Why PRP Can Improve Neck Skin but Cannot Lift, and Options for Neck Rejuvenation - Duration: 7:42.

Thank you for your question.

You submitted a photo of your neck and you state that you're 36-years-old and you have

an area of what appears to be loose skin and you're asking if PRP will be helpful to

treat this area.

Well, I can certainly share with you the way I counsel patients who come in with very similar

situations as yours.

A little bit of background, I'm a Board-certified cosmetic surgeon and Fellowship-trained oculofacial

plastic and reconstructive surgeon.

I have been in practice in Manhattan and Long Island for over 20 years.

And helping people with their neck in terms of rejuvenation through a multitude of options

is something that we do in our practice every day.

And especially significant is our use of PRP in our practice.

So, I'll help you understand what PRP can do and cannot do.

So to begin with, the choice of any type of intervention is based on a proper diagnosis.

And also understanding of course the limits of the tools you want to apply.

So we'll start first by getting a better understanding of what PRP is.

PRP stands for platelet-rich plasma.

Platelet-rich plasma is essentially a concentration from your own blood of the wound healing and

growth factors appropriate to stimulate collagen, to generate blood supply and have a lot of

different things that can do to help tissue essentially improve.

So when we use PRP in our practice, we use it for acne scars, we use it for wrinkles,

we use it for skin discolorations, under eye dark circles, we use in conjunction with Acellular

matrix for hair loss.

Now a lot of times, people feel or interpret PRP as being able to fill as well as to lift.

And in either situation, it's really about a perception.

Let's start with the filling part.

When you are trying to improve an area's appearance of volume, PRP can stimulate the

fat under the skin to blossom and increase in volume and that can look like a filler

but it's not going to be the same as an injectable filler.

As far as lifting is concerned, in an area, let's say over the cheekbone or in an area

where there is a layer of fat that can help be a cushion and may create some volume subtlety,

then it can lift.

Now in your situation, the diagnosis is probably not appropriate to use PRP as a standalone.

PRP can help the neck skin texture, it can help the neck skin quality.

But without a benefit of a physical examination, what it appears is you have a platysmal band

which is a muscle called the platysma and that band represents an attachment to the

skin.

And a lot of times, we can interpret this as a splitting of the muscle.

Sometimes it's really just about a localized or an area of atrophy which makes the muscle

more prominent.

And you are essentially dealing with a muscle that there are a variety of options which

can be used to address that muscle, everything from minimally invasive such the use of Botox®

to relax the muscle as well as moderately invasive or something like a suture suspension

procedure and more aggressive such as platysmaplasty and neck lifting surgery.

So, I think that is more appropriate based on the diagnosis.

Of course, an evaluation is not limited to the narrow area of the photograph of your

neck but a proper evaluation.

When I see my patients in my practice, I take pictures and I have them lift the different

areas and I show them and review their photos on the screen about what we can possibly do

based on what the issue is and then they decide with what they're comfortable with.

Sometimes, it's a stepwise approach.

Sometimes we do a little Botox® first, see how things look.

Then if that isn't as beneficial, we look at other options.

We look whether or not there's truly redundant skin or is there volume loss in the face.

And in that case, we can actually do something called structural volumizing to literally

expand the volume in the face, to help lift the neck but that's a different strategy

and it depends on your facial anatomy which we can't appreciate with the photo you submitted.

So I think that you will find that you will get a lot of opinions about what's best

for your neck.

And I think it's important that you get comfortable with the doctor that can guide

you in a way that hopefully you'll feel comfort with.

There are a lot of options and there are a lot of technologies that are being promoted.

And there's certainly no shortage of people that are ready to offer their best solution.

And I think certainly that majority of the time it's well-meaning but a lot of times,

it might miss the mark unless there's good communication between you and the doctor.

So meet with qualified, experienced cosmetic surgeons.

