I am chief medical author of Road to USMLE step 2 Clinical skills book.

In this book you will find a vital checklist of how to extract good history from the patient.

We already discussed about the pain, now we are going to discuss about non pain symptoms.

Let�s say a patient has dizziness.

How are you going to extract a history of dizziness?

You are going to erase this portion of location and grade, because in dizziness you will not

have intensity.

So let�s dive in.

D as in Duration.

How long has this dizziness been going on?

And if this dizziness last, how long does it last to you?

O - as in onset.

Is it sudden onset or gradual onset?

So in correctness of the dizziness you have to let the patient speak about the story of

dizziness.

They will have different stories all the time.

T - as in time of day.

Very important to ask if this dizziness happened at night time or in the morning unless the

patient is taking blood pressure medication and got up in the middle of the night and

got dizzy.

It is very important to extract that history.

O- as in occasion.

What were the doing when they had these dizzy spells?

�Doc, I was getting up in the middle of the night to go to the bathroom.� R - as

in here, you don�t have to ask radiation, you ask for �rate of progression�.

Do you think this is progressing?

Do you think this dizziness is getting worse?

A - As in associated features.

I - as in �increasing factors.� D - as in �decreasing factors,� and previous

history of dizziness and what medications have you taken for dizziness?

Welcome to our second lifestyle video

This week we have five snack ideas

that are healthy

and easy to take on the go

what we've considered is that

they have a short preparation time

less than five minutes

that they are easy to take with you

in the sense that they are not smelly

and that they are compact

and lastly that they are healthy for you

so its very important to know

what the right serving size is for the food that we eat

Let's go see the list

Dried fruits: about a handful

Berries: about half a cup

Yogurt: 200 gr. to 250 gr.

Veggie sticks: countless

Crackers: about 10

I hope these ideas help you with some healthy snacking

I like to use glass containers I keep from marmalade and other things

Today we are at Emirgan Park

Last week we were in historical peninsula

But for today we have a different plan for you

April means tulip festival

Because of that today we are in Emirgan Park

I found a silent corner in Emirgan Park and I sat down

Let me give you some information about the history of tulip

As you know our tulip subject is a sad story for all of us

This adventure starts in 16th century when Kanuni Sultan Süleyman

decided to give tulip bulp to Holland

And now sadly when you menton tulip you think of Holland

Despite that in here you can see all sorts of tulip

which I haven't seen before

By the way, Emirgan Park has three different mansion

Yellow mansion, white mansion and pink mansion

There are also cafes and restaurants

But if you say I prefer a different place

You can also have a picnic on your houndstooth cover

With your filter coffee, club sandwich and your book

and you can mesmerize people with your lovely instagram photos

I am kidding

I mean you can have a joyful time

It is also a great place to come with your family

Don't worry for your child

This place has a huge playground

You both can enjoy your time

I will say something

There is also one other mansion I forgot to tell you about

Can you see the mansion over there in turquois colour, I loved it

It is for the squirrel

There are many in here

We even see them when they got into their houses

Don't make me angry, it is just adorable

In a view like this I always get inspired

I had a notebook and pen with me

After a long trip in Emirgan Park

We decided to come to Bebek Kahvesi

As everyone know it is the most popular place in Bebek

Near the sea

You can have a nice breakfast

You can drink your coffee, tea

It is really lovely

I get a bit cold so I will drink some tea

There are girls with sportwear

But they are not running

They just put snaps

I live in the anatolian side

Because of that I don't know much about Bebek

But today I had a chance to look around

We will look around together as well

There are many cafes, boutiques and restaurants

And a great night life

Now we are in front of Bebek Park

And it has a great view

People come here to sit, to walk, to rest

and to spend some time with their children

There is also a playground over there

There are many boats in here

It is really lovely, I feel like I am on vocation

We saw a little boutique called Love It

It has great things

And the owner of the boutique is in front of me

Well I love to work on leather

Because of that my leather pieces are in demand

and love to guide them accordingly

I only design things I love

As you can understand from the name of the store

You can see all the pieces I love

In this season I used orange, blue

in the middle season grey

not the summer colours

The trend in this season is

double breasted dresses

So mainly I used them

What is your most wanted piece in this season?

