Hi everyone, how are you?

Today we are going to talk in respect to the pulp

Before properly talking about the pulp diagnostics

I would like to talk a little in respect to the biology of the pulp

The biology of the pathological events that will lead to pulpitis

and finally pulp necrosis

Let's go?

So let's start by remembering the following:

Three things aggravate the pulp

Caries

Trauma

and the dentist.

You think it's funny that I said "dentist"

You were waiting for me to say iatrogenesis.

No, it is actually "dentist" that I wanted to say

Because, honestly, iatrogenesis, is a beautiful word to substitute an ugly one

When a dentist goes and does something that isn't really correct,

We all say that the doctor committed another iatrogenesis

To avoid saying the the doctor did a **** to your tooth

No, I want to say that it's actually the dentist that aggravates the pulp

And iatrogenesis even more

Why?

Imagine a normal situation

you are doing a shallow resin cavity restoration

very shallow

plenty of water spray, for everything to be correct

In this case, is there something wrong?

No, there is nothing wrong

You are not committing an iatrogenesis

Right?

But you are aggravating the pulp

Just the fact that the pen is in contact with the dentin

spinning and cutting, even if it is very cold

the pulp is being aggravated

So, what aggravates the pulp is not only iatrogenesis

Even the most accurately done procedure will also aggravate the pulp

Of the three factors, the most aggressive is caries

More than 90% of the endodontic treatments have to be performed due to dental caries

This is not because of the intensity of the aggression

but due to the the duration

The bacteria stays for an undetermined time in the carious lesion

and doesn't leave until someone takes it out

While the bacteria is in the carious lesion, it produces toxins

These toxins go through the dentinal tubules and reach the pulp

continuously aggravating it

And how does that reflect in the severity of the symptoms that the patient experiences during Pulpitis?

To understand this, we have to remember the pulpar anatomical situation

the fibers

and vessels that arrive there

The pulp is a connective tissue

therefore vascularized

with many nerves, but with a very specific anatomical characteristic

It is found completely surrounded by hard tissue

The pulp is always in a normal situation where it is completely surrounded by the dentin walls

Which are hard

rigid and don't expand

And it is because of this, during an inflammatory process, there is no way for the pulp to swell

In any other bodily tissue, immediately when the body suffers an aggression

an inflammatory process is initiated, and we can see that this tissue

swells

It looks like an edema and grows

And there is no way for this to happen with the pulp

The inflammatory process can happen in one of the three processes that I mentioned

Caries

trauma

or the dentist

I'm going to stick more to the the biological process

caries

because it is the most common

In this case, a colony of bacteria will stay housed next to the pulp

initially, without contact with it

However, the bacteria produce a series of toxins

at the same time that they destroy the dentin

and the caries grows

Part of the constitution of the dentin

aside from collagen and hydroxyapatite

Is a set of very important proteins

called bioactive molecules or growth factors

I will highlight one of them

There is a bioactive molecule called TGF beta

that is part of the constitution of dentin

When the dentin is destroyed

the TGF beta is released

it falls into the dentin tubules and reaches the prolongation of the odontoblast

The odontoblast is in the most peripheral layer of the pulp and makes contact with TGF beta for the first time

What is the reaction of the odontoblast when the TGF beta arrives in it?

It realizes that there is something wrong

and initially begins to mineralize the tubules

and produce a harder dentin

Then it tries to produce an extra layer of dentin

in the pulp and flees to the pulp

As the caries evolves the odontoblast will take all these steps

Only if nothing is done, the caries will continue to progress

and none of this is going to help it

Ultimately the inflammatory process is so intense that the odontoblast will die

In this case what do we have?

An inflammatory process that is only increasing

mainly characterized by the increase of blood at the site

For us to have more blood we need to have

vasodilation

So, the blood arteries of the pulp will dilate more and more to bring more blood to that place of aggression

We have a deep carie

very close to the pulp

but often without yet exposing the pulp

A very intense inflammatory process

with the arrival of blood increasing each time

This blood coming up to the pulp chamber

where is it coming from?