I think that would be the ideal specialist, a cosmetic surgeon, someone who is able to

do everything from minimally invasive to more advanced cosmetic surgery to give you an overview

based on experience as to what would be best for a 36-year-old woman who has this issue.

Now let's check out the output of it, get back to your project, reload and we can see

that we can successfully change the placeholder color.

Now if you want to change this opacity mode then change it to 0.5, and you will see that

this is the half of its opacity as it was in its previous case.

So you see that we have practically copied all those things from here, and we have used

this class inside our custom.CSS, and immediately after placing it into our

custom.css, and attributing the properties we can now see that all those changes are

not taking place.

So we have made successfully those changes over here so far.

If you wish to change this color, this box Shadow property, where is the border here?

We can see that the border color is here and it states that it has this class.

It is practically using this class now; .has-success and .form-control.

And it has a box shadow of this one, so we just...

Now all we have to do we have to copy this one.

Let's get back to bootstrap.min.css, find one, and you can see that here are they.

And now you can see that this is the focus condition.

Practically it states that under this focus condition, this class .has-success , and this

class name .form-control, they will be having this

border-color property and this box-shadow property over here.

So now all we need to do first, we need to copy this class name, getting back to custom.css

and once again we are pasting it here.

We have successfully copied this class name out of this bootstrap.min.css, all we need

to do we need to add this .banner7 class, here..

So all I am doing here, in the previous case, on focus condition these input elements, those

were providing there a greenish box-shadow.

So all we need to do first, we need to change this property here.

Let's say we are providing yellow box shadow, in the same manner here.

Press control + s, get back to your project and reload.

Now we can see that on Focus condition, this is practically providing a yellow box shadow

instead of its previous green one.

But these changes will not take effect in other classes such as this one or in this

one, all this things will remain the same cause this property has been attributed to

has-success class name only under a focus condition.

On hover condition it will be executing the same properties.

So all we need to do, you need to copy this codes first, come back here, paste it,

change it to hover, press control + s, get back to your project and reload.

Now we can see that on hover condition, it is practically using box-shadow of yellow

color.

So we can successfully change this box Shadow property both under hover condition and under

focus condition.

And once again considerably you can see here no changes

are going to be placed here in this bootstrap.min.css.

Practically this is a safety precaution from A developer's view.

If your project gets weird under any circumstances, then if you want to undo all

those changes those you have made so far, and if you are going to make those changes,

if you are going to undo those changes into this bootstrap.min.css, practically

it will be an impossible task to do and probably you can never achieve it.

So if something goes weird then all we are going to do we are just going to delete

all those things and everything will be in a reset condition.

This is practically a tactical movement from developer's

point of view.

Roger Penrose, Fashion, Faith, and Fantasy in the New Physics of the Universe - Duration: 4:11.

This book of mine was published very recently in English.

I understand that it is soon to be translated into Polish.

The title "Fashion, faith and fantasy in the new physics of the Universe" was a title I

gave to three talks I gave in Princeton about 13 years ago.

So, it took me a long time to write the book up.

The three topics are to do with physics - physical theories which are very prevalent these days

and with which I have certain reservations.

One of them is string theory that is the fashion which is still fashionable, although not quite

so fashionable as it was when I took on the project, but the complaint I have mainly

is not so much that string theory does not have any implications which are

testable with current technology, but also that the main problem I have with it is that

it requires space to have more dimensions than the three that we experience.

And in my view if space has more than three dimensions there would be very grave problems

with trying to make it mirror the universe we know.

And this depends on some mathematical difficulties which I discussed in the book.

The faith is a much more important theory in physics, namely this is quantum mechanics.

Quantum mechanics quite unlike string theory as huge numbers of implications which are

explained by the theory and which we see in the current physical world.

So, there's no question about quantum mechanics being an excellent theory.

The faith, however, is that this theory applies at all levels.

So, this is the problem that Schrödinger pointed out with his cat. So, he said if the

theory is correct according to his own equation you could have a cat that was alive and dead

at the same time.