In summer because it is hot I sell mostly silk dresses

and this type of dress

It is really beautiful

This dress is great for summer

Great for tanned skin

and can be combined with gold jewellery

When did you open this store?

In September 2013

At first

I was working on my own collection

Then I did import

I brought imported product by 70%

In 30%, I worked on silk dresses in summer

Leathers in winter, I worked on my own collection

I also design personal night clothes

So it is mostly your design

Chronically Ill and Homebound = "Not Living"? [CC] - Duration: 6:58.

Hello, hello!

I'm a fan of Amandla Stenberg, so when they posted the trailer for their new movie Everything Everything,

I clicked.

As the trailer started my heart skipped a beat, I knew nothing about this story other

than this trailer was introducing me to a character who was chronically ill and a young

person of color and I got so pumped!

And then the trailer went on...

MADDIE: Dear Mom,

I know staying in this house keeps me alive

but this isn't LIVING.

Uh, what?

Risk Everything

No, Ollie! I have to know if I'm really sick!

What!

"From the studio that brought you Me Before You…"

Ohhhh.

I went to the internet to find out what the dang book was about and in case the trailer

didn't spell it out for you, she's not really sick.

It's inspiration porn with no intentions of being authentic to our narratives, just

to please and inspire the able audience.

It's not even original!

It's Bubble Boy only dramatic and with a splash of diversity!

These stories are told repeatedly, Stephen King's IT, The Sixth Sense with MSBP specifically

and don't even get me started on how many times we see sick/disabled people

not really being so or faking it.

This is not representation, this is harmful erasure.

What are you telling disabled people and specifically disabled youth when you consistently produce

media that says living with a disability is "not really living"?

When one of the only times sick/disabled youth get to see themselves on the big screen and

it becomes a horror film about manipulative parents holding them back?

Chronically ill people have ACTUAL stories that are never told in mass media because

people in production think disability pride is too confusing for non-disabled people to comprehend.

You can't be content as a sick and/or disabled person, what the heck does that even look like?

You'll never know unless you let us show you!

People do not need to be healthy in order to be happy and have fulfilling lives.

Some people are sick and disabled from the day they are born to the day that they die

and you're trying to tell people that they're never gonna be happy because of that?

That they're not "really living" because of that?

Where are our real stories?

Where is the story that happens all too often where a young person realizes they are sick

before anyone else and is gaslit and called an attention seeker and has to fight for years

to be acknowledged, and when they finally get their diagnosis it's a sigh of relief because

they finally know what's going on and can receive proper medical care.

What about their struggles economically and socially getting those needs met?

Where are our stories of confronting ableism and internalized ableism and conquering that?

No, instead you'd rather terrify disabled youth by leading them to think that their

illness might actually be parental abuse because they may not get to do the same things their

peers get to do.

You'd rather encourage them to "risk everything for love".

And I actually think a conversation on "risk" while being chronically ill is valid, um,

as a chronically ill person there actually have to be measured risks all the time.

Every life comes with certain levels of risk, we all take risks, but suicide missions for

the sake of a crush?

What?

And internalized ableism IS a thing as well, but it's always written by able folks who

just end up reinforcing it, reinforcing the idea that disability is a bad thing.

Ultimately, it's obvious that Hollywood really doesn't give a damn about people

with disabilities, they don't wanna hire them and they definitely don't want to

authentically represent them.

We're constantly at the bottom of the radar, politically, socially, and Hollywood is no

different and honestly it was just so cruel to be fooled into thinking just for a second that

we might actually get a positive and diverse representation of disability, a positive narrative

for disabled and possibly homebound youth of color, think of the comfort a character

like this could bring to other youth in the same position.