To get there blood comes from an arteriole

that will penetrate the apical foramen

Which is a rigid, hard hole, in the root apex

And this apical foramen penetrates

aside from this arteriole

a very fine vein to bring the blood back

and at least one nerve bundle

So, in a normal situation, these three "people" pass through this little place

It is only in the case of a very intense inflammatory process

that the arteriole grows well beyond normal

occupies an entire foramen

crushes and squeezes venola against the wall of the foramen

It also tightens the nerve bundle against the wall of the foramen

and an irreversible process is set in motion

The blood arrives but can not return

because the vein is completely pressed against the wall of the apical foramen

This is chaos

from the tissue's point of view

In the pulp chamber, the blood that is arriving and has nowhere to go

ends up spilling out of the blood vessels and initiating a coagulation process

The pain at that moment is unbearable

because we have an inflammatory process with a neurovascular bundle being pressed

We have an inflammatory process in which we begin to lack oxygenation

because we do not have a renewal of blood tissue

The blood arrives and there is no way to leave

So, the evolution of this process will ultimately lead to tissue necrosis

Without oxygenation, the cells begin to die

A process of pulp degradation begins in the region next to the caries

and it evolves

from the pulp chamber toward the apex

until the complete tissue necrosis

hopefully you can understand more or less how the process of pulpal inflammation evolves

and why it culminates in tissue necrosis

THAT'S DEFINITELY A BIT BIZARRE.

WHILE HE DIDN'T HAVE A BAD RUN UNDER THIS GIMMICK, IT WAS NOTHING LIKE THE UNDEAD GIMMICK

WHEN HE FINALLY RETURNED TO THE UNDERTAKER ROOTS.

IT'S STILL A BIT SHOCKING THAT THE WWE WENT FORWARD WITH THIS IDEA.

HE HAS A KEEN FASHION SENSE.

THROUGHOUT THE YEARS, UNDERTAKER HAS BEEN KNOWN FOR HIS AMAZING COSTUMES AND RING ATTIRE

THAT WOULD PUT NUMEROUS COSPLAY EXPERTS TO SHAME.

IF YOU EVER HAVE THE TIME, SCROLL THROUGH THE DIFFERENT OUTFITS AND SWAG TAKER WORE

THROUGHOUT THE YEARS.

PERHAPS TAKER BUYING INTO THE GIMMICK SO WELL IS WHAT HELPED HIM GET SO POPULAR.

AT A PARTY.

BROTHER LOVE WAS THE FIRST MANAGERIAL CHOICE OVER PAUL BEARER, WHO WOULD GO ON TO BECOME

THE LONGTIME MANAGER OF THE DEADMAN.

REALLY, SOMEONE THOUGHT THAT MR. I LOVE YOU WAS THE BEST CHOICE TO MANAGE AN UNDEAD MAN?

HE USED TO ROLL WITH JENNA JAMESON.

ACCORDING TO JENNA JAMESON'S BOOK, "HOW TO MAKE LOVE LIKE A PORN STAR," SHE AND

That was no coincidence, she was just our March Fools' joke and the

story behind it is the following: At Christmas, Link_GER made a very insulting video

about me where he criticised my understanding of balance in a terrible

cORISmASS song. Since I am the great Jeff Kaplan from the Overwatch team, your

Lord and Saviour, I could not accept that and had the amazing plan to create

a joke character that is a combination of the heroes that Link_GER

considers to be the most boring: Reinhardt and Lúcio, and we named her after

Link_GER's grandma Brigitte. We got a lot of comments of people who found

our last new heroes have just been abilities of multiple existing characters

thrown together and wanted something original for once. You want something

original? Here you go: The actual hero 27 is Zelda. She has an

interesting and very unique backstory. So, way back when the

three Golden Goddesses created the world,

They also left a relic that holds The world together in its inmost folds

and gives the wielder nearly unlimited power,

the Triforce, deep within the Temple of Anubis

and ordered the goddess Hylia to watch over their creation and the Triforce.

With advancing technology, humanity eventually sort of started playing god, creating

armies robots that were superior to any life form. And as it was bound to happen,

the robots turned against humanity, starting the Omnic Crisis. Hylia joined the humans in

the war, but the Omnics were so powerful that their strength

exeeded even that of the goddess. Hylia then gathered her closest followers and founded

Overwatch, her elite unit in the war against the Omnics. After 10 years of war,

Hylia barely won, however, she was gravely wounded and the destruction was

tremendous. She knew that another war of this dimension

could not be won and that the Omnics would strike back. The only solution she

could find was to use the power of the Triforce,

but the Triforce can only be used by a mortal. So in order to save the world,

she made the decision to sacrifice her divinity and immortality and was

reborn as the human Zelda. Today, 20 years after the Omnic Crisis and after her

rebirth as Zelda, the Second Omnic Crisis is about to happen and she seeks to enter

the Temple of Anubis and acquire the Triforce. Now, Zelda might be a human, but she

inherited some magical abilities from her previous life. On the

battlefield, she uses these abilities to support her team, so she is categorised

as a support character in Overwatch. Her primary weapon is called the

"Bow of Light". She shoots Light Arrows that can penetrate enemies.