And this is nonsense.

So, there must be something wrong at some level with the theory.

So, this is the argument that I make there.

The fantasy had to do with ideas of cosmology, most particularly the inflationary theory

of cosmology which is a current important essential feature of current cosmology.

If inflation is not correct which currently I believe.

I don't think that the inflationary - this is a phase which was supposed to have taken place

in the first, well, 10 to the minus 32 seconds – it's a tiny tiny tiny tiny fraction of

a second which this universe was supposed to have expanded in this exponential way.

And I have the view that this doesn't explain many of the features it's supposed to explain,

but on the other hand it does have a role in current cosmology which has to be replaced

if it's not there by the scheme which I do describe briefly in this book which is my

concept of conformal cyclic cosmology according to which the Big Bang was not the beginning,

but there was – prior to the Big Bang – an expanding universe and prior to that that

had a Big Bang and there was another expanding universe before that.

The universe in my scheme never collapses.

There are other theories where the universe goes through cycles.

In my scheme it never does that, but it's hard to explain that certainly in a few words,

but these ideas of mine are also described in the book – in the fourth chapter where

I show that there are alternatives to fashionable [string theory] or the faith that we have

in quantum mechanics at all levels and the fantasy which I believe is not… it's my own fantasy

which I describe also in in the final chapter of the book.

To be honest, I got inspired by his video and by the comments. Everyone was so supportive.

People told him how they were emos before too. They created a community. I want one too!

So it means I will talk about one thing..

One thing..

As I suffer from ... In my life.

There's one thing that haunts me everywhere. Everywhere. Whatever I wear, where I am, what I do,

what I say.. No matter what, it follows me everywhere.

Guess what.

You may already know.

Perhaps you thought my age was 14.

As I were for about four years ago.

I remember that moment when we talked about politics.

You know that I love politics..

.. Even if the world won't accept it.

I was talking about politics, I told why I thought that Sverigedemokraterna (SD) is a Nazi party.

It felt like when I was talking about politics, my soul got clean at the same time.

Atleast until someone told me to stop talking about politics.

J: .. racists, because they .. What, why?

(WOMAN): You're not 18. You can't even vote now. So it's useless for you to talk about politics now.

But .. I'm 18!

After the happening, talking about politics became scary.

Another story!

I was on a camping place with many deaf people together with my mom.

Someone deaf came to us.

(WOMAN) You have a beautiful daughter! She has grown up!

Grown up? Yes, I have, of course!

(WOMAN): So, will she start at the 7th grade in middle school now?

Different story.

I imagined ourselves married with sushi rings. We looked at each other and kissed each other.

I can't do this anymore.

I don't know why it happens to ME at all the time.

My friend told me it was because I'm little, so I started with heels.

What in the fuck, it was so painful!!

- Uhh okay. Then maybe you should wear more makeup?

I'm a student. I have no money.

- Get a car, then.

Okay. I'll get a car and I'll get stopped by the police regularly, because they do think I'm a 12-year-old who

took my parents' car and livin' the GTA life!

But there's one thing I'm surprised about - no one talks about the benefits.

We're the squad other 40-year-olds will bitch about. They will have a problem with us.

Why? Because when we're 40 years old, we will look like 20-year-olds, when the they will look like 80+.

We're gettin all the young boys.

Actually, we will always look like 20 year olds no matter what.

Like Twilight. But in the real life. We do not age.

And.. When we have cute babyfaces, nobody messes with us.

Because we're innocent, right..?

Buying clothes or shoes? Go to the children's department. HALF PRICE!

About money.. We'll never have the need to buy cremes that makes us look younger. We have it for free.

When buying pizzas, we do always get candy for free because they thinks we're kids.

Expensive tickets? Children's tickets!

The benefits are many, and yet nobody is talking about them.

Today, we're living in a world where the norm is about lookng older.

