hi I'm Tati and this is my November TBR so if you saw my October TBR video you know
that I had a very ambitious amount of books that I wanted to read in October
and unfortunately as you may have already guessed I did not read all of
them so for this TBR I'm basically going to be trying to read everything I didn't
read in October with maybe a few exceptions so I'm going to be going
through these titles pretty quickly because I did already talk about all of
these books in my October TBR video which I will leave linked in the
description if you want to hear me kind of explain what they're about a little
bit more so we have When We Caught Fire A Map for Wrecked Girls A Study in
Scarlet A Study in Charlotte all of the books in that series that are published
so far I did find out that there is a fourth book coming out next year Flame
in the Mist and Smoke in the Sun Illuminae Gemina and Obsidio A Whole New World the
complete Darkest Minds series and the To All The Boys I've Loved Before trilogy
and from my count that is still 20 books and that is a lot more than I was able
to read last month so I probably won't get through all of them this month
either but I'm going to try and I do have a few add-ons to this list that I
had to add on because they're new and I don't want to wait to read them so these
are my exceptions I have these on hold at the library and I'm going to probably
read them as soon as I can get my hands on them number one is the Darkest Star
by Jennifer L Armentrout this is the start of a new series that is set in the
Lux universe I personally really love the the Lux series and I cannot wait to
get back into it with this new book next is Otherearth by Jason Segel and
Kristen Miller which is the second book in the last reality series I read the
first book Otherworld during the summer biannual bibliothon this year and
I completely forgot that the sequel was coming out in the fall of this year
until I was editing my footage for my biannual bibliothon vlog so as soon as I
saw that I went and put it on hold I also put a hold on Little white Lies
by Jennifer Lynn Barnes which I don't know if it's out yet I don't think so
this is a story about Sawyer an eighteen year old girl who takes up her
grandmother's offer to participate in debutante season in the hopes that it
will help her solve the biggest mystery of her life I am very excited for this
one because I love Jennifer Lynn Barnes's mysteries especially but I
haven't ever read a book by Jennifer Lynn Barnes that I didn't like so I'm
looking forward to this one so those are all the books I want to read in November

booed and then you put them on someone's door my mom does that at her school
sometimes we got these towels in the dollar section and that's pretty much
our only Halloween decorations that we do here because we don't live where
trick-or-treaters come because it's like quite a long distance between houses and
well we're at Target they have the teal pumpkins or the teal buckets or whatever
they call it but anyways I was like hey that should have been around and I was a
child because trick-or-treating was always a little bit of a risk but
I think that's all that happened yesterday well I could very much TMI
your whole life but well mine is well so we all know that my innards don't work
very well that's why I need to have the pill cam and everything but unless it's
that my body is actually decided to adjust the food instead of just kind of
you know not like it usually does there's things rotting inside of me or
unless it has decided to absorb the food because it has been about four days wink
wink wink I really can't wink and I used to be
able to like remember all the other day I was like trying to move my eyebrows
ready but I can move them in and make my forehead all wrinkly but oh I did it
moving them up I used to be able to move them like where this one would have come
up and in that's one go down and in and then
opposite like really easily but now
pretty much the only thing I can really do I don't even know if I move my
eyebrows up technically when I'm doing that I think I'm just moving them from
not down I want to go to Trader Joe's today so hopefully this will come home I
think she's planning at her school right now and yes I am gonna wear the unicorn
horn as long as my head will allow it because I usually just don't wear things
on my head but I am so pumped for Christmas time this year well I guess I

hair spiders they're those like clumps the hair that go and that when their
hair goes into the dryer they get all like shriveling yeah that's what I call
a hair spider but look me I got these I think it Clare's last year my all got
them for me and I would wear these to Target and stuff and people were always
like you look so festive like I don't know why you would say that and my Santa
hat people also really like it when I wear this in public because they're like
oh wait I thought there was another oh these ones I got these for Christmas
last year there kitty ears let's try these on they're like my unicorn horn
but without uh-oh I don't know if I can wear these ones though because they kind
of hurt my head I don't like things to be touching my head or oh these ones
kind of squeeze my head but that's okay cuz look how cute they are
I'm gonna be a head be underwear like look it just looks fantastic earlier
today I don't I know bad I don't log into my computer every day yeah I'm
putting them all back um but a while back I had commented on the channel my

chips or something in the hotel and the chips had something on them because we
had like snack hour or something at the hotel I don't know what why why I even

away once you got to the hot glue part off to our house and today this is
actually going to make the spaghetti mm-hmm and I break off some meats and
I'll vlog her making spaghetti because I have my doubts that it's gonna work out
AG anytime we are cooking the spaghetti and I helped her do the garlic that's a
lot of garlic well that's also a lot of onions so I guess we're making a lot of
spaghetti we're making the spaghetti that Priscilla makes on pots pans and
Priscilla so I'll show you when we're actually cooking because right now we're
waiting for our meat to defrost spaghetti update I am watching bunny
well that's a commercial right now so Shh quiet phoned and that is what the
spaghetti looks like so far that is just meat and onions and garlic
and then she set up the strainer for what I don't know but oh I don't know I
don't eat meat so I won't be eating this with them I'm just helping her make it
my favorite part about cooking is just stirring so if you like I could sit
there and stir a bucket of ice and water and I'd be perfectly happy person cuz I
just like to stir things that's why I like to make the banana bread cuz it's
really mushy and when you start it like me there's a lot of noise in here
because there's a lot of things playing but well not done it needs to cook more
but they don't eat dinner for a while it's pretty much my dinner time though
so that I can eat dinner and then go to bed
I finished dinner but here's the finish or I finished eating dinner and here's
the finished product of their spaghetti and now I'm gonna take a bath and go to

was probably because a patient showed up to the ER
with chest pain
trouble breathing
had a sweat going
and looking real real bad.