As her alternate fire ability she summons a Phantom to fight for her.

Once the Phantom is summoned, she can order it to go to a certain spot by pressing

alternate fire on the floor or she can order it to attack an enemy by pressing

alternate fire on the enemy. The Phantom has 300 health, 250 of which is

shield health. It's equipped with a sword and a

shield and as soon as it takes damage, it will hold the shield up and will

be invulnerable from the front for 1 second.

Now, it doesn't deal a lot of damage by itself,

but it has other purposes that we will get to later.

She can also summon fairies by pressing E that will stay in one place

and heal in a small radius around them. They won't heal much at first, but as the

Phantom deals damage, their healing rate will be increased until Zelda di*s.

In addition, enemies that are within the range of the fairies will receive 30% less

healing. Only 3 fairies can be summoned at once and their effects don't stack.

Her shift ability is a very unique movement ability. It's a teleport that

switches the positions of Zelda and her Phantom.

I'm sure you've all been waiting to hear what her ultimate ability is.

It's Nayru's Love. Using it makes Zelda cast a protective spell that surrounds everyone

on her team with a magic barrier. This barrier reflects 30% of the received

damage back to the enemy. This can be useful in a lot of situations. Just

imagine, your team gets caught in a Graviton Surge, for example, and the enemy

Pharah is about to rain justice from above. Then Zelda uses her ultimate ability and Pharah

di*s during Rocket Barrage, which is very rare because it's probably the strongest ability in the game

And lastly, Zelda of course also has a passive ability,

it's called Dying Wish. When she dies, Hylia's soul leaves her body and heals

everyone on her team within a 20 metre radius by 50 health before ascending to heaven.

That's all about Zelda, hopefully you will like her and enjoy playing her,

we gave our best to create a unique hero based on things we didn't have in

Overwatch before, magic, summoning and a support character with stationary

healing. Actually we started designing her right after Link went live on the

PTR last year and since then we have been testing a lot of different

abilities on her. For example, in the very first version she didn't have the Bow

of Light, instead she would cast fireballs and her alternate fire was a bigger and

slower fireball that would follow her crosshair. Shift and E were the fairies

and the Phantom. 10 Later we gave her the bow instead and made her primary fire

a player piercing arrow and her alternate fire a barrier piercing arrow, but then

we decided that we didn't want another barrier piercing ability, so we moved

the Phantom to alternate fire and added her teleport as

new shift ability. Regarding the ultimate ability, our first version was that Zelda would

surround her team but not herself with barriers, and the barriers would negate

all damage for 2 or 3 seconds, but if Zelda died it ended early.

However, this would create a similar playstyle to that of the old Mercy, that

Zelda had to hide, and even after we limited the range of the ultimate ability, we didn't like it

and eventually changed it to what it is now. What took the longest was to decide what

benefit the Phantom should give her. We tested many different things, like

that the Phantom's hits would create a 1 second

burst healing from her fairies, the hits would increase her damage or per hit she would

gain a special arrow that dealt more damage or had other special properties,

but in the end we settled for lowering the healing of her fairies and

creating a mechanic that would gradually increase their healing rate by

dealing damage. So, the creation process was a lot of fun and challenged our

creativity as game developers to come up with the best possible design. She

is available for testing on our PTR right now, which is our Public Test

Region, so have fun testing her and don't forget to give us feedback about what

you think of her. If she seems to be too strong or too weak in certain aspects,

we are ready to make adjustments as quickly as possible to make sure that

she will be balanced for the live release.

I was totally kidding by the way,

she will be released as she is now, so that was just me being silly. We are also

working on a brand-new map, Hyrule Castle Town.

It's another great Assault map. And we really felt like the game needed another

Assault map. But we are seeing some issues with the Assault mode. We've

noticed that an inordinate number of these matches are going into overtime

with never-ending stalls and this is resulting in very long match times.