I mean, look at all the 12- year old's Instagrams! Almost every one of them looks like they're 30 years old.

When I was 12 ..

I was 12 year old.

So ... So ..

So stop the hate.

Spread love instead!

Lucky enough over the years I've ridden in so many

amazing different countries.

Canada, Mexico, France Spain, all sorts.

but right out my door, lovely single track,

and sometimes that's all I need.

- [Interviewer] What sort of riding do you love to do?

- Do you know, just what we've been doing, single track

Nothing beats it, I swear.

- Bucket list.

Do you have a bucket list trail that you want to do,

or mountain you want to shred down, or ...

- Do you know, I'm also lucky, I've kind of

ticked off what was always my bucket list,

and that was a trail called the Whole Enchilada

in Moab, and that's kind of got a bit of everything.

It starts up at high Alpine, and drops all the way down

to the canyon floor.

And I think it's like 27 miles long.

- Yeah?

- Like, of pretty much all downhill, yeah?

- Wow. - And it's got everything.

You've got roots, or schralpy turns at the top,

you've got river crossings, it's just ...

The famous slick rock, the canyon edge stuff,

it's just incredible.

- That's going on my list.

- Let's go tomorrow.

- It's pretty brutal, as well.

- Right, I've got one more question.

You're sat down on your bike,

CLASS REUNION

Fabio de Jorge Abraão Leite wants to know if Seth was a fricking idiot in college.

I'm at my 25th college reunion, I graduated 25 years ago from Yale

and now my class is getting together.

This university is at least 300 years old.

(It's 316.)

People bring their children to reunion.

And let's see how the food is today.

OK folks, now we're going into the residential college

where I lived when I was studying here.

Everybody who studies here at Yale, from their 2nd to 4th year, lives in a residential college.

Mine is Branford, which is really great.

It looks like something out of Harry Potter.

[ALERT: non-professional cameraman]

Now we're in the first courtyard.

[Also possibly drunk.]

There are four courtyards in total,

And this is where the students live.

It's incredible, we're practically living in a castle.

And you live with the same people for four years.

This is the main courtyard, I don't remember how to say courtyard in Portuguese

I'll put it in later.

Here's where we, I don't know, played football and baseball,

and the coolest thing is in this college we have the symbol of Yale, Harkness Tower.

Very impressive.

So it's really a community, you sleep here, you wake up here, you have breakfast here,

and then you go to class elsewhere in the university.

It's something very special about Yale - well, Harvard has it too -

but not many colleges have this system which creates a community within the community

It's practically a family for those who study here.

Let's go into the second courtyard.

As you can see, it's even a little romantic

and the incredible thing is I dated someone who lived in the same residential college.

That's great, because you're living right near your girlfriend,

but it's also a slight problem when you break ups with her.

Luckily, I didn't break up with her until after I graduated and got out of here.

[Actually, she broke up with me.]

So I'm spending like 80% of my time at this reunion with people who lived in my, in Branford, my college.

I know them much better than I know the others.

Even the cameraman was in Branford, Chuck, I'm going to show him here.

And now let's go into the last courtyard, I still don't remember how to say that in Portuguese.

It only costs like $60,000 per year, or $80,000, I don't know, to study here.

I don't know, but it's much more than when I studied here.

To me appreciation means servant leadership.

It means being there for the individuals who are on the front line. It means

walking the walk, and if you talk the talk you have to walk that walk as well.

Our front-liners, our housekeeping staff, our cooks in the kitchen who are working

with students on a regular basis and seeing them one-on-one...there's a great

appreciation for what they do and how they work with our students.

We're a pretty large institution and in that regard it takes a lot of people to make

it work the way it needs to so they're the ones that really are knowing

what's happening on a daily routine basis. I think the campus in general has

a tendency to step out of the box and say if they see something positive to

acknowledge it and I think that's what really keeps the institution even more

vibrant and going forward.