I really don't care if you see it or not. [SIGH]

I just want people to know... and be vocal about how this is not

good disability representation, I want people to understand how this negatively impacts the

disabled community, and specifically disabled youth.

Please share and uplift the works and the narratives of disabled people, please listen when members of the

disability community let you know what harms them.

It's time to start caring about what happens to us.

Thato's Journey in Faith

My name is Thato Mogkotho, I'm from Limpopo in South Africa,

and I'm the second of three girls.

I grew up going to many Protestant churches

where I was taught to detest the Catholic faith as not being Christian.

I never imagined that I would be here one day in Rome.

Getting here has been a long journey

and my friends in Opus Dei have helped me find the answers to many of my questions

about Catholicism, and to shake up some of my Protestant beliefs.

Emotionally it has not been easy to realize that I didn't have the full faith.

One of the most humbling experiences in this journey was about our Holy Mother and the Eucharist.

Holy Communion has always been just a symbol for me,

but not the actual body of Christ.

I am now looking forward to receiving the Body and Blood of Christ for the first time,

and to be welcomed into the universal Church where I'll receive the fullness of my faith through baptism.

Becoming Catholic in my world is not going to be easy

as many of my family and friends don't like Catholicism,

and I'll be the first one of them.

And my question is...

How can I strengthen my faith and also help others in my world to know Christ?

When we receive Our Lord in communion, what happens is not exactly the same as when we take in food,

in the sense that we assimilate, we somehow make what we have received part of ourselves.

In communion, the opposite happens:

We are the ones being transformed,

transformed into Jesus Christ.

Although in its most substantial aspect, communion only lasts a few minutes,

ten minutes for example, until the physical appearances of bread disappear,

nevertheless, God stays with us

Jesus Christ Himself stays with us in some way.

All the strength and ability to be strong in a hostile environment,

to be able to give faithful witness of what we believe and what we are

all of that comes to us from the Eucharist.

I think it first starts by the daughters and sons of Christ, to actually live out your faith

not just speak it or try to convert people through talking

but through actually being a light of the world and living out your faith,

and what Jesus teaches in the Bible.

It's overwhelming!

Because in the beginning of the whole journey and studying the catechism,

we actually had to go through the history of the church,

and coming here and seeing everything physically is just different,

It's going to be stuck in my memory, because I can see the picture

and not just my own imagination!

So it's a real blessing to be here

and to actually see the whole succession of the Church and the history,

and where it is currently.

I feel very blessed.

Today I'll be looking back at Chip and Dale..

Today I'll be looking back at Rescue Rangers.

Chip and Dale Rescue Rangers was an afternoon cartoon turned into a sidescrolling Nintendo

game in 1990 and recently landed on modern platforms through the Disney Afternoon Collection.

You make your way through different levels picking things up and throwing them at enemies,

while crossing obstacles and avoiding hazards.

Most of the 11 areas end with a boss, which usually involves throwing the ball straight up

Acorns refill hearts.

While collecting flowers and silver stars trigger a gold star to appear.

One button jumps while the other buttons is used to pick up and throw.

You can also duck and hide in a box, which is a great way to take out enemies.

And teaches children how to be cowards.

Dale is confined to the second controller during co-op, who you can just pick up and

carry if they're too dumb.

2-player is the most defining feature of Chip and Dale Rescue Rangers.

It's just challenging enough to warrant focus and communication, but never too frustrating.

If one person dies they have a grace period where they float around and the other player

can run forward.

And throwing boxes at each results in brain damage and temporary loss of motion.

The soundtrack is great and the environments have lots of variety, but it's weird how they

went with the branching paths, because why would anyone visit level E? or go through

AC?

They're really just optional.

Overall, Chip and Dales Rescue Rangers is staple of Nintendo era couch co op.

It's has a great balance of length, features, and difficulty for a wide range of people

to enjoy, and Disney Afternoon collection has a rewind feature that makes the game even

more accessible.

Let�s say a patient has dizziness.

How are you going to extract a history of dizziness?