just a little pain her jaw.
So how do we know that it's a myocardial infarction
or a "Heart Attack?"
Remember "infarction" meaning tissue death
due to a blockage of blood flow
and myocardium meaning the heart's muscle tissue.
So, no blood is getting to some part of the heart
and that's a problem.
So, if a patient shows up to the ER with
any indication they might be having a myocardial infarction
The ER doc is going to order an electrocardiogram,
or an ECG or EKG
same difference
they both mean: electrocardiogram
The EKG is a paper print out of the electrical activity
in different parts of the heart
over the span of seconds.
Now reading EKGs ain't easy…
and that's the reason why doctors are the ones to say…
"Call a Code STEMI."
Because they've had much more training in reading EKGs
and even then, they talk to our wonderful Cardiologists
that have had extensive training in reading electrocardiograms.
"Hey heart doctor?"
"This is the ER doctor."
"I've got a patient here who's EKG looks like a myocardial infarction"
"But, you're the heart specialist, you tell me what you think.
Now, just because we have our ER docs, and hospitalists
and cardiologists floating around the hospital
doesn't mean we, as nurses, don't need to know
what a heart attack looks like
because it can strike at any moment.
Picture yourself on a telemetry floor
where all the patients are wearing cardiac monitors
with their heart rhythm showing on a screen
at all times.
Your patient just finished their chocolate boost and pureed peas
and they need hit the commode
and pronto.
So, you're in there with your gait belt
your air freshener and your clothespin
helping them transfer from the bed, to the commode.
Suddenly they start clutching their chest and saying
"Oh boy I feel weak."
"Ugh, it hurts to breathe!"
You're gonna glance up at the cardiac monitor
to see if there are any rhythm changes
and if there are
you've got some calls to make.
Because remember
as nurses we're the eyes, the ears
and the nose
kept on the patients, for the doctors, when they can't be there
which is the majority of the time.
So, while they don't expect us to know a cardiologist level
interpretation of an EKG
they do expect us to know the tell-tale signs
of a major event
like a myocardial infarction.
So, that's why on the NCLEX...
you're gonna see a question like this...
"The nurse is caring for a patient that suffered from a myocardial infarction."
"While monitoring the patient's EKG for signs of a myocardial infarction,"
"the nurse may observe which of the following?"
Now, I like to say "I" or "You" whenever I'm doing these questions
because it's gonna be me or you in this scenario, right?
Now I also like to "de-robotize" and "re-humanize" these questions
so they sound more real world
and less like those well-rehearsed BLS video scenarios.
Now I don't know if you're the type to say...
"Doctor…"
"while I was monitoring the patient's EKG"
"I observed signs that the patient might be"
"suffering from a myocardial infarction."
But that clearly ain't the way I talk
and that means, it ain't way I think..
Then I'll remove redundancies and carefully rephrase it
so, it's easier for my brain to compute.
"I'm carin' for a patient that's had a myocardial infarction."
"What am I gonna see on the EKG"
"that shows signs of an MI?"
That makes a lot more sense to me.

Now my doppelganger that ran off earlier
He gave you the first answer already,
and that is: "ST Elevation."
"ST Elevation" means the "ST" part, or "segment," of the rhythm is elevated.
hence the name STEMI.
ST, segment, Elevated Myocardial Infarction
Now, the ST elevation occurs when part of the heart ain't getting any blood
because one of the coronary arteries that supplies it
is completely blocked off.
And we all know what happens when tissue doesn't get any blood.
It dies.
Not good.
Correct choice number 2 is "Pathologic Q-wave"
Now you might say
"Ay! Nurse Stefan, wait a minute."
"Normal Sinus Rhythm has the Q-wave"
"What gives?"
and you betcha' it does.
And that's why Mother NCLEX threw in there
that IMPORTANT word: "pathologic"
"Pathologic Q-wave" means the wave was caused by a disease process...
this one being caused by myocardial tissue death.
Correct choice number 3 is "T-wave inversions"
This is yet another sign that could indicate myocardial injury or infarction
in this case, again, a full blockage of blood causing myocardial tissue death.
So, there are your three correct answers
but let's talk really quick about the three incorrect answers
and why they're not correct on the test.
The "U waves" are thought to be caused by
repolarization of the Purkinje Fibers or papillary muscles
which can be found in healthy patients
but also, may be a sign of electrolyte imbalances.
Not necessarily a MI.
Incorrect choice number 2 is "Prolonged PR-intervals"
which means slowing down of the signal
between depolarization, or "firing" of the atrium
and depolarization, or "firing" of the ventricle.
We'll see these prolonged PR-intervals, in first degree heart block.
And the last incorrect answer is "Irregular PR-intervals"
which can be thought of as inconsistent lengths of the PR-interval.
We'll see these irregular PR-intervals
in other types of heart block
like second, and third-degree heart blocks.
So for the purpose of the NCLEX
you should be connecting
"Myocardial Infarction EKGs"
to "ST Elevation"
"Pathologic Q-waves"
and "Inverted T-waves"
Now I remember this by saying
"Stefs a cutie."
Remembering that the Q and T waves
both have something unusual going on with them
and that mnemonic phrases
aren't always true at 6 in the morning.
CHILD: "ha-ha"