When we look at Control, Hybrid, and Escort compared to Assault we were really

surprised to see that Assault was sort of an outlier in terms of how long

matches were taking. To solve this for Season 10, the Assault maps will

have an additional 5 minutes per point to give the attackers a chance to break

the stall. Also, for all new Assault maps, just like on the Horizon Lunar Colony

map, we will put the defender's spawn with the area where they can't be damaged at

all right next to the point to compensate for the extra time the attackers get.

In addition, we still feel like the Control mode takes too long, and since

nobody likes that mode anyway we will further reduce the match time to a best

out of one, so, whoever wins the very first round will win the match. You have

to know that, even though statistics and the community feel one way about

something, if we as game developers disagree with it we will still do it.

It's all about our feelings as professional bronze players. Regarding

the Hanzo rework that we mentioned first a year ago, his rework as Link from

Skyward Sword is almost ready to be put on the PTR. That's all for now, have fun

Simons: A big herd of walkers shows up outside.

They invade the building.

There's a big shootout here on the dance floor.

They start fighting together in the beginning.

[Gunfire]

Man: Action!

Simons: We had stabbing going on.

We had shootouts going on.

We had over a hundred extras in here.

I had a bunch of stunt walkers.

[Grunts]

[Walkers growling]

Sometimes we have to interpret the stage directions

because once you're in the situation,

things change and things are different.

And you don't want those jumping moments.

So Monty went,

"Let's just get some pads and let's work it out."

And that's the way we got it done.

[Walkers growling]

Simons: Lennie comes in.

He's looking around this very dark space.

He sees the ghost of Henry,

and Jared takes that little beat...

[Grunts]

We obviously can't hit Lennie

with a real pool cue,

so we had balsa wood,

which weighs almost nothing.

So it'll splinter like a real stick,

but it won't hurt him at all.

[Grunts]

And that way, we can pull off

a pretty solid hit to the back.

[Both grunting]

Simons: He's trying to stab Morgan

in the face with this broken, sharp pool cue.

What we're gonna do is have Jared jump up

and put his weight behind it,

trying to drive it down.

That's gonna give Morgan that beat that he needs

to get a leg up underneath him.

[Grunting continues]

And just do a donkey kick, sending him flying out the door.

[Grunting]

He gets that fantastic moment of, "What are you doing?

Oh, no, really?"

In the scene directions,

it says that Morgan is kind of passive.

That was the hardest acting job I think I've ever done...

...not to be just smiling as he was eaten.

[Screaming]

We have a cheek prosthetic

that's got a blood tube fed through it.

It had a piece on the top that they can rip off.

Then it has this under piece

that has teeth marks in it and is porous.

So it'll start pumping blood out the wound.

[Screaming]

The myriad of terrible things

that I've done warrants a death like this.

[Sobbing] Let go! Let go!

[Screaming]

[Groaning]

It was fantastic.

Part of the job of Fuches is

to take some of the culpability away from Barry,

that it's not that Barry decided that he wanted to kill people.

Fuches is the one who has kind of

steered him down that road.

The funny thing, too, about Fuches is that

in the pilot, initially, he was a totally different character.

He was one-dimensionally evil.

But it was decided, you know, that that was kind of a bummer.

And it was more interesting if A, it was a bummer.

B, it was Stephen Root, and he had nothing to do.

-Well he had nowhere to go. -He had nowhere to go.

It was just, like, he was on ten, like, meanness-wise

from the beginning.

He genuinely cares about Barry. He just...

like all the people you care about,

you manipulate them, right, Alec?

-Am I the only one? -(BOTH LAUGHING)

~~BILL HADER:~~ ~~I think Barry thinks he can do both things sometimes because~~

killing is something that he's so good at,

It's sort of like drunk-dialing in a way,

he's just sort of like,

"You know what, I just want it, so I'm gonna do it."

Then I think, you know, he's literally

waking up the next morning sometimes thinking,

"Okay, that may have been a mistake."

We landed on this idea early on,

that LA is a town of people who are from somewhere else.

And we like the idea that everybody, on both sides,

are kind of like Barry in a way,

where they're trying to better themselves.

Goran and his guys are kind of new in town,

and they're trying to put their best foot forward,

and look like they know what they're doing.