You are going to erase this portion of location and grade, because in dizziness you will not

have intensity.

So let�s dive in.

D as in Duration.

How long has this dizziness been going on?

And if this dizziness last, how long does it last to you?

O - as in onset.

Is it sudden onset or gradual onset?

So in correctness of the dizziness you have to let the patient speak about the story of

dizziness.

They will have different stories all the time.

T - as in time of day.

Very important to ask if this dizziness happened at night time or in the morning unless the

patient is taking blood pressure medication and got up in the middle of the night and

got dizzy.

It is very important to extract that history.

O- as in occasion.

What were the doing when they had these dizzy spells?

�Doc, I was getting up in the middle of the night to go to the bathroom.� R - as

in here, you don�t have to ask radiation, you ask for �rate of progression�.

Do you think this is progressing?

Do you think this dizziness is getting worse?

A - As in associated features.

I - as in �increasing factors.� D - as in �decreasing factors,� and previous

history of dizziness and what medications have you taken for dizziness?

Donald Trump will probably stay in Paris climate deal, likely won�t reverse Obama�s Iran

President Donald Trump seems ready to break one of his big campaign promises, even as

he wavers on whether he should do likewise with another one.

While Trump�s chief strategist Steve Bannon is urging the president to keep his campaign

promise about pulling America out of the Paris climate accord of 2015, the president is surrounded

by more influential advisers who are urging him to stay in, according to a report by the

Associated Press and The New York Times.

These include Secretary of State Rex Tillerson, who is a former chief executive of ExxonMobil

and whose voice has been reinforced by that of many other major corporations.

Similarly, Trump�s daughter Ivanka and son-in-law Jared Kushner have also pushed for him to

not leave the Paris climate accords.

Although staying in the Paris climate accords would avoid reversing a historic commitment

to fighting man-made climate change, as well as stave off potentially seriously diplomatic

ramifications, this doesn�t mean Trump will suddenly be pro-environment.

George David Banks, a senior White House climate policy adviser, has urged Trump to remain

in the Paris climate accords but substitute Obama�s anti-climate change plan with one

that is more friendly toward large corporations.

Things are a little more tense when it comes to whether Trump will fulfill his pledge to

get rid of the Iran nuclear weapons deal that was worked out under the administration of

President Barack Obama.

While Tillerson acknowledges that Iran is abiding by the terms of the deal, the Trump

administration is still considering violating the deal by reapplying sanctions due to Iran�s

continued support for terrorism, according to a report by CNN.

�Iran remains a leading state sponsor of terror through many platforms and methods,�

Tillerson wrote in a letter to Congress.

�President Donald J. Trump has directed a National Security Council-led interagency

review of the Joint Comprehensive Plan of Action that will evaluate whether suspension

of sanctions related to Iran pursuant to the JCPOA is vital to the national security interests

of the United States.�

Dallas, Texas August 22, 1984

Protestors marched through the streets, destroyed property

spray-painted walls, broke windows

and threw dirty diapers and beer cans just outside the Republican National Convention

Someone stole an American flag from a flagpole

from a downtown building

Eventually that flag ended up in the hands of Revolutionary Communist

Youth Brigade member Gregory Lee Johnson

At the height of the protests,

ohnson poured kerosene on the flag and set it on fire.

While the flag burned, he chanted stuff like

"Red, white and blue, we spit on you, you stand for plunder, you will go under"

and "Reagan, Mondale, which will it be? Either one means World War III."

Although no one got physically hurt because of it,

some people who saw Johnson do this were pretty offended by it.

Soon after, police arrested him and charged him with violating a Texas law that said you can't vandalize respected objects,

if such action were likely to get people mad.

A Texas court convicted Johnson and he was sentenced to one year in person and fined $2,000.

He appealed his case to the Fifth Court of Appeals of Texas, but lost the appeal.

He appealed again to the Texas Court of Criminal Appeals

and they actually overturned his conviction, saying the First Amendment to the Constitution protects flag burning as symbolic speech.

