolong the
growth cycles and get past several years and up to 5 years. But differentiating the highly
aggressive to the moderately aggressive, I would say the moderately aggressive tends
to last longer with our injection treatment compared to the people who are very highly
aggressive. And we of course discuss that in consultation.
Now as far as the options you have moving forward, I would say that you are almost stuck
in a way because you now have taken that first step of doing a transplant. I would certainly
expect that you would maximize or continue taking your finasteride on a regular basis
but I would say that you are, as far as future transplantations are concerned, it depends
a lot on how much more hair you lose and in what timeframe.
Generally speaking, from my training from when I did facial plastic surgery and studied
hair transplant and for the decades to follow, it has been a general standard in our community
to not operate on someone who is young because of their relative rate of progression and
what is the desirable outcome and what is an acceptable outcome. You know, when we think
of someone who comes in in their 40s and they want a hair transplant, well, they can handle
having relatively smaller grouping of hair that can look appropriate for their age group.
We've seen patients in our practice who are well into their 30s who have had 4 transplants
and the top of their scalp looks like they barely have any hair and they have very wispy
hair.
Now there are options that exist beyond these numbers that are getting thrown around especially
when it comes to the beard hair option is body hair transplantation. And although, we
don't currently offer that in our practice, I have seen very good results with that and
it might be a strategy that will help you maintain a certain amount of coverage as you
get older. So certainly, the question about ethics is still a question that is not appropriate
for me to give you an opinion on. Basically, from my perspective, it's not about ethics.
It's about really what kind of results can I meaningfully deliver to my patients. So
I think that it's critical that you get the maximum benefit of your current treatment
strategy. And so there may be a value for you to consider something like Hair Regeneration
to help enhance the benefits of finasteride. And also I didn't mention, we also help
people who have gotten transplants and we've been able to improve the quality of their
hair transplants as well.
So learn more about these other options. And certainly, discuss this concern you have with
your surgeon and hopefully, your finasteride will continue to work. We've had history
from patients where they said that they've lost a lot of hair in terms of their progression
and then they were stable for a while and they started losing again. So one can hope
that may be the case for you so that you can go into your 30s without having significantly
more loss but it's very difficult to predict the future and a lot of these patterns that
I'm describing, young highly aggressive, young, moderate progression, is based actually
on reviewing the patient's history and looking at their scalps at different stages as opposed
to trying to predict from one point forward. But the early age of onset is certainly an
aggressive pattern.

You submitted several photos and you expressed in your question that you had lower eyelid
surgery done approximately 3 weeks prior.
And that you are concerned in particular with the appearance and sensation of the left lower
eyelid where you feel like it looks a little bit like pulled down and you're seeing a
little bit more of the inside of the eyelid with the left eye.
And you're asking about whether or not this appearance is normal.
Well, I can certainly share with you my understanding of your situation with just the photos alone
with the absence of a physical exam.
A little bit of background, I'm a Board-certified cosmetic surgeon and Fellowship-trained oculofacial
plastic and reconstructive surgeon.
I have been in practice in Manhattan and Long Island for over 20 years.
I am very well known for revision eyelid surgery and patients come to our practice from all
over the world for complications of eyelid surgery such as lower eyelid retraction and
ectropion and things like that or problems commonly caused occurring after lower eyelid
surgery.
So to begin with, this appearance that you have is actually fairly common after lower
eyelid surgery.
Not necessarily a good thing but it is an approach that a lot of plastic surgeons and
facial plastic surgeons prefer to do which is an external approach to the lower eyelids.
This is called a transcutaneous blepharoplasty.
And generally speaking, in my experience of seeing patients like yourself, the surgeon
will approach the lower eyelid fat pockets and address everything by making an incision
underneath the eyelashes and getting into the fat pockets and then removing some skin
with the desire to improve tension in the skin of the eyelid.
It is generally my experience that patients will end up with their eyelids being rounded
and pulled downward.
And in some situations, the eyelids will actually evert and the term is ectropion.
Now the reason why it's an interesting question whether or not it's normal, well it depends
a lot on the philosophy of the doctor who is performing the surgery.
In my practice, majority of my procedures I do from the inside of the eyelid.
It's called a transconjunctival blepharoplasty.
And it is essentially, with my understanding of the approach and my preferred approach
to helping people with under eye bags by working from behind the eyelid and preserving the
skin and muscle in the front which then begs the question what about the fine lines and
wrinkles under the eyes?
Well, what we generally do is instead of trying to remove skin, we actually use lasers and
platelet-rich plasma (PRP) to improve the skin quality.
And this combination for me has been very effective.
And since we do so many of these procedures, certainly with over 20 years of experience,
I can tell you that the overwhelming majority of patients maintain the position of their
eyelid and the shape of their eyes.
Therefore, for me, it's the preferred procedure.
Now that being said, when patients like yourself have come to my practice very distressed about
their appearance where you're seeing too much of the white of the eye and that's
referred to as lower eyelid retraction.
And if the eyelid is everted, it's called ectropion and for you, it's not exactly
everted but there's a gap.
What I tell those patients is that unless there's immediate issue related to eye health
or integrity, where the ability to close the eye and protect the eye is compromised, you
generally have to wait and allow for some return of muscle function and some degree
of elastic stretching of the tissue in order to see if it returns to its better position.
Generally speaking, when situations like this, your eyelids will likely at least not evert
away from your eyes with time.
You may want to look at your before pictures and compare and see how much of the white
of your eyes were showing before surgery and certainly we don't have the benefit of seeing
those photos with your question and seeing what the eyelid position looks after surgery.
It's important that you have this discussion with your doctor.
Your doctor and you made this decision to approach the surgery in this particular way
and I'm sure your doctor explained to you what to anticipate.
And of course, it's important to maintain the follow-ups.
Now, it is again necessary to at least understand what is happening and a lot of times, a doctor
will recommend in a situation like this basically one intervention and that is upward massage
where you push the eyelid upward and try to get some stretch so that the eyelid can actually
move upward and the scar tissue or healing tissue doesn't pull the eyelid further down.
And that's probably the one therapeutic measure that is essentially universally accepted
as something you should do at this stage of your healing process.
In the absence of feeling the eyelid and being able to see if it's tethered or stuck, I
think that might be worthwhile doing again after contacting your doctor and asking about
that.