It also argued that Johnson did not hurt or threaten anyone by burning the flag.

Texas was like nuh-uh, and it asked the Supreme Court to hear the case.

The Supreme Court agreed, and they heard arguments on March 21, 1989.

At first, the Court considered if the First Amendment protected non-speech acts

since this wasn't about Johnson's verbal communication.

So basically, they wondered if the act of burning the American flag should be considered expressive conduct.

Ultimately, they determined it was.

In a highly controversial decision, the Court ruled 5-4 in favor of Johnson.

The Court said that Texas could not ban flag burning

They also argued that Texas' law that said you can't vandalize respected objects didn't prevent disturbing the peace.

n fact, another Texas law already existed to prevent disturbing the peace without targeting flag burning.

Justice William Brennan delivered the opinion, but I am going to quote Justice Anthony Kennedy, as he put it pretty well.

He said, "The hard fact is that sometimes we must make decisions we do not like. We make them because they are right, right in the sense that the law and the Constitution, as we see them, compel the result.

And so great is our commitment to the process that, except in the rare case, we do not pause to express distaste for the result, perhaps for fear of undermining a valued principle that dictates the decision. This is one of those rare cases."

The court had two major dissents.

The first, by Justices William Rehnquist, Byron White, and Sandra Day O'Connor, argued that the American flag had a "unique status" that should be protected from desecration.

Rehnquist wrote, "The flag is not simply another "idea" or "point of view" competing for recognition in the marketplace of ideas.

Millions and millions of Americans regard it with an almost mystical reverence regardless of what sort of social, political, or philosophical beliefs they may have."

Justice John Paul Stevens had a slightly different dissent.

He argued it was more than about the flag being an important symbol.

He argued that Johnson was only punished for how he expressed his opinion, not the opinion itself.

So basically, the decision automatically made laws in 48 of the 50 states invalid.

However, just two weeks later, Congress passed the Flag Protection Act, which made it a federal crime to desecrate the American flag, kind of like a middle finger to the Supreme Court.

But the Court had the last laugh.

The same five person majority of justices struck down the law in the 1990 case United States v. Eichman.

Since then, many in Congress have tried several times to pass an amendment outlawing flag burning, but each time they come up short.

Texas v. Johnson, in many ways, started the American flag burning debate,

which continues to this day.

It has remained important to those who value true freedom of speech, even if that speech is offensive to the majority of people.

Hi, I'm Jonathan Bird and welcome to my world!

One of my favorite places to dive is Eastport, Maine. The water is cold, but the bottom is

covered in an incredible amount of colorful marine life.

Eastport is near the Canadian border, at the mouth of the Bay of Fundy.

The huge tidal range in Eastport means that the water is almost always flowing one way

or the other as the tide comes in or goes out. The moving water full of plankton feeds

a ton of invertebrates, making the bottom lush.

It gives me plenty of things to film on every dive.

There is pink soft coral feeding on the plankton.

Anemones cling to everything, even an old drain pipe.

The lobsters reach epic proportions!

And fish thrive, like this camouflaged Sea Raven, blending in with the bottom.

A shrimp rests on a sponge.

And a whelk, a kind of large snail, searches for a meal.

Unfortunately, the whelk is about to become dinner for my favorite animal, the Atlantic

Wolffish.

Wolffish might have big teeth, but these fish are no threat to people. They like to eat

whelks and crabs.

Wolffish often get together in pairs during the summer to lay their eggs. They hang out

in the same den under a rock.

After the female lays around 10,000 eggs, the male kicks her out and guards the eggs

for several months until they hatch. Here you can see the egg mass behind the male,

way inside his den.

Over the years I have become friendly with a wolffish I call Gene. He can't resist

a whelk or two when I bring them right to his front door. It took me a couple of years

to get him comfortable enough with me to come all the way out of his den. Even then, he's

pretty skittish.

I've heard about Pacific wolffish that are bigger and friendlier, and I want to go meet

them, so I'll have to travel from Maine, all the way to British Columbia.

My journey takes me from Eastport to Vancouver.

Next I have to drive out to Port Hardy, where my dive boat awaits.

Well, four hours to Port Hardy on the beautiful roads of Vancouver Island.

In the drizzling rain, I board the Mamro, my dive boat for the week.

Soon, under a clearing sky, we're underway. Captain Dan drives the boat past gorgeous

forest wilderness, on our way to Clam Cove, where we will anchor the boat for a few days

in search of Pacific Wolfeels.

Pacific wolfeel is the local name for this impressive fish. But it's not an eel at

all. It's actually a fish, just like the Atlantic wolffish, so should be called a Pacific

wolf fish.

Whatever you want to call it, I am going to find one. We depart aboard the skiff for our

first dive.

I start suiting up in anticipation. The water is 45 degrees, but my drysuit will keep me

warm.

And now I'm ready! Voila! Ready to dive...the cold Pacific Northwest.

My cold water dive gear is bulky and cumbersome, but I'm used to it.

Lets go check it out!

Descending through the kelp, I find a world as rich in marine life as Eastport, driven

by similar tidal action and nutrients.

I swim over the edge of the wall, to search for a Pacific Wolfeel.

There! Sticking just its head out from its den, a fully-grown male Wolfeel stares at

my camera.

Nearby, a younger one, with darker coloration, also curious what I'm up to, but not about

to come out.

Both have prominent teeth, just like the Atlantic Wolffish, but they appear just as docile.

Soon, I turn back to observe the big male, and he starts coming right at me! He's coming

out of his den. He's a lot longer than an Atlantic Wolffish.

I'm not sure what he's up to.

He comes up to closely investigate my lens. Maybe he sees his reflection.

He lets me hold him in my hand. They told me the Pacific ones were friendly, but this

is incredible! I'm beginning to think maybe this fish has been fed quite a bit, and he's

looking for a handout. But I didn't bring him any food!

He tries my arm to see how it tastes, but dry suits apparently aren't very good eating.

Before long, he gets the idea that I haven't got anything to eat, so he goes over to see

my dive buddy Tim, who just happens to have brought a snack.

Now I would have bet that there is no way a Wolfeel would eat a sausage. But as it turns

out, they love sausages. Who'da thunk?

After he got his snack, the wolfeel heads back to his den, and then I could see why

they call them wolfeels. This fish has a body 6 feet long and it's more eel-like than

fish.

So why do we call the Atlantic ones fish and the Pacific ones eels? Good question, but

they are both fish, and probably one of the most interesting fish to interact with in

the entire world. No matter what you call them, they're cool!

that students, medical students were learning the clinical cases.

They were learning if you had - Cholecystitis, Cholangitis, whether you have diverticulitis,

diverticulosis.

They were cramming up the cases.

I don�t want you to cram up the cases.

I want you the understand the concept.

Let me give you an example.

Abdominal pain.

How will you approach abdominal pain?

You can have any clinical scenario, but the approach remains the same and you will nail

the diagnosis every time all the time.

So, let's discuss when you are extracting the history from the patient, always remember

(AWD - All Wheel Drive) (FWD - Front Wheel Drive) - (RWD - Rear Wheel Drive) So I am

going to make a little car and I am going to make little cars like this so you can imagine

and remember the history.

A as in abdominal pain, A as in Appetite.

W as in weight gain or weight loss.

D as in dysphasia do you have swallowing issues.

Front wheel drive.

Front means mouth right?

Nassau, vomiting.

You are going to ask the history of nausea and vomiting.

Whether you have any bleeding vomiting, blood in the vomiter.

So that is how you are going to ask the history of hematemesis.

Rear wheel drive means what?

Diarrhea or you are having some salmonella, are you having some hematemesis so this is

how you are going to extract the history and ask the associated parts in the abdominal

pain.

Very important to be crisp and precise.

very important for your differential diagnosis in Step 2 Clinical exam.

Don�t cram just understand the concepts.
